Millennium will consider requests from qualified institutions including hospitals, universities, scientific conferences and patient advocacy organizations, to provide financial support for high quality patient or healthcare professional education. Educational programs that address the treatment of hematologic cancers, such as multiple myeloma and mantle cell lymphoma, are of a particular focus. Examples of the types of educational programs that Millennium will consider providing support for include: accredited continuing medical education programs, grand rounds, tumor boards, scientific conferences and/or journal clubs. Millennium will also consider requests for funding to educate the public, patients, residents, interns or fellows on disease states and medical conditions, again with a focus on oncology and hematologic cancers.

Charitable DonationsMillennium considers requests to provide charitable donations to 501(c)(3) or 501(c)(6) tax-exempt organizations, including donations for patient education, patient support activities, or general fundraising activities for organizations that benefit patients, families, or caregivers.

Scientific & ProfessionalDevelopment GrantsMillennium will consider requests to support independent scientific and developmental activities including: research initiatives (outside of clinical trials), young investigator awards, educational support for medical residents, fellows and trainees, and subsidies for independent, third-party educational and professional meetings.

Requests will only be accepted for consideration by Millennium through this website and must be submitted at least 45 days in advance of the program start date.

To ensure your grant request is reviewed and processed in a timely manner, the following documentation should be attached to your online application:

• Letter of request • Agenda or list of topic(s) to be covered• Needs assessment and/or program objectives• Signed and dated W-9 document with federal tax ID• 501(c)(3) or 501(c)(6) designation letter, if applicable

You will be required to fill out a detailed budget online.

Further details on the information required are provided during the online application process.

Requests will not be reviewed until all of the required information has been provided. Please allow a minimum of 45 days for processing.

What types of programs are outside the scope of Millennium Corporate Giving?

Millennium is not able to provide support for the following types of requests:

• Grants that would be payable to an individual healthcare professional or physician practice• Travel grants for licensed healthcare professionals• Support for education that is routine and/or a required part of medical training, e.g. textbooks, journal subscriptions, or materials for Board Reviews• Clinical Trials (Call 1-800-589-9005 for these requests)• Investigator Initiated Studies (These requests should be submitted at iis.millennium.com)• Display/Exhibit Fees (These requests should be forwarded to your Millennium Sales Representative or submit at displays.millennium.comIf you have any questions about the online application or about the types of programs for which Millennium will provide funding, please contact us [email protected].

Educational Grants Grant Guidelines Submitting a Request

In accordance with the guidelines of the Accreditation Council for Continuing Medical Education (ACCME), Millennium does not influence or control the content of any educational program it supports. In addition, Millennium will only consider providing support for programs that are balanced and scientifically rigorous. Multi-sponsored educational initiatives are strongly encouraged.

Any type of funding provided by Millennium may not be tied, in any way, to the past, present, or future prescribing, purchasing, or recommending for prescribing or purchase of any Millennium drug.
