Established in 1993, the AstraZeneca HealthCare Foundation is a Delaware not-for-profit corporation and a 501(c)(3) entity with a mission to promote public awareness and education of healthcare issues and support nonprofit organizations consistent with its charitable purpose.

MISSION AND PROGRAMS

Connections for Cardiovascular HealthSM

In 2009, the AstraZeneca HealthCare Foundation launched the Connections for Cardiovascular HealthSM (CCH) program with a $25 million contribution from AstraZeneca Pharmaceuticals. The program awards grants to nonprofit organizations working in innovative ways to improve cardiovascular health at the community level.

National Breast Cancer Awareness Month

Now a legacy program, the Foundation has supported National Breast Cancer Awareness Month (NBCAM): a group of national public service organizations, professional medical associations and government agencies who work to improve breast cancer awareness, share disease information and provide greater access to services.

AstraZeneca Employee Disaster Relief Fund

The Foundation provides support for AstraZeneca employees affected by federally declared disasters in the United States.

The Center for Social and Economic Policy Research at West Chester University

The Foundation’s university evaluation partner, the Center for Social and Economic Policy Research at West Chester University, Pennsylvania, provides evaluation support of CCH-funded programs, identifies key learnings based on Foundation practices and provides representation on the Foundation’s grant review committees.

Across the country, cardiovascular disease affects over 92 million people and

remains a leading cause of death.1 The AstraZeneca HealthCare Foundation’s

Connections for Cardiovascular HealthSM (CCH) program seeks to address this

continuing problem. Ever since the CCH program’s launch eight years ago,

we’ve not only provided funding to organizations working in innovative ways at

the community level to help improve heart health, but also provided them with

ongoing program evaluations. More recently, we developed formal learning

opportunities to help them build capacity and sustain their programs. We have

learned along the way with our Grant Awardees and we’ve seen first-hand

the incredible impact their programs are making in the lives of participants in

their communities. Now, as the Foundation and our Grant Awardees focus on

sharing lessons learned broadly, we look forward to the impact of the CCH

program being extended to help even more individuals and organizations.

This year, the Foundation hosted two Learning Forums to support our Grant Awardees’ efforts to share valuable information from their programs and help them build capacity. We brought together representatives from our Grant Awardees’ organizations, along with our evaluation and communications partners, to engage and learn about program sustainability, dissemination and storytelling.
