Case # 1

Juan is a 47 year old who is an aspiring artist. He has

been drinking two 40‟s every night for the past few

months. He complains that he and his girlfriend used

to drink together but now that she quit she, “gets on

him” about his drinking. He tells you that he‟s tried to

cut back on his drinking before but that he always

starts to drink when one of his paintings gets passed

over for an online art gallery.

– Double-Sided Reflection

Client: It really helped to have a smoke after found out I didn‟t get the job…. But then I started coughing again and my wife gave me the “death stare”.

Clinician: So on one hand you felt that smoking helped you cope with your disappointing news, and at the same time you noticed you were coughing again and your wife wasn‟t happy about that…

Client: Yeah… I mean, the cough ain‟t that bad… but my wife is worried.

Clinician: She‟s worried about you.

Page 35: Motivational Interviewing - Hofstra University · Motivational Interviewing Collaboration Evocation Autonomy Spirit Express Empathy Develop Discrepancy Roll with Resistance Support

Client : Look, I’m gonna die sometime, might as well enjoy myself!

Confrontation: Yeah well you are gonna die much quicker with that attitude.

Reflection: You feel like sometimes its not worth it to make the effort and that maybe you should just have a good time.

– “You come here every month! This is hard work you‟re doing”

– “ I saw you in the waiting room reading to your kids. Its clear that you really care a lot about your family”

– “Your frustration is understandable, you have been trying to work on this for some time.”

– “It must have taken a lot of courage to come in today knowing you haven‟t met your weight loss goals”

Affirmation Examples

www.jonathanfader.com

Page 39: Motivational Interviewing - Hofstra University · Motivational Interviewing Collaboration Evocation Autonomy Spirit Express Empathy Develop Discrepancy Roll with Resistance Support

Write down 1 affirmation for the following statement: “I'm sick of this, everything just keeps getting messed up. I do good for a week and then my kids start getting on my nerves, talking back and crap. I've worked my whole life for them and they don’t appreciate me a bit! I go to my program. I think it's helping and then I relapse and my girlfriend is always nagging me about my meds. No one at this place likes me. You don’t know what its like to have the problems I have. Everyone just judges until they have a problem and then they get it. I just wish everyone would leave me alone!

Practicing Affirmations

www.jonathanfader.com

Page 40: Motivational Interviewing - Hofstra University · Motivational Interviewing Collaboration Evocation Autonomy Spirit Express Empathy Develop Discrepancy Roll with Resistance Support

• Collect material already offered

– So far you‟ve expressed concern about your family, getting a job, and staying clean…

– What else?

• Link something just said with something discussed earlier

– That sounds a bit like what you told me about that lonely feeling you get when no one comes to visit.

• Transition: Draw together what has happened and transition to a new task

– Let me summarize what you‟ve told me so far. You came in because you were …, and it scared you when . . . Then you mentioned… and now…

– …Where does that leave you?
