1 Introduction This document describes the CDISC implementation of the Michigan Neuropathy Screening Instrument (MNSI) questionnaire, a standard questionnaire administered on a CRF that is typically used in clinical trials to measure pain response. The MNSI CRF preceded the CDISC CDASH CRF standards and based on its public domain status, cannot be modified to CDASH standards. The representation of data collected for this questionnaire is based on the Study Data Tabulation Model Implementation Guide (SDTMIG) QS domain table, which can be found at the CDISC website at http://www.cdisc.org/sdtm. These implementation details for this specific questionnaire are meant to be used in conjunction with the SDTMIG, but are recorded separately since this questionnaire may be used in many different therapeutic area implementations. All questionnaire documentation can be found on the CDISC web site (http://www.cdisc.org/content2909). 2 Copyright Status This instrument is in the public domain. CDISC has included the Michigan Neuropathy Screening Instrument (MNSI) as part of CDISC Data Standards. This means that CDISC developed QSTESTCD and QSTEST for each question based on the actual question text on the questionnaire. There may be many versions of this questionnaire in the public domain. CDISC has chosen to use this version as the data standard. The CDISC documentation of this instrument consists of: (1) controlled terminology, (2) standard database structure with examples and (3) case report forms annotated with the CDISC SDTMIG submission values. CDISC acknowledges the MDRTC as the source of the items in the survey instruments that are included in the CDISC SDTM Pain Data Standards. 3 The QS Domain Model
3.1 Assumptions for Questionnaire Domain Model All assumptions for the QS domain from the SDTMIG apply for this supplemental implementation guide including those referenced in the CDISC notes. Additionally, the following assumption applies to the Michigan Neuropathy Screening Instrument (MNSI):
• Michigan Neuropathy Screening Instrument (MNSI): The Michigan Neuropathy Screening Instrument (MNSI) is designed to screen for the presence of diabetic neuropathy. The MNSI is designed to be used in an outpatient setting by primary care or other providers. The first part of the screening instrument consists of 15 self-administered "yes or no" questions on foot sensation including pain, numbness and temperature sensitivity. A higher score (out of a maximum of 13 points) indicates more neuropathic symptoms. The second part of the MNSI is a brief physical examination involving 1) inspection of the feet for deformities, dry skin, hair or nail abnormalities, callous or infection, 2) semi-quantitative assessment of vibration sensation at the dorsum of the great toe, 3) grading of ankle reflexes and 4) monofilament testing. Patients screening positive on the clinical portion of the MNSI (greater than 2 points on a 10 point scale) are considered neuropathic and referred for further evaluation.
• The evaluator of the questionnaire is stored in QSEVAL and for the MNSI form the Subject provides the evaluation on page 1 and the INVESTIGATOR provides the Physical Assessment evaluation on page 2.
• The MNSI CRF is to be provided at a baseline visit. QSTPT=Baseline provides that time point reference.
• Terminology: a. QSCAT, QSTESTCD and QSTEST are approved CDISC controlled terminology. b. Additional standardization of the QSORRES, QSSTRESC and QSSTRESN fields
can be found in Section 4: Mapping Strategy.
3.2 Example for Michigan Neuropathy Screening Instrument Domain Model The MNSI example below shows the terminology for QSCAT, QSTEST, QSTESTCD and QSORRES that should be utilized for this scale. Values for QSORRES are for prospective data collection. Sponsors mapping legacy data should retain legacy values for QSORRES. A full list of value sets for QSORRES, QSSTRESC and QSSTRESN fields is provided in Section 4: Mapping strategy.

Example: Michigan Neuropathy Screening Instrument (MNSI) This example shows data from one subject collected at one visit for a MNSI questionnaire. The example uses standard CDISC controlled terminology for QSTESTCD, QSTEST and QSCAT. QSSCAT, QSORRES, QSSTRESC and QSSTRESN uses MNSI controlled terminology indicated in Section 4: Mapping Strategy. The scoring of the original results is based on the MNSI Scoring document on the Michigan Diabetes Research and Training center web site. Rows 1-37: Represent the questions from the MNSI Form for one visit for SUBJID= 2324-P0001. All original results are represented with the standard terminology in QSORRES. This result is then transformed into a standard numeric score in QSSTRESN. All summary scores can either be entered or they can be derived via the detailed questions for each summary score. When derived, QSDRVLFL is set to “Y” Rows 22, 28: Represent specify: “text” answers with no text provided in QSORRES. These records are for illustrative purposes in order to display the QSTESTCD/QSTEST values and contain the value of XXX, the actual missing record would not be created in the QS domain. Rows 16 and 37: Represent the summary scores. All summary scores can either be entered or they can be derived via the detailed questions for each summary score. When derived, QSDRVFL is set to “Y”.
