NOTICEMoKan Dial Telephone Company’s

Lifeline ServiceLifeline service is a government assistance program that provides discounts on monthly telephone service for eligible low-income consumers. Lifeline is supported by the Fed-eral Universal Service Fund (USF). Only one Lifeline benefi t is available per household. A household is defi ned, for purposes of the Lifeline program, as any individual or group of individuals who live together at the same address and share income and expenses. A household is not permitted to receive lifeline benefi ts from multiple providers. Violation of the one-per-household limitation constitutes a violation of the Lifeline rules and will result in the subscriber’s de-enrollment from the program. Lifeline is a non-transferrable benefi t and the subscriber may not transfer his or her benefi t to any other person. Will-fully making false statements to obtain the benefi t can result in fi nes, imprisonment, de-enrollment or being barred from the program.

Who qualifi es for reduced Telephone Charges? To participate in the program, consumers must either have an income that is at or below 150% of the Federal Poverty Guidelines or participate in one of the following assistance programs:

Medicaid

Supplemental Nutrition Assistance Program (SNAP) (formerly listed as Food Stamps)

Supplemental Security Income (SSI)

Federal Public Housing Assistance (Section 8)

Low-Income Heat and Energy Assistance Program (LIHEAP)

National School Lunch Program (free lunch program only)

Temporary Assistance for Needy Families (TANF)

General Assistance

Food Distribution Program

Additional eligibility criteria may apply to residents of federally recognized tribal lands.

In most cases, consumers will have to provide proof of eligibility upon application for Lifeline service. In addition, consumers must complete a Lifeline application and make certain certifi cations upon signing up for Lifeline and agree to re-certify their continued eligibility on an annual basis.

Should you have any questions; please contact a MoKan Dial Telephone Company representative at (913) 837-2219 or via our toll free number (800) 758-1715.

Hoops is a young, ener-getic and awesome Labra-dor mix. He is a sweetheart who loves everybody and everything. He is ready for a forever home fi lled with unconditional love. His es-timated DOB is 05/29/2015. Approx. weight is 26 lbs and growing!

All PPAS puppies and dogs are up to date on all of their vaccines (including rabies if age prohibiting), and will be

micro chipped and registered in the adopter’s name at the time of adoption. Adopters get a free consultation at Cottonwood Animal Hospital within 14 days of adoption. Any dog that is six months of age or older will be tested for Heartworms and if found negative, will be kept on Heartworm prevention during their shelter stay, after the adoption it is up to the adopter to continue the highly recommended Heartworm preventative treatment. Also all PPAS shelter dogs are treated with a monthly topical fl ea/tick preventative and we ask that adopters continue that treatment as well.

The shelter is open to the public: Mon, Wed, Thurs 11-6 and Sat 11-4.  If you are interested in this dog or puppy, please either email Prairie Paws Animal Shelter at [email protected], for an application visit our website @ prairiepaws.org or call 785.242.2967 for more information.
