Micheline’s Pups Receipt Contract BREED ___________________ DATE ______________________________ SEX ___________________ PRICE ______________________________ D.O.B. ___________________ It is the consumer’s right pursuant to section 828.29. Florida Statutes to receive a certificate of veterinary inspection with each dog or cat purchased from a pet dealer. Such certificate shall list all vaccines and deworming medications administered to the animal and shall state that the animal has been examined by a Florida-licensed veterinarian who certifies that, to the best of the veterinarian's knowledge the animal was found to have been healthy at the time of the veterinary examination. In the event that the consumer purchases the animal and finds it to have been unfit for purchases as provided in section 828.29(5) Florida Statutes, the consumer must notify the seller within 2 business days of the veterinarian’s determination that the animal was unfit. The consumer has the right to retain, return, or exchange the animal, and receive reimbursement for certain related veterinary services rendered to the animal, subject to the right of the dealer to have the animal examined by another veterinarian. To the best of my knowledge this is a healthy puppy, without congenital or hereditary diseases (except______________). Therefore, you as purchaser, waive pursuant to Florida Statutes 828.29(6) the right to return the puppy for congenital or hereditary disorder. Purchaser agrees to have puppy checked by a veterinarian within two business days of sale. If the veterinarian certifies at the time of the purchase, the puppy had a congenital or hereditary disorder (except _________________), Seller will afford the remedy provided by Florida Statute 828.29(6). This excludes veterinary costs related to the certification. Purchaser agrees to care for the puppy that is the subject of this transaction according to the written instructions provided by seller. Seller shall not be responsible for illness of disease that is the results of fleas or ticks, illness or disease that is contracted after the date of purchase, or illness or disease that results from the failure to provide care for the puppy in accordance with the written instructions provided by the seller. Should the puppy be deemed within fourteen days of purchase, unfit for purchase for some illness or other than types outlined above, purchaser agrees to return the puppy within that time period to Seller or such other place as the Seller may deem appropriate, for treatment. In case of such illness, purchaser will be afforded the remedy provided by Florida Statute 828.29(5) excluding reimbursement of veterinary expenses for treatment which will have been paid directly to sellers veterinarian. Purchaser must present documentation and veterinarian statement in writing by registered mail within time allowed. The puppy that is the subject of this transaction is sold “as is, where is.” Seller makes no expressed or implied warranties including those of merchantability or fitness for a particular purpose. Specifically, Seller does not warrant that the animal shall attain a particular size or that the puppy is for for showing or breeding. Seller will not accept for return or exchange any puppy for any reasons except those outlined above. This contract shall be governed by Florida State Law, and for purposes of jurisdiction and venue. This transaction shall be deemed to have taken place in Ocala, Marion County Florida. If any provision of this contract is found to be ineffective, the remainder shall still be binding and effective. Purchaser represents that he/she has read the foregoing and that this agreement has been entered into freely and voluntarily and that he/she has read the entire agreement and that he/she agrees to all terms contained herein.     HypoglycemiaHypoglycemia is a common ailment in toy breed dogs, Hypoglycemia is known as LOW BLOOD SUGAR!! Shih Tzus, Yorkies, Poodles, and other toy breeds CAN develop Hypoglycemia even when they appear healthy. This is because they do not have enough fat stored to use for energy. Your new puppy should be monitored closely!!

    CAUSE: Common causes are stress (separation from the litter), lack of food, quality of food, or change of food, over exercise, not enough rest or just simply being too cold.

    PREVENTION: Feed your puppy a high quality DRY puppy food, or a dehydrated dog food. I have also had good success with Bil Jac for small breed puppies. You can add a teaspoon of Karo Syrup or sugar to the puppy’s water to keep the sugar level normal if you have a pup that is having sugar drops. Keep water available AT ALL TIMES! Leave food out 24 hours a day, 7 days a week, at LEAST until the puppy is 16 weeks of age (longer if the puppy is smaller than average). Never feed the puppy sweets, fried foods, or plain milk. DO NOT allow the puppy to become over-chilled or tired. Keep your puppy warm and dry, while allowing the puppy to get plenty of rest and become accustomed to its new home.

    SIGNS AND SYMPTOMS: Symptoms include, but not limited to, slow heartbeat and/or breathing, weak, disoriented, wobbly gate, glassy eyes, seizures, and in some cases loss of consciousness, coma, and even death. Please seek treatment if any of the symptoms arise.

    TREATMENT: If you notice your puppy exhibiting any of the able symptoms, try giving your puppy some treats(cheese) or anything the puppy will eat. If your puppy will not eat, try giving it some Karo Syrup, honey or Nutri-Cal (bought at pet stores). If your puppy becomes unconscious try rubbing Karo Syrup or Nutri-Cal onto its gums and under the tongue. Keep the puppy warm. A drop in blood sugar will drop the temperature of the puppy. If none of the above treatments work, SEEK VETERINARY CARE IMMEDIATELY!!

    Hypoglycemia is NOT a disease or a genetic inherited sickness. Hypoglycemia is very common in small breed dogs, but can be managed with PROPER care.

    I cannot control how the puppy is cared for after it leaves my home, therefore I will not take a puppy back OR replace a puppy that gets Hypoglycemia or passes away from it!

    **Under NO CIRCUMSTANCE will Hypoglycemia be covered under the health guarantee.**
