Mansfield District Council provides Bereavement services which cover four cemeteries, closed churchyards and a joint involvement in the Mansfield & District Crematorium. The staff provide a caring and sensitive service for people at a difficult time. The Bereavement Service staff are able to give helpful advice and guidance relating to all aspects of funerals and memorialisation. This brochure has been produced to provide more information of the service that we provide. When someone dies, you will need to inform a number of people and organisations and complete

certain documents needed by law. If you are a relative or friend you can do some of these things

yourself. Others will need to be done by the executor or administrator of the estate. There is plenty

of support available to help you through this difficult time ( please see page 14 for details of how we

can help).

What to do in the first few days You won't be able to do everything right away, but in the first few days it's important, if you can, to:

tell the family doctor, contact a funeral director, if you intend to use one ‐ if the death occurs at

home or in a nursing home the funeral director will need to be contacted to move the deceased to a

Chapel of Rest and begin arrangements for the funeral ‐ you should check the will for any

special requests.

What documents will you need when registering a death? When you go to register the death you must take with you: The medical certificate of the cause of death, signed by a doctor unless the coroner is issuing the paperwork. If available take the deceased's birth certificate, marriage/civil partnership certificate and NHS Medical Card.

What documents will you receive from the registrar?

After the information has been recorded into the death register, the registrar will issue

the necessary forms and certificates.

If a post‐mortem is not being held, the registrar will give you:

• A Certificate for Burial or Cremation (called the 'green form’) giving permission for the body

to be buried or for an application for cremation to be made

• If the deceased is to be buried or cremated outside of England or Wales the coroner will

issue the necessary forms

• A Certificate of Registration of Death (form BD8); issued for the Benefits Agency.

You will be able to buy one or more death certificates. These are certified copies of the original

register entry. These will be needed by the executor or administrator when sorting out the

deceased person's affairs and need to be originals not photocopies. Anything that has to be

closed down or claimed will need a certificate. Most companies will return the certificate once

they have had sight of it.

The registrar will also give you a booklet called 'What to do after a death', offering advice on

probate and other administrative issues that will need to be sorted out around this time.

What must you do when someone dies continued? • obtain a Medical Certificate of Cause of Death signed by a doctor or if the coroner is involved

take instructions from the coroner's officers regarding registration of the death

• register the death at the Register Office. You'll find details of how to do this in the section on

registration

• if relevant, complete the reverse of the white BD8 form and send to the local Jobcentre Plus or

Benefits Agency (given when you register the death)

As early as possible: • contact the executor as soon as you can to enable him/her to start the process of obtaining

probate if necessary. The executor is usually nominated in the will

• if there is no will, decide who will apply to sort out the deceased's affairs and contact the

Probate Registry to apply for 'letters of administration' if necessary

Documents you will need in the first few days You'll need to gather together the following documents as soon as possible ‐ to enable registration of

the death and to start funeral arrangements.

Documents • Medical Certificate of Cause of Death, signed by a doctor unless the coroner is involved and there

has been a coroner's post mortem

• birth certificate *

• marriage/civil partnership certificates *

• NHS Number/NHS Medical Card *

• organ donor card (if appropriate)

It is not necessary to have these documents but you will need to provide information contained

within them, e.g. date and place of deceased's birth, spouse/civil partner's full name, etc.

How do you register a death?

How and where you can register a death, and information you will need to provide

A death must be registered within five days from when it occurred. This period can be extended in

exceptional circumstances and if the coroner is involved. The registration must take place in the

district where the death occurred.

If it is difficult for you to get to the appropriate register office, you may visit your local office and

declare the necessary information. In this case the registration by declaration may result in a delay in

the issue of the document needed for the funeral arrangements. If you need further advice and help

Mansfield & District Crematorium The Chapels Mansfield & District Crematorium was officially opened in June 1960 as a joint venture between Ashfield, Mansfield and Newark & Sherwood District Councils. It has two chapels, Thoresby which seats 80 persons and Newstead Chapel seats 40 persons. We allow 30 minutes for the actual service and services are timed at 40 minute intervals to allow for the movement of mourners, however if this is not long enough for the proposed service then it is possible to book an additional service time for a nominal fee or to use the drop down screen in the Newstead Chapel so that the service can be relayed into there should you have a large number of mourners. It is possible to either hold the whole service at the crematorium, or have a service elsewhere and the committal at the crematorium. The staff will assist in any way possible to ensure that families can personalise their loved ones service and we have an extensive music portfolio for this as well as welcoming photos, orders of service, different modes of transport and coffin, live music etc. Both chapels have loop systems and disabled access and toilets.

The Garden of Remembrance

The Garden of Remembrance provides a peaceful and tranquil place in which to gather your thoughts and also, if you wish, to have your loved one’s remains scattered in the woodland area. A record is kept of the general location in which the cremated remains are placed which also means that in future it is possible to arrange for a loved one’s remains to be placed in the same general area as those of a previous loved one. No memorials are permitted in the woodland area.

If family wish, memorial trees, shrubs, bronze kerb plaques are memorials for their loved ones in the Garden of Remembrance to the building. For further information please contact a member of staff who will explain the procedure to you.

Floral tributes left in the Gardens of Remembrance are usually cleared on a Wednesday morning in order to preserve the beauty of the grounds. In the event that you might wish for your floral tribute to be left longer than that date it is essential that you notify the office so it can be noted. On Mothers Day, Valentines Day, Grandparents Day and Remembrance Sunday floral tributes are automatically left for two weeks. Christmas wreaths are left until the last day of January when the grounds are cleared. It is important to note that we do have a large wildlife population of both squirrels, rabbits and so prickly foliage will help protect your floral tributes from being eaten or played with.

The Book of Remembrance The Books of Remembrance are on view daily, providing a lasting memorial to your loved one. The name of the deceased and a suitable epitaph may be recorded under the date of death. The pages will be turned daily so that inscriptions may be viewed. The lettering is hand‐written in the finest traditions of mediaeval illuminated manuscripts and the volumes are richly bound and tooled. A digital Book of Remembrance is also available which allows you to see your loved ones entry whenever you want and also allows it to be viewed on the internet — details can be provided by the Bereavement Service staff.

Other Memorials Other forms of memorialisation in the Gardens of Remembrance can also be provided to the next of kin or with their written permission. These can include Bronze kerb plaques which are located around the pond area, plaques in front of flowering shrubs and trees or memorial vase blocks (as per picture) which provide a means of remembering a loved one through the provision of a memorial vase and inscribed plaque. Details of these can be obtained by contacting Bereavement Services staff.

Columbarium Vaults These form a memorialisation located in a secluded woodland setting at the crematorium provides a means of temporary repository for your loved ones remains. Forming a more traditional headstone type of memorial they provide a repository for two sets of remains on a lease basis as well as providing a flower vase for floral display and inscribed front facing plaque which can have a photo plaque attached. Decisions relating to the purchase of the lease on such a memorial need to be considered before a funeral takes place to ensure that clear instructions are given relating to final disposition of the cremated remains.

In all instances where the exclusive right or burial is purchased (this is not applicable for children's graves) there is a requirement for the right to be transferred to a new owner should the original owner pass away. The exclusive right of burial is usually on a 50 year lease at Mansfield and primarily gives the owner the right to state who will be interred in the grave and what memorial will go on the grave. The actual land remains under the ownership of the Mansfield District Council who will carry out maintenance on the land. At the end of the 50 year term the Local Authority has a duty to attempt to notify all owners and see if they wish to re‐new the lease if the lease is not renewed then the rights transfer back to the Local Authority. In all instances where the right is purchased the registered grave owner will receive a Deed to the grave. This is a legal document and should be kept in a safe place. Bereavement Services must be contacted for advice if it is wished to transfer the Deed to a new owner.

Mansfield Woodhouse Cemetery Mansfield Cemetery Chapel Whilst Mansfield District Council maintain the Welbeck Road closed churchyard it is the Cemetery on Leeming Lane which was opened in 1910 which is currently used and is the final resting place of many well known local families who have shaped Mansfield Woodhouse. Leeming Lane also offers woodland burial facilities and has a chapel. This cemetery has recently undergone improvements to its infrastructure as part of our commitment to ensuring the facility is a fitting resting place for your loved ones. Pleasley Hill Cemetery A small village like cemetery which was opened in 1897 which still offers a community feel. Warsop Cemetery Opened in 1917 and comprising once again a community feel. This cemetery has recently been doubled in size and is in the process of having improvement works to its infrastructure.

Mansfield Cemetery Mansfield Cemetery was opened in the 1870’s and is the final resting place of many prominent people who have helped shape Mansfield. The Victorian section of the cemetery is home to some very important and interesting memorials and is a Grade II listed Garden of Special Interest due to a lot of the original trees still being in situ. There is a beautiful double chapel in Mansfield Cemetery which is ideal for small intimate funeral services or for wedding blessings. This cemetery is currently undergoing a massive refurbishment of its infrastructure.

The Cemeteries Mansfield District Council manages and maintains four cemeteries within its environs offering a full range of internment options including woodland burial, (where you can have full kerbsets around the perimeter of the grave) lawn burial (where you may have just a headstone and nothing in front of that) cremated remains grave (these are half the size of a full grave and take a minimum of four sets of cremated remains allowing you to have a headstone only) and children's graves (again of lawn style but in designated areas specifically reserved for children). Family Research Bereavement Services receive many requests from people researching their family history. The registers for Mansfield District Council Cemeteries and Mansfield & District Crematorium are held at the Mansfield & District Crematorium office. Bereavement Service staff will happily conduct searches on your behalf and it is advisable to leave a list of details with them so they can give you all the relevant information.

Safety Inspections of Memorials Bereavement Services are currently carrying out safety inspections on all memorials within Mansfield and area Cemeteries. The inspections are being undertaken in accordance with advice issued to local authorities by the Health & Safety Executive and the recent Memorial Safety Guidance 2009 issued by the Ministry of Justice. Whilst every effort will be made to ensure that all memorials are left in a safe and secure way, occasionally a memorial may have to be cordoned off for a period of time. A yellow safety‐warning label is fixed to any memorial that is found to be unsafe. This is to advise owners to contact Bereavement Services and not to touch the memorial unless they are having the memorial repaired by a qualified memorial mason. Any new memorial must also be fixed with a ground‐anchor system. For further information on the memorial safety inspections please contact a member of the Bereavement Staff at the Mansfield Cemetery Office.

Burial Services A full service may be held in the Cemetery chapel, followed by the burial, or alternatively a service could be held in a church or other venue and the coffin brought to the cemetery for burial only. The Cemetery chapel is non‐denominational and are available to all religions, sects and beliefs. Bereavement Services staff will prepare the grave for the burial, and a Burial Attendant will ensure that the service takes place safely and smoothly. Following the burial, when the mourners have left, the grave will be back‐filled and re‐instated with turf or grass seed, depending on the season. The grave will be topped up and re‐turfed or seeded if necessary. The grave will be maintained as part of the general maintenance of the cemetery, further details are available from the Bereavement Services Office.

Graves

Burials take place in graves, which are laid out in regular pattern within the cemeteries. A grave may either be un‐purchased, which means that the rights to it remain with the Council, or purchased, which means that the right to bury in the grave and to place a memorial on it are transferred to the owner. The ownership of the land does not transfer ‐ the Council owns all the land within the cemeteries. At the end of the period the lease may be renewed, otherwise the rights transfer back to the Council. A grave may be purchased in advance of need, or it may be purchased subsequent to the burial. The grave owner receives a Deed to the grave, which is a legal document and should be kept in a safe place. Bereavement Services should be contacted for advice on the procedures required if it is wished to transfer the Deed to a new owner.

What happens if the death is referred to a coroner? If a death is reported to the coroner which does not need to be the subject of an inquest (when death is a

result of natural disease or illness), a certificate giving the cause of death will be sent to the registrar of deaths

on completion of the coroner's enquiries. You can then go ahead and register the death.

In a small number of cases ‐ where the cause of death is unclear, sudden or suspicious ‐ the doctor or

hospital or registrar will report the death to the coroner. In this case registration of the death will be

delayed as an inquest will need to be held.

Inquests: It is the duty of coroners to investigate deaths which are reported to them and which:

• appear to be due to violence

• are unnatural

• are of sudden and of unknown cause

• occur in legal custody

The coroner will preserve confidentiality as far as possible but you should remember that the system is based

on public court hearings. If you request it, the coroner will explain the reasons for the procedures adopted in

particular cases as long as the coroner is satisfied that the person has a proper interest and a right to know.

An inquest is not a trial. It is an enquiry to establish who the deceased was and how, when and where they

died. After the death the coroner will issue an interim death certificate to enable the estate to be dealt with.

On conclusion of the inquest, the next of kin will be provided with an explanation about how, where and

when a copy of the death certificate can be obtained.

Registering a stillbirth How is a stillbirth defined? A stillborn child is legally defined as a child born after the 24th week of pregnancy who did not, at any time after being born, breathe or show any other signs of life. What do you need to do to register a stillbirth, either inside or outside Mansfield District Council. When a child is stillborn, a doctor or midwife will issue a medical certificate of stillbirth. The person who registers the stillbirth must take this certificate to the registrar. Every stillbirth in England or Wales must be registered in the district in which it takes place. A stillbirth may not be registered more than three months after it occurred. If the stillbirth occurs in Mansfield District Council you will be able to make an appointment to attend any of the registration offices in the county. For more information call the Registration Service helpline. If you live outside Mansfield District Council or if the still‐birth occurs outside Mansfield District Council and it is not convenient for you to register the stillbirth in Mansfield District Council, you may prefer to make a declaration of the stillbirth to your local register office. Staff from your local register office will be able to offer you advice on the procedure for this and you can contact them by looking in your local phone book. Please note that registration by declaration does result in a delay in the issue of the document needed for the funeral arrangements.
