To begin the background screening process, have the applicant fill-out and sign a consent

form. Church Safety Central has included a sample consent form on their site for you to model. The directions for locating the CSC Consent Form are below:

Creating a New Report

The purpose of this tool is to help you create a new order.

1 Click on “Create New Report” link on the left side of the

screen.

The next screen asks you to provide the applicant’s general information.

General Information

As you tab through the fields, the text defaults to upper case. The program will populate

the fields with the appropriate slashes and dashes. Once you have populated this screen,

read over the information to make certain it is typed correctly. Notice that the navigation menu on the left side of the screen is highlighted and will continue to remain highlighted as you progress through each of the searches.

At the bottom of the screen there are two optional fields, a “Reference Code” and a “Comments” field. The “Reference Code” will allow you to

connect this report with a specified piece of data such as an internal billing

code, a department name or job title. Essentially, you can populate this field

with whatever data would be helpful in identifying this candidate.

The “Comment” field is on every page within this web application.

Remember, when utilizing this menu, that each package set up for you is associated

with a price. So, if you remove the checkmark to the left of a product that is part of

a package, it will not reduce the cost of the report. You can add products to a

InstaCheck® Results

The purpose of this tool is to help provide identification and additional addresses if we find

an association between the applicant’s name and the social security number he/she

provided.

Here is a list of the results you may see while inside of the InstaCheck®

feature:

“Verified” This is the most common result. It means we were able to find an association

between the candidates name and the social security number he/she provided.

“Has been issued by the Social Security Administration”

This result means we were able to determine the social security number provided is

valid; however, we could not find an association between it and the name the

applicant provided.

There are three reasons why

this screen may appear: 1. The candidate is new to the

country and has no public

record tied to him/her.

2. The candidate is fairly young

and has no public record tied to

him/her.

3. Our data may not be correct or

complete and you should ask

for additional identity

documents from the applicant

such as a Social Security Card,

Passport, etc.

There are three (3) results that

you may see that will raise a red flag to you. They are: 1. “Social Security Number was

issued to someone who is

deceased”

2. “Social Security Number was issued prior to the applicant’s date of birth”

3. “Social Security Number has never been issued”

IMPORTANT: InstaCheck®

is provided for identity verification and fraud detection purposes only. This information is not a

consumer report under the Fair Credit Reporting Act and may not be used as a factor in evaluating an individual for employment,

promotion, reassignment, or retention as an employee. Moreover, this information may contain errors. Recent moves and data

entry errors can cause inconsistencies and inaccuracies when comparing applicant information to database information.

In each of these cases, check your data entry and close the wizard to discuss the

results with your applicant when appropriate. The reports that were closed will now

be housed in the “Unsent Reports” section.

County/Federal Criminal Search

The purpose of this search is to help identify the counties associated with the search

subject. Underneath the counties, there are multiple addresses. These are the addresses

the applicant may have been associated with in that county. To the right of the county, is a

number of days, surrounded by parenthesis. This represents the average turn-around time

in that particular county.

To add an additional city or county to your search, populate the fields at the top of the

screen and click on the “Apply” button. The information you provided is added underneath

the county of current residence. If you provided a city, we will find the associated county

and run the search.

BE AWARE: You are charged for each county you select. The only county that is

automatically selected is the one associated with the address you entered on the

general information screen.

Name Selection This screen allows you to select which name or names you would like to submit for

research. Toward the bottom of the screen, there is a list of names. The only alias that is

automatically selected is the name that was provided on the general information screen. To

the right of the name is the word “Current”, which simply indicates that it was provided on

the general information screen.

The other names listed in this table are those found associated with the applicant through

the InstaCheck®

feature. Typically, the results will include the person’s name you provided

on the general information screen and that same name without the middle initial.

The name you should submit to LexisNexis Full Service Screening is the name with

the most information or the person’s full legal name. For example, William L. Smith provides more information than William Smith.

You can add additional names by populating the fields at the top of the screen and clicking

on the “Apply” button. You should then see the additional alias in the table with a

checkmark to the left.

BE AWARE: You will be charged per name, per county that you select. Be careful to

select the only names you are interested in searching.

Driving Record

The purpose of this tool is to allow you to search for moving violations. The name and date

of birth that was provided on the general information screen is automatically populated. At

this point, you will need to provide us with the name exactly as it appears on the

individual’s driver license. This is important, as there are certain states who will return the

request as “not found” if the name you provide does not exactly match what they have on

file.

If the state you requested is available instantly, you will be able to view this request in your

“Pending Reports” menu once you finish entering the applicant.

Step Action

1 Enter the name exactly as it appears on the driver’s license.

If you completed Step 1 on the General Information screen, you will only have to add the driver’s license number

and the state.

Multi-State Court Search (formerly National Criminal Search)

The purpose of this screen is to enter the names to be searching using the Multi-State Court

Search (formerly National Criminal Search). The name you provided on the general

information screen is populated in the First name and Last name fields on this screen as

well as the “Names Selected” text box. The only name we will automatically search

is the name you provided.

If you entered additional aliases in the names selection screen earlier in the

process, you will need to duplicate those entries here. Do this by changing the

name in the first name and last name fields and click on the “Apply” button.

You will see the alias you entered with a checkmark to the left of it in the “Names Selected” box.

BE AWARE: You will be charged for each name you submit.

The “Multi-State Court Search” page will appear and you will be provided with an

instantaneous result. If the applicant has no record in this file, you will see a “Clear”

result. If the candidate has a record, you will see “Record” as the result. Records are

returned if we are able to find potential matches between the data in our files and the

applicant. You will also see the data accompanying the record we found such as

jurisdiction, offense, charge date, sentencing information, etc.

Search Summary The “Search Summary” screen appears and list the searches that you requested. Look at

this list closely. The number of each product you requested is to the left of each product.

If this number is incorrect, you can click on the “Previous” button and change the

selection requirements before sending the report to LexisNexis Full Service

Screening.

BE AWARE: If the product is instantaneous you have already been charged.

Pending Items Review

If you look down at the bottom of the screen, you will see information regarding when this

report was submitted, what searches were selected, and when the report will be complete.

The time stamp on each report is in Eastern Time.

LexisNexis will send you an email to let you know when this report has been completed.

The report that you submitted will be at the top of the

screen.

2 Click on the “Partial” link.

This will allow you to view the report in its partial status.

Folder Descriptions

�Completed Reports Folder

This is the default folder for this application. This menu allows you to view available

reports that have been sent back from LexisNexis Full Service Screening. To the right

of the applicant’s name, under “View”, you will see a link titled “View”, which you

can click on in order to view the background report.

�Unsent Reports Folder

The “Unsent Reports” is essentially a draft folder. If you are in the middle of

entering an applicant and find you do not have the information you need, or if you do

not have time to complete an entry, you can close the wizard and the draft will be

saved here. You will want to keep track of what is in the menu as it has not been

sent to LexisNexis Full Service Screening for processing and is waiting work on your

side.

�Pending Reports Folder

This menu allows you to keep track of those reports which have been successfully

sent to LexisNexis Full Service Screening, but are waiting work on our side. You can

use this menu to keep track of individual applicants. There is also a link you can

click on, which allows you to view a report in its partial status.

�Deleted Folder

The “Deleted Reports” menu allows you to view the names of the individuals you

have deleted from your inbox. Once you have deleted a report, you will only be able

to see the name of the applicant, his or her social security number and the date the

report was completed in your “Deleted Reports” menu for tracking purposes. The

actual report itself is removed.

Remember, when utilizing this menu, that each package set up for you is associated

with a price. So, if you remove the checkmark to the left of a product that is part of

InstaCheck® Results

The purpose of this tool is to help provide identification and additional addresses if we find

an association between the applicant’s name and the social security number he/she

provided.

Here is a list of the results you may see while inside of the InstaCheck®

feature:

“Verified” This is the most common result. It means we were able to find an association

between the candidates name and the social security number he/she provided.

“Has been issued by the Social Security Administration”

This result means we were able to determine the social security number provided is

valid; however, we could not find an association between it and the name the

applicant provided.

There are three reasons why

this screen may appear: 1. The candidate is new to the

country and has no public

record tied to him/her.

2. The candidate is fairly young

and has no public record tied to

him/her.

3. Our data may not be correct or

complete and you should ask

for additional identity

documents from the applicant

such as a Social Security Card,

Passport, etc.

There are three (3) results that

you may see that will raise a red flag to you. They are: 1. “Social Security Number was

issued to someone who is

deceased”

2. “Social Security Number was issued prior to the applicant’s date of birth”

3. “Social Security Number has never been issued”

County/Federal Criminal Search

The purpose of this search is to help identify the counties associated with the search

subject. Underneath the counties, there are multiple addresses. These are the addresses

the applicant may have been associated with in that county. To the right of the county, is a

number of days, surrounded by parenthesis. This represents the average turn-around time

in that particular county.

To add an additional city or county to your search, populate the fields at the top of the

screen and click on the “Apply” button. The information you provided is added underneath

the county of current residence. If you provided a city, we will find the associated county

and run the search.

BE AWARE: You are charged for each county you select. The only county that is

automatically selected is the one associated with the address you entered on the

general information screen.

Name Selection This screen allows you to select which name or names you would like to submit for

research. Toward the bottom of the screen, there is a list of names. The only alias that is

automatically selected is the name that was provided on the general information screen. To

the right of the name is the word “Current”, which simply indicates that it was provided on

the general information screen.

The other names listed in this table are those found associated with the applicant through

the InstaCheck®

feature. Typically, the results will include the person’s name you provided

on the general information screen and that same name without the middle initial.

The name you should submit to LexisNexis Full Service Screening is the name with

the most information or the person’s full legal name. For example, William L. Smith provides more information than William Smith.

You can add additional names by populating the fields at the top of the screen and clicking

on the “Apply” button. You should then see the additional alias in the table with a

checkmark to the left.

BE AWARE: You will be charged per name, per county that you select. Be careful to

select the only names you are interested in searching.

Driving Record

The purpose of this tool is to allow you to search for moving violations. The name and date

of birth that was provided on the general information screen is automatically populated. At

this point, you will need to provide us with the name exactly as it appears on the

individual’s driver license. This is important, as there are certain states who will return the

request as “not found” if the name you provide does not exactly match what they have on

file.

If the state you requested is available instantly, you will be able to view this request in your

“Pending Reports” menu once you finish entering the applicant.

Step Action

1 Enter the name exactly as it appears on the driver’s license.

If you completed Step 1 on the General Information screen, you will only have to add the driver’s license number

and the state.

Multi-State Court Search (formerly National Criminal Search)

The purpose of this screen is to enter the names to be searching using the Multi-State Court

Search (formerly National Criminal Search). The name you provided on the general

information screen is populated in the First name and Last name fields on this screen as

well as the “Names Selected” text box. The only name we will automatically search

is the name you provided.

If you entered additional aliases in the names selection screen earlier in the

process, you will need to duplicate those entries here. Do this by changing the

name in the first name and last name fields and click on the “Apply” button.

You will see the alias you entered with a checkmark to the left of it in the “Names Selected” box.

BE AWARE: You will be charged for each name you submit.

The “Multi-State Court Search” page will appear and you will be provided with an

instantaneous result. If the applicant has no record in this file, you will see a “Clear”

result. If the candidate has a record, you will see “Record” as the result. Records are

returned if we are able to find potential matches between the data in our files and the

applicant. You will also see the data accompanying the record we found such as

jurisdiction, offense, charge date, sentencing information, etc.

this list closely. The number of each product you requested is to the left of each product.

If this number is incorrect, you can click on the “Previous” button and change the

selection requirements before sending the report to LexisNexis Full Service

Screening.

Pending Items Review

If you look down at the bottom of the screen, you will see information regarding when this

report was submitted, what searches were selected, and when the report will be complete.

The time stamp on each report is in Eastern Time.

LexisNexis will send you an email to let you know when this report has been completed.

Folder Descriptions

�Completed Reports Folder

This is the default folder for this application. This menu allows you to view available

reports that have been sent back from LexisNexis Full Service Screening. To the right

of the applicant’s name, under “View”, you will see a link titled “View”, which you

can click on in order to view the background report.

�Unsent Reports Folder

The “Unsent Reports” is essentially a draft folder. If you are in the middle of

entering an applicant and find you do not have the information you need, or if you do

not have time to complete an entry, you can close the wizard and the draft will be

saved here. You will want to keep track of what is in the menu as it has not been

sent to LexisNexis Full Service Screening for processing and is waiting work on your

side.

�Pending Reports Folder

This menu allows you to keep track of those reports which have been successfully

sent to LexisNexis Full Service Screening, but are waiting work on our side. You can

use this menu to keep track of individual applicants. There is also a link you can

click on, which allows you to view a report in its partial status.

�Deleted Folder

The “Deleted Reports” menu allows you to view the names of the individuals you

have deleted from your inbox. Once you have deleted a report, you will only be able

to see the name of the applicant, his or her social security number and the date the

report was completed in your “Deleted Reports” menu for tracking purposes. The

actual report itself is removed.
