• OHRS/MIS Trial Balance Submission Due October 31, 2014

• Q2 CAT SRI submission to the LHINs – via SRI Due November 7, 2014 Remember: New Budgets effective April 1/14 – new

budgets from MSAA 2014/17 will be populated into the SRI file- please review for accuracy.

Use Budget Adjusted Column for any new funding. CAT SRI small cosmetic changes to prior years. A new CAT SRI User Guide is being developed and

should be out soon – changes are cosmetic too.

Advancement of Community Practice

The Advancement of Community Practice initiative was developed by the MH LHIN in response to feedback obtained during the Integrated Health Service Plan (IHSP) engagement sessions. It addresses the following: • Creates an environment of continued learning

• Provides an environment for operationalizing “Best Practice” and “Ongoing Sustainability of Practice”

• Provides a mechanism for centralizing discussion, resources and decisions for high quality service delivery

• Improving the health of those living in the MH LHIN community by enhancing the competency of staff and caregivers

Regional Learning Centre

• Was designed to strengthen the skills and abilities of direct care providers in our communities through education, development and focused learning.

• The transfer of new knowledge and best practice skills will enable direct care providers to achieve increased levels of expertise and standards of care provision.

• The new centre brings educators, knowledge, and best practice together to support practitioners in the Mississauga Halton LHIN community.

Regional Learning Centre

• During the first year of the program at the Regional Education Centre, four community educators will provide educational opportunities for front-line staff and supervisors in a variety of topic areas that include: • Hospice Palliative Care • interRAI Training/Refresh (assessment skills) • Enhanced Personal Support Worker, and • Health Equity

Regional Learning Centre

• Community Educators are also available on a mobile basis with access to IT services and mobile projectors enabling educators to come to the practitioners’ environment if they are unable to attend the learning centre.

• The Regional Learning Centre is a key objective under the Advancement of Community Practice (ACP) initiative.

Background Adult Day Services (ADS) provide a responsive

support to clients and families living in the community to help them maximize their level of functioning

Through partnerships ADS offer a variety of

services/programs to support both client and caregivers in the community

MH LHIN ADS Committee evolved as an innovative framework to drive continuous quality improvement within the MH LHIN community

Purpose

To identify opportunities to enhance Adult Day Services (ADS)

To establish and implement the vision for LHIN-wide Adult Day Services.

To work with providers and caregivers to inform the MH LHIN ADS vision

To advise and make recommendations to the Caregiver First Collaborative

Target Population

Individuals in the community requiring support to remain living within their home environment, maximizing optimal level of

functioning

Committee Membership

Membership includes representation from

consumers and leadership from a variety of health service provider and caregiver

representation.

Role of the Committee Support the development and operation of an integrated Adult Day Services Identify opportunities to optimize and integrate existing resources and capacity within the system Provide input/suggestions and recommendations (short and long-term) for existing Adult Day Service initiatives
