Conducting the Practice Survey

The survey questions were designed to reflect the feedback received from the Patient Reference Group

The survey was open from December 2013 to March 2014. 114 patients completed the survey. (35 more responses than previous year)

Patients were made aware of the survey on the electronic display in the surgery, notices attached to prescriptions, via the practice web site and members of staff encouraged patients to participate.

For patients unable to access the internet, paper versions were available at reception, and help was offered to those who needed it to fill out the forms.
