FERPA Release Sample for Researchers

This format may be used for research but must be edited and the form image submitted with applications to the Miami Research Ethics and Integrity Office as part of the initial review.

To be valid, this google form must use the "Restrict to Google Apps @ miami login" and "Collect Email Addresses" options found on the settings menu.

Do not simply ask for their UniqueID and password.

The sample below is for a stand-alone release with minimal information about the project. A complete consent-FERPA release would include all the elements of consent and can be used as the means for obtaining research participation consent.

Release of Educational Record for "title of project"By having logged in using my confidential Miami information and providing the information below, I release to the researchers (course grades, complete transcripts, test scores, writing assignments etc) for the use described below.

If released, educational records will be linked to other research data.
