completed Bloodborne Pathogen Quiz and Hepatitis B Vaccination Consent or Declination Statement form. These are then to be submitted to the School Nurse assigned to the employee’s work site.
All employees participate in annual review of work practice controls and procedures to minimize transmission of communicable disease/illness in the school setting.
Training Quiz and Vaccination Consent/Declination FormPlease complete both forms and turn them
into the School Nurse assigned to your work site.
Please contact your School Nurse if you have any questions regarding this training or anytime you have a question or concern related to the content of this training.
