INTRODUCTION• Project :- KWPCL,Raigarh• Cooling Tower :- NDCT (171 M height)• Project In Charge :- Satpal Singh Bal• Safety Officer :- Rahul Kumar• Date of Accident :- 21.07.12• Time of Accident :- 09:15 Hrs• Nature of Injury :- Fatal• No.of Fatal :- 5nos

THE INCIDENT• On 21.07.12 all the 5 workers were engaged for lift no:

105.(height:160 M)

• One of them was for curing and others for JF lifting.

• All were working on JF no:96.

• During the lifting operations,suddenly Jumpform No-96 came out of its position and fell down.

• All five people working on the jumpform met with fatal accident.

WHY HAPPENED ?• During investigation it is seen that the bottom of the upright key hole, holding the main stringer sheared off and as a result entire stringer collapsed.

• It is due to using only one pin during lifting of the jump form.

• During lifting the trolley might have stuck up and the workmen put extra ordinary pressure to lift the jump form using the chain pulley block.

• This caused overstress on the upright and it failed.

WHAT WENT WRONG?

• The procedure for jump form lifting is to use two pins, one in load position and the other as safety pin. But at the accident location only one pin was used.

• Had there been two pins the accident would not have occured.

• The single lock pin was also located at the bottom most slot of the upright which made the upright to fail.

• It appears that the jump form over turned and then fell down.

LOSSES:• Five workers lost their life.• One worker got injured with multiple 

fracture.• Staff members were attacked by mob, 

one of them severly injured.• Office and store were attacked by the 

mob and all office equipments destroyed and files torn out.

• Mob damaged equipments standing outside. like hydra, passenger lift etc.

• Mob looted many materials including construction materials worth more than 30 lacs.

• Compensation amount : 12.50 lacs each.• Time loss, reputation loss.

FUTURE COARSE OF ACTIONPRECAUTIONS:• Check list should be followed religiously and signed off before allowing 

lifting of jump form.• Damages if any found in the jump form parts need to be replaced 

immediately.• Wearing full body harnesses even with in the area of jump form and 

platform.• Training and retraining of workers involved in Jump form erection and 

lifting.SYSTEM IMPROVEMENTS:• Jump form must be certified by Third party agency.• Design check for uprights and other jump form components.• Constant checking while fixing of JF at different levels by experienced JF 

supervisor/engineer and safety personnel to be ensured.• Life line arrangement for anchoring full body harness.• periodic safety audits exclusively for jump form erection, opearation and 

maintenance.• Flare up of cooling tower above neck to be reviewed and reduced.
