• Go to www.msqh.com.my

• Click Application for Hospital Survey

• Fill in the form

• Submit to MSQH

• Your application will be review by MSQH

REGISTER FOR ACCESS

1. The system will automatically send an email to user after MSQH create the survey

2. Click Register

SIGN UP HOSPITAL ADMIN

• Fill in the form

• Click Sign up

• MSQH will received Email notification for approval

SIGN UP HOSPITAL ADMIN

• After approved by MSQH, hospital admin will received Email Notification.

• Click

HOSPITAL ADMIN ROLE

• Hospital Admin allowed to:

1. Create new user for Head of Department and Respondent

2. Activate User

3. Disable User

4. Change/Reset user password

• Click user list of user

• Click

User’s Role in eHAP 5th Edition

HOD

•Endorsement/overall responsibility for the service standard

•HOD role can be assign to HOD and Respondent

Hospital Admin

•Overall responsibility for the pre survey assessment (assigning, amendments, submission)

•Hospital admin cannot be HOD or Respondent

Respondent

•Key in the information

•Respondent role only can be assign to respondent

CREATE USER FOR HOD & RESPONDENT1. Enter user’s name

2. Enter user’s email address

3. Choose user role :

Head of Department / Head of Services• HOD role can be assign to HOD & Respondent

Respondent• Respondent role only can be assign to respondent

4. Click Save

5. System will automatically send the notification email to user and contains details of username and password

6. User will received email from [email protected]

Note:If user do not received a password, hospital admin can reset the user password by click the edit button and change user password

CREATE USER FOR HOD & RESPONDENT1. Hospital Admin activate the user.2. Click Activate under actions column3. HOD/Respondent will automatically received Email notification to login

IDENTIFY AND SELECT SERVICE STANDARDS

2.Click Edit

ASSIGN HEAD OF DEPARTMENT AND RESPONDENT


SELECT HEAD OF DEPARTMENTHead of Department can be select in Head of Department and Respondent

SELECT RESPONDENTRespondent can be select from user which hold HEAD OF DEPARTMENT role or Respondent role

SELF-RATING BY RESPONDENT AND HEAD OF DEPARTMENT


SELF RATING

1. Self-rating must be enter by Respondent2. Head of Department at the same time can see what is entering by respondent3. Click Hospital Survey4. The list of service standard that assigned to you will appear

1. Click on criterion2. Enter the rating in rating Items

3 Self Rating will automatically calculate4. Go to next criteria

HOSPITAL COMMENTSAll rating of 2, 1 & NA must be commented in the Facility Comment section

RESPONDENT SUBMIT SURVEY TO HEAD OF DEPARTMENT

• Step for Respondent to submit survey to Head of Department as follow:1. All criteria has been rating2. Self Rating must be 3 or 43. Go to Dashboard and click “Respondent self-rating

less than 3” to view.

If you have rating less than 3, the pre survey assessment cannot be submit to Hospital Admin

RESPONDENT SUBMIT SURVEY TO HEAD OF DEPARTMENT

1. Click2. Click ok3. The Survey successfully submit to head of Department

H.O.D SUBMIT SURVEY TO HOSPITAL ADMIN

• Step for head of department to submit the survey to Hospital Admin as follow:

1. All criteria has been rating2. Self Rating must be 3 or 43. Go to Dashboard and click “respondent self-rating less than 3” to

view. If you have rating less than 3, the pre survey assessment cannot be submit to

Hospital Admin

4. After successfully submit to Hospital Admin the Status column change to Hospital Admin Review

5. Once submitted the data became read only and no changes can’t be made

H.O.D SUBMIT SURVEY TO HOSPITAL ADMIN

1. Click Submit to Hospital Admin under Actions2. Click ok3. The Survey successfully submit to hospital admin4. The status change to Hospital Admin Review

HOSPITAL ADMIN SUBMIT TO MSQH• Step for Hospital Admin to submit the survey to MSQH as follow:

1. All criteria has been rating2. Self Rating must be 3 or 43. All Head of Department endorsed or verify the standards

(Submit to Hospital Admin) The status = Hospital Admin Review

4. After successfully submit to MSQH the Status column change to MSQH Review

5. Once submitted the data became read only and no changes can’t be made

HOSPITAL ADMIN SUBMIT TO MSQH1. Click Submit to MSQH2. Click ok3. The Survey successfully submit to hospital admin4. The status change to MSQH Review5. No changes can’t be made

TECHNICAL COMMENTS


TECHNICAL COMMENTS

• MSQH will review the Pre Survey Assessment (PSA)

• Technical comments will appear after MSQH review the PSA

• The hospital admin will received an Email/Letter for any necessary amendments on the PSA

TECHNICAL COMMENTS

• MSQH Submit Survey to Hospital Admin

• Hospital Admin click on Dashboard

• Click View Details on Technical Comments Submitted box

TECHNICAL COMMENTS

• The list of Technical Comments will appear

• Click on Criterion No and make an amendments
