:Introduction

Multidisciplinary care when professionals from a range of

disciplines work together to deliver comprehensive care that

addresses as many of the patient's needs as possible. This

can be delivered by a range of professionals functioning as a

team under one organizational umbrella or by professionals

from a range of organizations, including private practice,

brought together as a unique team.

Definition of team:

a group of people with complementary skills who are committed to a common purpose, performance goals, and approach, for which they hold themselves mutually accountable.”

Multidisciplinary team: in the context of health care, means that health care providers from different professions work together to collaboratively provide diagnoses, assessments and treatment, within their scope of practice and areas of competence.

decisions are made based on reviews of clinical

documentation such as case notes, test results, diagnostic
