actividad investigadora y comercial de LETI ha

estado ligada al campo de la inmunología.

Muestra destacada de esta vocación es la

División de Alergia, creada en 1941, que pone a

disposición del especialista los más avanzados

productos para el diagnóstico y tratamiento

etiológico de los procesos alérgicos.

Since its foundation in 1919, a significant part of

the scientific and commercial activity of LETI

has been related to the field of immunology.

A major outcome of this vocation is the Allergy

Division, created in 1941, which offers allergy

specialists a complete portfolio of the most

advanced products for the diagnosis and etiological

treatment of allergy.

LETI bases the manufacturing and standardizationof its allergen extracts on the recommendations ofthe Nordic Council on Medicines(1) and follows theSpanish regulations for immunological products forhuman use (Royal Decree 288/1991 BOE 12/3/91)and the Note for Guidance on Allergen Products(CPMP/BWP/243/96) of the European Union.

The processes and procedures of manufacturing andcontrol comply with the criteria of Good ManufacturingPractice criteria (GMP) and have ISO 9001 Certification.

LETI, en la fabricación y estandarización de los extrac-tos alergénicos, sigue las regulaciones españolas paraproductos inmunológicos de uso humano (Real Decre-to 288/1991 BOE 12/3/91), las directrices para produc-tos alergénicos (CPMP/BWP/243/96) de la UniónEuropea y las recomendaciones elaboradas por elNordic Council on Medicines(1).

Los procesos y procedimientos de fabricación y control seajustan a los criterios de las Normas de Correcta Fabrica-ción (GMP) y están avalados por la certificación ISO 9001.

Several methods have been used to evaluate thepotency of allergen extracts. The methods based onquantifying the response of the skin in vivo are thebest documented and most reliable for determiningthe biological activity of extracts(13).

Following the recommendations of the EAACIImmunotherapy Subcomitee(14) and the AmericanAcademy of Allergy, Asthma and Immunology(AAAAI)(15), the potency of the reference standardsis based on the total allergenic activity. LETI standardises its reference extracts in vivo bymeans of dose-response bioassays and the newpreparations of allergen extracts are comparedwith these reference extracts by means of in vitrobioequivalence tests.4

Estandarización biológicaBiological standardization

Para evaluar la potencia de los extractos alergénicosse han empleado numerosos métodos. Los méto-dos basados en la cuantificación de la respuesta invivo cutánea son los mejor documentados y los másfiables para determinar la actividad biológica delos extractos(13).

La Unidad de Alergia de Laboratorios LETI, siguiendolas recomendaciones del Subcomité de Inmuno-terapia de la EAACI(14) y de la Academia Americanade Alergia, Asma e Inmunología (AAAAI)(15), elaboralos estándares biológicos de referencia basándolosen la actividad alergénica total. Con este fin, estan-dariza sus extractos de referencia in vivo mediantebioensayos dosis-respuesta y las nuevas prepara-ciones de extractos alergénicos se comparan conestos extractos de referencia mediante ensayos debioequivalencia in vitro.

La unidad de valoración utilizada por LETI es la unidadHEP, cuya definición es:“La actividad de un extracto alergénico es 10 HEP(Histamine Equivalent in Prick testing) por mlcuando produce una reacción cutánea del mismotamaño que la que produce una solución positivade referencia, constituida por histamina 54,3 mmol/l(dihidroclorhidrato de histamina a 10 mg/ml),cuando ambas soluciones se administran usandola misma técnica (prick test) en un mínimo de 20individuos clínicamente alérgicos frente al extractoalergénico correspondiente”(1).

La unidad 10 HEPL se basa en la cantidad de prepa-ración de referencia interna (PRI) que produce invitro el 50% de inhibición de unión a IgE específica,usando el método REINA-Competición, y en laexperiencia en su uso clínico.

LETI uses the HEP unit, which is defined as:“the activity of an allergen extract is 10 HEP(Histamine Equivalent in Prick testing) per ml whenthe extract provokes a specific skin reaction in themedian sensitive patient with a wheal of the samesize as a wheal provoked by a positive referencesolution consisting of histamine 54.3 mmol/l (forexample histamine dihydrochloride 10 mg/ml),when both solutions are administered using thesame technique (prick testing) on at least 20individuals who are clinically allergic and cutaneouslyreactive to the allergen concerned”(1).

The unit HEPL is based on the quantity of in-housereference preparation (IHRP) that in vitro producesa 50% inhibition of specific IgE binding using anELISA test based on antigen capture (REINA-Competition) and on clinical experience.
