    InstructionsforResidentialRebateApplications

    INTRODUCTION

    TheMinnesotaSmallWindTurbineRebateforResidencesprovidesfinancialsupportfortheinstallationofsmallwind

    turbinesystemscompletedafterJuly1,2009withratedcapacitiesnotmorethanthirtyfivekilowatts(35kW)at24.6

    milesperhour(mph). Therebateamountis35%ofeligiblesystemandinstallationcostsupto$10,000. Tobeeligible:

    asmallwindturbinesystemmustbeinstalledatanexistingprimaryresidenceinMinnesota; asystemmustbeinstalledbyabondedand/orlicensedcontractorasapplicableunderstateandlocalrequirements; asystemmusthaveaminimumaveragewindspeedof12.0mphathubheight; aninstallationmustcomplywithallapplicablefederal,stateandlocalrequirements;and aninstallationmustbecompletedwithinsixmonths(180days)ofapprovaloftherebateapplication.ThisprogramisfundedbytheAmericanRecoveryandReinvestmentActof2009(ARRA),Pub.L.1115andauthorizedby

    LawsofMinnesota2009Chapter138. Federalandstateguidelinesrequirethatprojectsadheretoanumberofreporting

    andprojectimplementationstandardsasoutlinedbelow.

    siteassessorcanprovideanobjectiveevaluationofyourspecificlocation. Thesiteassessorneednotbethe

    installerwhocompletestheinstallation.

    TERMSANDCONDITIONS

    RebatesmaybeawardedforeligiblesmallwindturbineinstallationscompletedonorafterJuly1st,2009. Rebateswillbeawardedonafirstcome,firstservedbasisuntilallavailablefundshavebeenreserved. Mailedapplicationswillbe

    timestampedashavingbeensubmittedat4:00p.m.onthepostmarkdate.

    ForsystemsbeginningconstructionafterApril30,2010: Participantsmustsubmitanapplicationtoreserverebatefunding. Participantsmustsupplyallnecessarypermitswithrebatereservationapplication. Rebatereservation

    applicationssubmittedwithoutallnecessarypermitsmustprovidepermitapplicationdocumentationandan

    explanationoftimelinetoobtainthepermit. Rebatereservationapplicationswithoutallnecessarypermits

    maybeapprovedonaconditionalbasisprovidedthatevidenceofthemissingpermit(s)isprovidedtothe

    rebateadministratorwithin30daysoftheapplicationdate.

    Ifaprojectisapproved,theapplicanthas180daystoinstallthesystem,assemblethenecessarysupportingdocumentation,andsubmitfinaldocumentationtotheOESclaimingtheirrebate.Extensionsmaybegranted,

    butmustbeappliedfornolaterthan5daysbeforeapplicationapprovalisscheduledtoexpire.

    ARebateConfirmationFormandRebateClaimFormwillbemailedtotheapplicantuponapplicationapproval. ClaimtherebatebycompletingtheinstallationandsubmittingtheRebateClaimFormwith

    supportingdocumentationwithinsixmonthsofreceivingtheRebateConfirmationForm. Noinstallation

    workshouldbedonebeforereceivingtheRebateConfirmationForm. Failuretoadheretothisrequirement

    mayvoidtherebate.

    ResidentialSmallwindturbinesysteminstallationscompletedfromJuly1,2009throughApril30,2010mustmeetprogramrequirementsandtheapplicantmustsubmitalloftheinformationintheaboveRebateApplication

    Checklisttoclaimtherebate.

    Therebateamountis35%ofeligiblesystemandinstallationcostsupto$10,000. Therebateamountcannotexceedthetotalcostofaninstallationminusfederal,utilityorotherincentives.

    Aninstallationmustbeathomeownersexisting,primaryresidence. Rebatescannotbeusedforaninstallationinnewconstruction. Note:Ahomethathasbeenissuedapermanentoccupancycertificatebeforethesmallwindturbineinstallationisstartedwouldbeconsideredexistingconstruction.

    SITEREQUIREMENTS1. SitemustbewithinMinnesotaborders.2. Sitemusthaveminimumaverageannualwindspeedof12.0mphathubheight.Thefollowingevidenceofminimum

    windresourcewillbeacceptedwithrebateapplications:

    EQUIPMENTREQUIREMENTS1. Allmajorsystemcomponentsmustbeneworremanufactured.2. A5yearwarrantyisrequiredonmajorsystemcomponentsandinstallation,withtheexceptionofbatteries,for

    whicha2yearwarrantyisrequired.

    3. Thetowerdesignmustbeapprovedbyaprofessionalengineerforusewiththeproposedturbine.

    APPLICATIONCHANGES Majorchanges(suchaschangeofinstaller,towerheight,location,orsystemsize)requirepriorwrittenapproval. Requestsforextensionoftherebateexpirationdatemustbemadeinwritingatleast5businessdayspriorto

    ARRARequirementsNationalEnvironmentalPolicyAct(NEPA)

    OESisrequiredtonotifyDOEifitdeterminesthatanactivitycategoricallyexcludedfromNEPAreviewappearstopresent

    anextraordinarycircumstanceasdefinedunder10CFR1021.410(b). ApplicantswillneedtocertifytoOESthattheir

    proposedsmallwindturbineinstallationactivitieswillNOT:

    1. Threatenaviolationofapplicablestatutory,regulatory,orpermitrequirementsforenvironment,safety,andhealth,includingrequirementsofDOEand/orExecutiveOrders;

    2. Requiresitingandconstructionormajorexpansionofwastestorage,disposal,recovery,ortreatmentfacilities;3. Disturbhazardoussubstances,pollutants,contaminants,orCERCLAexcludedpetroleumandnaturalgasproducts

    thatpreexistintheenvironmentsuchthattherewouldbeuncontrolledorunpermittedreleases;or

    4. Adverselyaffectenvironmentallysensitiveresources. Environmentallysensitiveresourcesinclude,butarenotlimitedto:

    (i) Property(e.g.,sites,buildings,structures,objects)ofhistoric,archeological,orarchitecturalsignificance

    designatedbyfederal,state,orlocalgovernmentsorpropertyeligibleforlistingontheNationalRegisterof

    HistoricPlaces;

    (ii) Federallylistedthreatenedorendangeredspeciesortheirhabitat(includingcriticalhabitat),federally proposed

    orcandidatespeciesortheirhabitat,orstatelistedendangeredorthreatenedspeciesortheirhabitat;

    (iii) WetlandsregulatedundertheCleanWaterAct(33U.S.C.1344)andfloodplains;

    (iv) Areashavingaspecialdesignationsuchasfederally andstatedesignatedwildernessareas,nationalparks,

    nationalnaturallandmarks,wildandscenicrivers,stateandfederalwildliferefuges,andmarinesanctuaries;

    (v) Primeagriculturallands;and

    (vi) Specialsourcesofwater(suchassolesourceaquifers,wellheadprotectionareas,andotherwatersourcesthat

    arevitalinaregion).

    CompliancewithNationalHistoricPreservationAct

    Priortotheexpenditureoffederalfunds,allbuildingrelatedprojectsmustundergoreviewunderSection106ofthe

    NationalHistoricPreservationActof1966(36CFR800).Section106appliestoprojectsthatincludehistoricproperties

    listedinoreligibleforlistingintheNationalRegisterofHistoricPlacesthathavepotentialtobeaffectedbyfederally

    fundedactivities. AllbuildingsandpropertiesmeetingthefollowingcriteriawillbesubjecttoSection106review:

    Anyprojectinvolvinggrounddisturbingactivity(excavation,utilityinstallation,etc.);or Buildingsatleast45yearsold;and IndividuallylistedintheNationalRegisterofHistoricPlaces(NRHP);or WithinadesignatedNationalRegisterHistoricDistrict; EligibleforlistingintheNRHP(eitherindividuallyoraspartofadistrict);or Subjecttoanapprovedpreservationeasement.

    ItistheresponsibilityoftheapplicanttoprovidetheinformationneededtocomplywithSection106requirements.

    MinnesotaDataPrivacyAct/TennessenWarning

    TheMinnesotaDataPrivacyActrequiresthatcertaininformationyouprovideonthisformremainasprivatedata. The

    informationaboutyouthatiscollectedontheSmallWindTurbineRebateProgramapplicationwillbeclassifiedaseither

    publicorprivatedata. Publicdatawillbeaccessibletothepublic. Privatedataaboutyouwillbeaccessibleonlytoyou

    andStatepersonnelandtheirauthorizedsubcontractorswhoadministertheprogramandreportingrequirements. The

    datayougiveusaboutyourselfisneededto:
