What to Bring?1.

Your child, who MUST be 5 years old on or before September 1, 2016.

Childs birth certificate with official seal.

3. Childs social security card.

4.

Certificate of immunization Form 3231.

5.

Hearing/Vision/Dental Screening Form 3300.

6.

Proof of residency (power bill, utility bill, lease agreement, etc.)

7.

Photo ID of parent/guardian who will be registering the child
