OriGinsThe origins of GCE (Gas Control Equipment) go back to the start of the 20th century when Gas Welding was first invented. The GCE group was formed as an independent company in 1987 through the merging of two of the worlds leading gas and welding companies into one inde-pendent unit. GCE has grown rapidly since its establishment and is leading the restructuring of the European gas equipment industry through mergers and acquisitions. Through its extensive Research and Development programs GCE has set standards that have become the benchmark for the whole industry.

GCe serviCesThe main industrial customers for GCE are wholesalers and local distri-butors. However in some markets GCE distributes equipment with the full cooperation of the main gas supplier for that market.

For these companies GCE provides both commercial and technical support. A significa t part of the sales volume in this area also comes from key end user accounts such as shipyards, repair shops, OEM custo-mers and welding machine manufacturers.

All Medical Gas Applications require the highest safety stan-dards, specialised knowledge and a full understanding of patient re-spiratory requirements. GCE fully complies with the European Medical Directive 93/42/EEG and the QSR (Quality System Regulations known as the GMP standards).Western Europe is the traditional market for a whole range of products for all different types of customers and together with the growing Eas-tern European market, these are GCE’s main sales area. Our sales force is also active in a number of other markets including North and South America, North Africa and the Middle East, and continues to expand to other parts of the world, such as China and India. As a result, our pro-ducts are designed in accordance with the requirements of all Europe-an standards as well as those of other regions where they will be used.Supplies to the major Gas Companies account for the majority of sales however in each market various distributors, healthcare providers, hos-pitals and other manufacturers of medical also play a significa t role in the distribution of GCE-branded and CE-marked products.GCE supply complete systems for supplying oxygen, nitrous oxide, other gases and vacuum to hospitals, ambulance service providers, emergency services and home care providers, as well as other special services using this equipment.

The Mediselect range of regulators are available in both Pin Index and 5/8“ BSP Bullnose connection with a universal fir tree low pressure outlet compatible with all oxygen therapy tube diameters.Mediselect regulators are manufactured from nickel plated brass and offer high levels of accuracy and ease of operation. They incorporate both sintered bronze filters and pressure relief valves, minimising the risks of contamination and increasing the level of carer and patient safety. Models are manufactured with two or nine pre-adjusted flow rate settings. Optional quick release BS 5682 Schrader sockets are available on some models. Fir tree outlets unscrew to leave 9/16” outlets compatible with most types of accessories eg. humidifiers.

The Mediselect range of therapy regulators operate on the indirect single stage fixed pressure principle. These cylinder-mounted regulators (for use with oxygen, and air) are pre-set to 4 bar during production and locked with a tamper proof seal. These different gas version medical regulators can be used for hospital and emergency use.A pressure gauge is incorporated to register the pressure of gas entering the regulator, giving indication of the cylinder contents.All regulators are supplied with a 5/8” BSP Bullnose inlet connection, standard 3/8” or quick release BS5682 Schrader socket outlet connection and a pressure relief valve.

The Medievac+ vacuum regulator system includes an optional accessory, the safety jar. It is an additional protection of the vacuum regulator and the hospital vacuum network, when the collection jars overflow. The filling capacity of 100 ml and the safety valve function, provide the user with extra time to stop the suction therapy.

The jar can be easily and safely disconnected from the vacuum regulator and autoclaved at 134 °C for 18 minutes, in line with hospital protocols. GCE Mediline also recommends the use of the front mounted filter that is connected on the safety jar for increased safety. The plastic shell of the filter is very convenient to mount; it enables hygienic handling as direct contact with the membrane is avoided. The use of this filter is also recommended by the standard (EN ISO 10079-3 part 6.5.2.1). Medievac+ compliance is based on EN ISO 10079-3 standard.

During exhalation oxygen collects in a reservoir. At the start of the inhalation, the diaphragm detects the negative pressure in the nose. This opens a valve delivering the oxygen to the patient at the start of the breath.The oxygen delivered correlates closely to the established method of constant flow across a range of breathing rates.The Sabre Elite automatically adjusts oxygen delivery on a given setting to the patient’s breathing rate. For higher breathing rates there is less time for the oxygen to collect. The patient will therefore get smaller pulses, but more of them. For lower breathing rates there is more time for the oxygen to collect. The patient will therefore get larger pulses, but less of

them.A simple way of looking at oxygen savings by considering efficiency. The % efficiency is calculated by dividing the oxygen delivered to the lung by the total flow delivered. In constant flow delivery only a small proportion of oxygen gets into the lung where it can be absorbed. About 60% to 80% of the oxygen delivered is wasted.Efficiencies are in the region of 20% to 40%.

The pulse of oxygen is at the start of the breath where the gas is mixed with air that goes into the lung to be absorbed.During the later part of inhalation (where the oxygen would otherwise go into dead-space) and duringexhalation the Sabre Elite saves the gas. The oxygen saving factor is primarily derived from efficiency, but other factors are relevant. The traditional constant flow settings of 2 and 4L/m are limited. By having a large range of settings with smaller increments significant further savings are possible.

The Sabre Elite has a saving factor of approximately 3 based on efficiency. If the incremental saving of having a wide range of settings that will accurately meet the patient’s need is taken into account, a saving of 3 × 1.6 = 4.8 may be claimed.

The ECOlite 4000 is an electronic gas-conserving device with built in alarm functions. It operates with a gas cylinder set at an outlet pressure of 1.6 - 5 bar. An optional 1.6 bar outlet pressure version is available for use with a liquid oxygen container.The oxygen flows through a pressure-reducing valve before entering the ECOlite 4000 and then onto the patient via a nasal canula. EAsE IIThe New Sabre EASE II portable systems provide a compact low resistance method of self-administering O2 or O2/N20. The EASE demand valve is constructed in a way that creates minimal breathing resistance to the patient and can deliver high flows when required.

For greater user comfort the EASE II is both smaller and lighter than the original Sabre EASE. The new ‘Easy Grip’ handle is another user friendly addition.

The EASE II system is available to deliver Nitrous Oxide/ Oxygen upon demand. It is available for use as a portable system with first stage regulator, or as a low pressure pipeline version. The EASE is designed to administer effective pain relief within a wide range of medical situations including immediate care, general practice and general nursing obstetrics.

Conforming to BS 4272: Part 2:1996, the demand valve is designed to deliver up to 300 litres per minute of gas with minimal inspiratory effort by the patient. This valve also contains a removable patient valve that houses the exhalation flap and anti contamination valve. This can be removed and cleaned before replacement and re-use.

AMbULAnCE pAnEL IIThe Ambulance Panel ii is the next generation of ambulance panels. it is designed for permanent installa-tion and use in road ambulances. l ong experience in this market has resulted in a product that gives the customer a lot of options and flexibility.

FLexiBiLityDue to the design and functions there are a big range of variants available. The functions and variants can be combined upon customer’s request.

MODuLArThe modular concept means that same components can be used for many variants, resulting in short lead times and full fl xibility. The Ambulance Panel II is designed with same shape for recessed and exposed.

reCesseDRecessed mounting is when the ambulance panel is mounted in the wall. The inlet could either be mounted at the end or at the back depending on the wall construction and the space behind the wall. The only part that will be outside the wall is the quick connector.

exPOseDExposed mounting is when the ambulance panel is mounted on the wall in the ambulance. The inlet could as the recessed, also be located at the end or at the back. When using the end inlet the hoses are fastened on the wall and fully visible. For some countries and regulations this is a must and the Ambulance Panel II has the features complying to these requests. The panel is fastened easily by 4 screws. Depending on the wall construction the fastening could either be done by a counter fastening frame or nuts and washers.

inLetThe inlet connection the the Ambulance Panel II is standard 3/8 thread. This is a standard thread with conical sealing that has been used many times in this kind of application. Using a threaded connection gives the assembly easier and the parts easy to maintain and exchange.

quiCk COuPLinGsThere are many variants of quick couplings on the Ambulance Panel II. The most frequent standards are used on the Ambulance Panel II. For special quick couplings please contact GCE. By using standard components and quick couplings the assembly is easy and the maintenance access able and effective.

OutLetThe outlet is standard 3/8 thread connection. If additional panels are needed, the Ambulance Panel II can have extra outlets for panels mounted in series. The panels are then connected with a standard hose and hose connections.

MAnuAL switCh-OverThe manual switch-over is a selector that chooses what cylinder is supplying the ambulance panel. There are three steps 1-0-2, where the 0 is off 1 is cylinder one and 2 cylinder two. The manual switch-over can be incorporated in the panel beside the other quick couplings or it could be mounted separately.

Manual switch-over

GAuGeOne of the additional functions in the Ambulance Panel II is the gauge on the panel. The gauge is showing the working pressure in the system and gives continuously surveillance and is fully visible by the staff in the ambulance.

FLOwseLeCtOrThe AP II module can be equipped with a flow selector. The flow selector is permanently attached to the AP II module and can be delivered with the flow settings 0-15 and 0-25 liter per minute. This integrated module gives the AP II a compact design and minimize the products sticking out from the wall.

suCtiOnThe integrated suction ejector is a variant in the AP II system giving the end user a complete range of functions. The suction ejector uses the pressure in the system to create suction by the venture principle. The unit is operated by a simple on of button and a regulating knob to adjust the negative pressure.

GSS automatic gas source switch switches automatically between the gas cylinder supply and wall outlet connection. Preference is always given to the more economic wall outlets. As soon as the gas socket is connected, GSS changes over from the wall cylinder to the gas outlet. With Sabre MARS the operator does not have to make judgements on the lung capacity of the patient to select a tidal volume; only the basic size or type of patient being cared for. Once this simple operation has been made the MARS will control both tidal volume and pulse frequency within the parame-ters laid down in the British Standards.Even if the wrong setting is selected there is no risk of over inflating the patient’s lungs. The MARS always ‘fails safe’ and will vent excess gas whilst giving the operator an audible warning that the equipment is not being used correctly.
