Lifting Equipment Failure

We recently suffered two events using mechanical lifting eyes.

Both events had very similar immediate, underlying and root causes.

Both involved lifting equipment with lifting eyes

Both raised the load to a significant height

Both had issues with the threaded holes

Both had a dropped object resulting in a significant near hit

Recommendation

All lifting eyes must have WLL, unique ID and correct colour code

Recommendation

All dynamo lifting eyes to be removed from equipment onboard the rig

All suppliers to be instructed not to send equipment with lifting eyes attached
