REQUIREMENTS FOR THE LICENSING OFPESTICIDE EXPORTER

1. Duly accomplished and notarized application form (original copy) withdocumentary stamps required.

2. For: a. Corporation/Partnership - Copy of SEC Registration b. Cooperative - Copy of CDA Registration

c. Single Proprietorship - Copy of Certificate of Registration of Business Name with DTI3. Copy of latest Income Tax Return (ITR) and Financial Statement (FS).4. The exporter submits to the authority the supply-demand analysis of theproduct/s intended to be exported to insure that exportation is made onlyafter satisfaction of local demand.

5. The exporter submits a letter from the Department of Agriculture orrelevant Regulatory Agency of the importing country to justify that theproduct to be exported is not a banned nor restricted and allowed to beexported to the importing country.

6. Product Registration of pesticide to be sold.7. Schedule of fees (See Attached).

A license good for one (1) year shall be issued upon approval ofapplication.

REQUIREMENTS FOR THE LICENSING OFPESTICIDE IMPORTER - DISTRIBUTOR

1. Duly accomplished and notarized application form (original copy) withdocumentary stamps required.

2. For: a. Corporation/Partnership Copy of SEC Registration

b. Cooperative Copy of CDA Registration

c. Single Proprietorship Copy of Certificate of Registration of Business Name with DTI 3. Copy of latest Income Tax Return (ITR) and Financial Statements (FS). 4. Distributorship Agreement/ Certification from the pesticide supplier. 5. Recommendation/ Inspection Report from FPA Regional/ Provincial

Officer. 6. Registration of Pesticide Warehouse/s.

7. Xerox copy of Responsible Care Officer (RCO) /Accredited ResponsibleCare Officer (ARCO) ID. 8. Product Registration of pesticide to be sold. 9. Schedule of fees (See Attached). A license good for one (1) year shall be issued upon approval of

application.
