Report on Inception Meeting with Institute of Public Health Nutrition

Date: 10 July, 2017 Venue: IPHN, Conference Room, Mohakhali, Dhaka

Caritas Bangladesh has organized inception meeting with Institute of Public Health and Nutrition (IPHN) for USAID’s Improving Nutrition through

Community based Approaches (INCA) project on 10th July, 2017 at IPHN conference room. The meeting was chaired by Dr. ABM Muzaharul

Islam, Director, IPHN and Line Director National Nutrition Services (NNS). Dr. Ramesh Chandra Saha, Deputy Director IPHN was present as

Special Guest. Mr. Francis Atul Sarker, Executive Director, Caritas Bangladesh presided the meeting. Total 31 participants including Assistant

Director, Program managers, Deputy Program Manager, and other personnel from IPHN along with USAID’s INCA project staff participated in the

meeting. Mr. Amanullah Deputy Program Manager, NNS, IPHN moderated the meeting.

In the opening speech Director, IPHN addressed the malnutrition situation in Bangladesh; he mentioned situation of Chittagong and Sylhet

division is most vulnerable. As projects are for a specific period activities and approach should sustainable beyond the project. In welcome

speech Mr. Francis Atul Sarker, briefly shared the working area of USAID’s INCA project. He expressed his thanks to IPHN authority for their

valuable time in inception meeting and also informed that Caritas is taking steps to work in Sylhet and Chittagong on nutrition.

Mr. Alomgir Husan, Project Director, USAID’s INCA then presented a details power point presentation on USAID’s INCA project. After the

presentation an open discussion, feedback and query session took place. USAID’s INCA team provided answer of all the queries. Feedbacks and

recommendations from the participants are given below:

1. As new operational Plan (OP) of NNS has been approved, activities of USAID’s INCA should be synchronized with NNS activities.

2. New project in Sylhet and Chittagong Division specially Haor and CHT region.

3. NNS will form mothers group in community. So formation of CNSG group should be synchronized with mothers group.

4. Activities under the project need to follow Govt. Policy, Action Plan and sustainability. As Community Clinic services all the areas of

community this is not accepted not to create a parallel entity (WIC) in community for health services. {It was informed to all that WIC

will provide only nutrition services mostly nutrition sensitive services.}

5. Use existing GoB approved module, guideline and SBCC materials. Developing new materials is a long process to get approval.

6. Govt. is struggling to get actual information on number of pregnant, breastfeeding and child. NNS expects Caritas will share the pregnant

mother list with them. If it is possible Caritas can register children upto 5 years.

7. Different and innovative approaches need to undertake. During courtyard session project may provide nutritious food to mothers as

incentives (This recommendation was strongly opposed by Director IPHN, He suggested not to make this type of activities with the

community people who are not sustainable and change their habit seeking assistance. Instead of this, Govt. has some social incentive

program project can link beneficiary to this safety net programs.

8. Target set in the indicator is very high. To track the project towards target baseline survey, end line survey and mid line survey need to

conduct and findings are expected to share with IPHN.

9. Stunting is a long term consequence of food insecurity. So to address stunting food insecurity must be address. Activities need to

undertake to ensure food availability. (Director IPHN, suggested project may provide technical advice.

10. Any new materials need to be placed to IEC technical committee for their approval. Draft need to be shared immediately.

11. Caritas may arrange another consultation workshop to decide which module and materials it need to implement project.

12. There is a BCC working group who sit for the meeting by-monthly. Caritas should joined the group and share their activities and

approaches.

In his speech Dr. Ramesh Chandra Saha expressed thanks to Caritas for its nice presentation on USAID’s INCA project. He also suggested sitting

with respective program manager for planning the activities jointly to avoid duplication.

In his closing Speech Dr. A.B. M. Muzaharul Islam requested to give him draft copy of MoU. He thanked Caritas Bangladesh and his colleagues for

their presence and active participation.

Inception workshop of USAID’s Improving Nutrition through Community based Approaches (INCA) project in Bhola District.

On 18 July, 2017, Caritas Barisal Region organized an Inception workshop of USAID’s Improving Nutrition through Community basedApproaches (INCA) project at Conference hall of Bhola Civil Surgeon Office, Bhola District.

Doctor Rathindra Nath Majumdar, Civil surgeon of Bhola district present as Chief Guest in the Inception Workshop. Mahmudur Rahman Azad,Deputy Director of Family Planning, Bhola, Dr. Nityananda Chowdhury, Upazila Health and family planning officer,

Charfashion Upazila Health and Family Planning Officer Dr Siraj Uddin as well as upazila Health and family planning officers and Upazila familyplanning officers from Seven Upazilas were present in the event as distinguished guests.

Also teachers, journalists, civil society, representatives of Local Government along with USAID’s INCA project staff from Bhola District andCentral office participated in the Workshop.

The event was chaired by Mr.Francis Bepari, Regional Director of Caritas Barisal Region. Mrs. Lily A. Gomes, SBCC Specialist of USAID’s INCAproject made a PowerPoint presentation on the USAID’s INCA Project.

Md. Shahadat Hossain, Senior Health, Education officer of Bhola civil surgeon, moderated the ceremony.

On behalf of the Caritas Bangladesh Mr. Francis Bepari, Regional Director of Caritas Barisal Warmly welcomed participants to the USAID’s INCA

Project workshop. In the welcome speech he mentioned about Caritas Bangladesh history and USAID’s INCA Project goal, objective and working

area in Bhola District. He requested to the all respective sector through the civil surgeon of Bhola to work in collaboration for improving the

nutritional status of women and children in selected undernourished and rural in target areas.

After the welcome speech Mrs. Lily A Gomes, SBCC specialist, USAID’s INCA Project presented a details power point presentation on USAID’sINCA project. After the presentation an open discussion, feedback and query session took place.

Doctor Rathindra Nath Majumdar, Civil surgeon of Bhola district expressed his positive impression in speech to work together to improve

nutritional status of women and children across the area. At the same time, he requested to all respective members; Community Clinic, Union

Health & Family Welfare Center and existing nutrition service providers and organizations. Also he advised to the Caritas to participant and

submits monthly report in regular meeting in civil surgeon office.

Mahmudur Rahman Azad, Deputy Director of Family Planning expressed thanks to Caritas for going to implement on USAID’s INCA project. Hewelcomed the project and said the project will Increase access to nutrition knowledge at community level.

Total 84 participants in the workshop came from Health and family planning, Upazila family planning, civil society, journalists, and

representatives of others NGO staff members were present in the Inception workshop.

Caritas Chittagong Region has organized an inception meeting with Health and Family Planning Department of Noakhali district for USAID’s

Improving Nutrition through Community based Approaches (INCA) project on 30th July, 2017 at White Hall Complex, Mayizdi, Noakhali. The Chief

guest was Dr. Md. Shamsur Rahman, Civil Surgeon, Noakhali and the workshop was Chaired by Mostofa Kamal, Deputy Director, Family Planning

department.

The Special guests were;

Professor A.H.M. Khairul Anam Choudury Salim, Upazila Chairman, Subarnachar Upazila

Md. Anowarul Azim ,Upazila Vice Chairman, Hatiya Upazila

Md. Harunur ,Upazila Nirbahi Officer (UNO), Subarnachar Upazila.

Ranjon Francis Rozario , Assistant Executive Director (Programs), Caritas Bangaldesh

And UH& FPO of Noakhali Sadar Upazila, Subarnachar Upazila , UFPO of Subarnachar Upazila and also attended Medical Officer- MNCH ,

Medical Officer- Disease control of Sadar Hospital and UN , NGO Officials, Journalists and USAID’s INCA Project staff.

The workshop was started with registration and introduction with each other and after introduction session James Gomes, Regional Director,

Caritas Chittagong Region delivered welcome speech to all and after that Lily Anthania Gomes, SBCC Specialist, USAID’s INCA Project shared the

project briefly through power point presentation. After presentation, open discussion session was started when various queries, suggestions and

recommendations came out and answered the questions from Caritas and suggestions and recommendations were noted down.

All the guests and participants attended actively and delivered valuable suggestions and recommendations in the workshop. Some major

suggestions and recommendations are given below.

Major Suggestions and recommendations:

Joint GO-NGO advocacy, information and services should be introduced.

Overlapping of same activities should be diminished through strong coordination

Use of long term method should be introduced for ensuring sustaining nutrition services

Need to include Family Planning services with the project activities

GoB and NGO joint action Plan should be introduced

Need to form a district level common platform to strengthen nutrition services

Improve monitoring and supervision systems through strong coordination

Arrange local level workshop (Upazila & Union) to orient the people about the objectives and working modalities of the project

Reduce demand side gap through strong coordination with GoB , UN and NGOs

Growth monitoring and Promotion should be introduced

Conduct baseline survey and share with stakeholders to compare with project’s end line result after three years

Focus early marriage in project activities

Monthly progress report sharing with GoB administration, health and family department

Special guests gradually delivered speeches where most of them emphasized the joint coordination among GO-NGOs. AED-P, Caritas said Caritas

always tries to follow all guidelines of Govt. and integrate with different events. Chief Guest, Dr. Samsur Rahman, Civil Surgeon, Noakhali

delivered his speech that malnutrition is a problem in Bangladesh that exist not only in poor communities, it also exists in all communities. So we

have to emphasis on nutrition awareness through all aspects to combat against malnutrition. And The Chairperson Mostofa Kamal, DDFP,

Noakhali delivered his speech that malnutrition is a problem in Bangladesh and especially in the Noakahli district and nutrition situation will

improve if health, family planning and other relevant department work with strong coordination and at last he closed the workshop with vote of

thanks.

1. Introduction

USAID’s Improving Nutrition through Community-based Approaches (INCA) Activity is a 3 years project. The project will implement targeted

nutrition-specific and sensitive interventions at household and community level that will leverage and build on existing government, USAID and

other donor efforts in the field of nutrition, WASH and agriculture. The project will increase knowledge of and access to nutrition and health

related services which will further reduce stunting within the crucial first 1000 days of life. It is pre requisite to collect the demographic

information, existing GO and NGOs health facilities existing, the NGOs working and available mode of transportation in the USAID’s INCA

working area with a view to analyze the data to make informed decision making and project planning.

2. Objectives of mapping:

The objectives of USAID’s INCA mapping are –

Planning for project intervention

Community Nutrition promoter (CNP), Local Service Provider (LSP) identification and recruitment

To Take strategic decision where to form Women Information center

To know the demographic information of INCA working area

To map the NGOs working in the USAID’s INCA working area

To understand the existing health and GO and NGO health facility in the USAID’s INCA working area

3. Methodology: The following steps have been followed to collect, analysis and interpretation of data.

Step 1: Development of tools: Mapping tool has been developed through consultation with INCA technical team.

Step 2: Select team for data collection: The management of INCA decided that field supervisor will collect data from field.

Step 3: Orientation to the field supervisor on developed tolls

Step 4: Data collection: The data has been collected by field supervisor. The data has been verified by field coordinator.

Step 5: data entry: Data has been entry by project Officer-M&E to make the hard copy to electronic version.

Step 6: Data analysis: Data has been analyzed by Senior Expert M&E with the help from technical person and direct overall guidance from

program director in different perspectives to make useful information for decision making. The analyzed data has been presented in tables.

4. Result and discussion:

The data revealed that total population is 3,187,404 where male are 15, 89,594 and female are 15, 89,594. The total population is

come from 610,662 households. There are 11 Upazila in 3 working districts of USAID’s INCA. The average population per Upazila is

289,764. Interestingly average male and female population per Upazila is very close which is 145,255 and 144,509 respectively. In an

average there are 5.22 members per household. Upazila wise population distribution is furnished in below table.

There are 106 unions in USAID’s INCA working area. The total population among the Unions are 3,295,133 where total male 1,670,151 and

female 1,624,982. The total population is from 617,267 households. In an average total population is …… total member per households in

average is 5.34. Like Upazila average both male and female population per union is close i.e. 15,756 and 153330 respectively.

There are 23 NGOs working within the UAID’S INCA working areas. The NGOs are BRAC, CWFD, DORP, ICCO, Marie stoops, Red Crescent society,

UNICEF and Shanirvor Bangladesh, BAPSA, BURO Bangladesh, DSK, Prijom Bangladesh, Sagorika Samaj Unnoyon Songstha, SAJJ, SEBA Social

foundation, Smiling sun, sopiret, BEES, BRAC, DUS, FPAB, Friedship, RIC, Smiling Sun and SSUS. The NGOs working in three working districts of

USAID’s INCA differ that has been summarized in above table. All of the NGos are not working with nutrition. Among the NGos Shanirvor

Bangladesh, BRAC, ICCO, BAPSA, BEES and SSUS are working with nutrition. In Bhola districts Shanirvor Bangladesh, BRAC, ICCO is working with

Nutrition. In Noakhali districts BAPSA, BEES and SSUS are working with nutrition. Surprisingly in Lakshmipur districts none of the NGOs working

with nutrition. Now we would like to see the the upazila were no nutrition project is working. In below table it is observed that there are 7

Upazila where no project exclusively working with nutrition.

There are 404 Govt. health facility exist in USAID’s INCA working area. Among them there are 3 sadar hospital, 11 Upazila Health complex,311

community clinics and 74 Union health and family welfare center (UH&FWC) are remarkable. Although there are 312 community clinics are

working in USAID’s INCA working area but Fakirhat community Clinic merged with Char Fayzuddin CC in Hazirhat Upazila of Bhola districts.

In the USAID’s INCA working area a total of 312 community clinic exists in which there are 27,161 registered pregnant women 39,374 lactating

mother and 84,959 under 2 years of children. In an average there are 3.06 Community Clinics which need to provide service of 31,086

populations. In an average each Community Clinic need to provide service 10,159 population and very specifically 87.05 pregnant women,

126.20 lactating mother and 272.30 under two years of Children. The number of community clinic per union very from 1-8. The distribution of

number of community clinic per union is given below table. Unfortunately, it is evident from the data that there are 4 unions where there are no

Community Clinics. These are Madua, Hazipur and Vhobanipur union of Daulatkhan Upazila in Bhola district and Charbata union of Subarnachar

in Noakhali district that need special attention.

A total of 1216 Local Service Provider (LSP) were identified from the villages who has been come from different profession like skilled /Unskilled

birth attendant, village doctor, teacher, religious leader and public representatives. After that LSP will finalize based on set criteria for the

project.

7. Women Information Center (WIC) mapping:

What is (WIC)

Where women can get information related to nutrition, livelihoods and empowerment.

Will improve the women’s access to affordable nutritional support services, such as technical advice, counselling, referrals services, homestead

based production inputs, IT based nutrition awareness, business and marketing support, and will also serve as a gathering place for women,

where they can more easily be reached with a variety of free health care services.

Forecast the Success of New Theme

Mapping solutions that allow project to conduct in-depth location analysis and make better site selection decisions. Gain key insights

about health facilities and people surrounding current or potential locations for establish WIC.

Gain a deeper understanding of the govt. health facilities specially CC (community clinics) that contribute to the support of currentlocations and find appropriate remote location with those same characteristics where project can contribute to improve the nutritionalstatus.

Conduct altitudinal analysis and analyse the important relationships between the data relevant to the spatial influences that affectproject services including initiative, movement counts and remoteness to challenges women and children.

Identified remote locations In project targeted Upazila (8) are entirely or partially char islands; CC are therefore unable to service the

needs of all pregnant and lactating women as a result of volume, distance that women have to travel and communication/ transport

difficulties between chars.

Identified locations of CC facilities through the project identified health facilities which includes Community Clinics (CCs), Union Healthand Family Welfare Centres and Smiling Sun Clinics. Some area, which is 100% catchments population with geographic isolated area, CCsare over burden.

Find the Most Suitable Location

Through the demographic data and communication mapping conduct analysis and find areas where there is a space in the location for WIC.

How mapping conduct (Methodology):

Data collection from possible sources of Districts /Upazila Statistics Office, Upazilla Parisad, Union Parisad.

Information collection from Health and Nutrition Service provider GOB. And NGOs of Upazila, Union and Community level.

Individual contact and FGD with community member.

Individual contact with Health Worker.

Physical visit and data collection from Community Clinic.

Analysis from the collected data reveals that there are 217 villages where there is no community clinic. People usually goes nearby

another union to get health and nutrition services that is about in average 5.54 Km away from their village. So, we recommend while

forming Women information center (WIC) to priority over the villages.

Process to Developing WIC:

The project will identify 40 WIC from the selected priority village.

The Information center is one- or two-room low-cost buildings with basic facilities for meetings and storage.

The canters will be managed by female local service providers, of 4-5 female entrepreneurs (total of 200 entrepreneurs) who will use the

centres as a base from where they can offer their services to rural women.

The canters will regularly be made available to health services, nutritional information and public service providers.

In the longer run the canters will develop into become business hubs, giving women access to a wider range of markets and support

services and creating opportunities for companies and public service providers to scale-up their business and operations.

The business center will be equipped with basic IT Technology like computer, printer, photocopy and internet facilities to enable the use

of cost-efficient remote service delivery, e.g. skype meetings, mobile banking, on-line ordering and courier

6 : Recommendation:

It is recommended to provide special focus on Lakshmipur district while planning as deprived from exclusive nutrition project

It is also recommended to focus on 7 Upazila i.e. Bhola sadar, Daulatkhan and Charfassion in Bhola district and Kamalnagar and ramgati

of Lakshmipur district and Hatiya and Subarnavhar in Noakhali district as no project working on nutrition there as well

Madua, Hazipur and Vhobanipur union of Daulatkhan Upazila in Bhola district and Charbata union of Subarnachar in Noakhali district

seek special attention as there are no community clinics in these unions.

To form Women Information Center (WIC) ,it is recommend to priority on unions those have none/one/ two community clinics

Special attention will be needed on Hazirhat Union of monpura upazila in Bhola district as two Community clinic is merged there i.e. Char

Fayzuddin CC and Fakir Khat CC
