Bone Management® is key to successful implant procedures. “Tissue is the Issue but Bone Sets the Tone.” Besides the prosthetic aspect, achieving minimally invasive surgical techniques, along with predictable results, facilitates less traumatic and more controlled surgical procedures. This presentation will review our 10 year research experience investigating factors that contribute to reproducible single tooth implant esthetics. Clinical illustrations will highlight the following factors: 1) bound edentulous site architecture, 2) adjacent tooth factors, 3) buccal bone factors, 4) implant location, 5) immediate vs. delayed placement, and 6) implant and abutment design factors. When considered in aggregate, the data generated contribute to defining a standardized protocol for implant planning, placement and restoration that lead to highly reproducible outcomes.

Patients who are planned for a full of partial arch fixed implant supported maxillary restoration will need assessment of the location of the crowns relating to implant placement strategy and long term maintenance. If a flange is used and covers the implants, preventing ease of access for hygiene, long term problems can occur. This presentation will develop planning algorithms that allow for precise implant positioning resulting in less bone removal and zero to minimal flange. The use of .stl planning images superimposed on cone beam scans will be demonstrated resulting in implant prosthetics that mimics natural teeth. These patients have excellent access to the implants following traditional prosthetic principles.

Horizontal and Vertical Ridge Augmentation – Myths Versus Reality (Lecture)

Many techniques and augmentation materials have been described for horizontal and vertical ridge augmentation. These include guided bone regeneration (GBR), ridge expansion/splitting, autogenous onlay block grafting, titanium mesh techniques, distraction osteogenesis, and interpositional osteotomy. While many techniques offer excellent results in some situations few can be said to guarantee success. Many questions still remain on which methods yield the most predictable results for dental implant placement in grafted bone. The aim of this lecture is to critically evaluate the current evidence to determine the predictability of various bone augmentation techniques for horizontal and vertical ridge defects for implant placement. A series of cases have been gathered to illustrate risk assessment and predictable management of various critical size defects ranging from moderate to severe defects.

Autogenous Bone graft is still the gold standard for the reconstruction of severe atrophy of the maxilla or the mandible. Two & 3 dimensional bony defects need for the reconstruction special surgical procedures with autogenous bone grafts to assure at long term an acceptable functional and esthetical result. Biomaterials have here their limitation and are for such reconstruction until today not an alternative due to their poor regeneration potential. Autogenous bone graft harvested from intraoral sites, especially the retromolar area of the mandible, and used following the split bone block (SBB) technique is offering many possibilities for intra operative facilities and stable long term results. Splitting the thick cortical block to 2 or 3 thin blocks is augmenting the number of blocks allowing the reconstruction of larger atrophic crest and giving a better adaptation to the recipient site with individual determination of the width and the volume of the grafted area. Filling the space and gaps between the thin block and the remaining crest with particulate bone chips is reducing the time needed for revascularization of the graft improving its vitality compared to the original thick block. Implants inserted in this grafted bone presented in long term similar osseointegration as implants placed in non grafted bone.

Vertical and horizontal augmentation presents one of the greatest challenges of bone regeneration in implant dentistry. This is primarily due to the difficulty of the surgical procedure and its potential complications. Patient selection, patient preparation for surgery, precise surgical techniques and postoperative management are the key factors in reducing the rate of bone graft complications. To predictably achieve successful bone augmentation angiogenesis, clot stability and space maintenance should be employed. The SAUSAGE TECHNIQUETM is a minimally invasive and safe approach that is utilized to reconstruct severely atrophied ridges. The choice of grafting materials as well as the long-term preservation crestal bone preservation after ridge augmentation will be presented.Another important factor is flap closure during bone augmentation. The key to achieving wound closure is not only the clinician’s ability in obtaining tension free release flap but also good soft tissue quality and quantity. In an attempt to achieve wound closure and hence graft stability, the buccal mucosa are often broadly released, and this often results in a severe apical translocation of the mucogingival line, loss of vestibule and keratinized mucosa (KM).Recent research on vertical and horizontal ridge augmentation as well as soft tissue reconstruction will utilizing a collagen matrix CMX will be presented. Utilizing these procedures may lessen the need of harvested autogenous bone and may generally lead to decreased morbidity and therefore increased patient comfort and satisfaction associated with these regenerative procedures.

Preserving Soft and Hard Tissue Around Implants in the Aesthetic Zone. New Perspectives and Treatment Workflow (Lecture)Stavros Pelekanos, DDSSingle implant restorations in the esthetic zone require often demanding surgical and prosthetic therapeutic procedures in order to achieve long term stable results. Despite the clinical and scientific evidence, the clinical handling and the treatment sequence and workflow is of outmost importance determining the final outcome of the implant restoration.

The soft and/or hard tissue enhancement in the majority if not all of the cases in the esthetic zone is absolutely necessary in order to achieve a highly esthetic and natural appearance of the implant crown. The timing, the material selection and the type of the prosthetic restoration further complicate the decision-making and affect the final result.

This presentation will focus on the methodology and treatment sequencing for the single implant-abutment-crown complex and give new perspectives for the treatment planning in the esthetic zone. Moreover, retrospectively various treatment modalities will be discussed and a variety of clinical cases will be presented.

Robert Horowitz, DDS Surgical Techniques to Enhance Orthodontic & Other Surgical Techniques to Enhance Orthodontic & Other Advanced Surgical Therapies

The number and scope of instruments and equipment necessary for optimal diagnosis and treatment has grown exponentially. One of the most critical instruments in surgical armamentarium has been, for over 10 years, Piezosurgery. This micro vibrating, ultra precise, ultra-high tech piece of equipment is used in most of Dr. Horowitz’s surgical procedures related to the eventual placement of dental implants. Whether the goal of the initial surgery is atraumatic extraction, ridge split for width augmentation, implant placement, osteotome or lateral approach for sinus augmentation, the Piezosurgical unit has a place. A novel use for this unit is performing corticotomies to assist orthodontic tooth movement. Cases will be shown with both clear aligner and fixed appliance therapy. The addition of Piezosurgery to the group of procedures in PAOO (also called POPA, Wilckodontics) is an important addition to orthodontic therapy and is generally performed by periodontists and oral surgeons. Using surgically enhanced orthodontic therapy, tooth intrusions are more predictable, open bites can be closed more easily, and with hard and soft tissue grafting, arches can be expanded more effectively to get better occlusal schemes and torque roots palatally/lingually to keep the facial surfaces inside the alveolar housing. There are appropriate tips and protocols to make these and other surgical visits (surgical debridement for treatment of peri-implantitis and crown lengthening to name a few) simpler during the procedure and more predictable in outcome. This workshop will demonstrate multiple uses of Piezosurgery. At the end, the participant will have seen both still photos and intraoral videos (through an operating microscope for unparalleled visibility) to familiarize him-or herself with clinical tips that will be usable in many patients.
