1) Purchase: Any of the following Minwax® products between 10/24/2016 – 11/11/2016 in a single purchase at a local homecenter or retailer.Please complete the chart below to determine the actual amount of rebate check.

2) Send: (A) This completed certificate, (B) the original store identified cash register receipt (dated between 10/24/2016–11/11/2016) with the purchase prices(s) circled, and (C) all proof(s) of purchase. For products with paper labels, cut the UPC symbol from the package. For all other qualifying products, write the UPC number in the spaces provided below the grid to the left.
