Close “the Gap”; Reach “Capacity,” Achieve 35,000.

• Capacity is reached when a DSA produces 75% conversion rates; 3.75 organs transplanted per donor and 10% DCD (with 2.75 OTPD) and generates sufficient resources to transplant the resulting organ supply.

Results of Michigan Transplant Centers’ PDSAs

• A LEAN “no show” process was created that decreased the patient “no show” rate from 30% to 19% in 2 months.

• A children’s hospital has implemented an approach team.• A kidney program has added a transplant surgeon and a list

management coordinator.• A thoracic program has added 2 transplant surgeons.• A liver program has added a transplant surgeon.

Results of Michigan Transplant Centers’ PDSAs

• Daily e-reports reflect every vented patient for virtual rounding.

• Length of time required to prepare a patient for placement on the transplant waiting list has been shortened.

• Two transplant centers have dedicated a hospital FTE to support and facilitate organ donation activities.

• Quicken spread of best practices to 950 hospitals that represent 90% of U.S. donor potential.

• Close the Organ Transplantation Growth & Management Collaborative.

• Sustain national accountability of all three estates for achieving/exceeding national goals (75%, 3.75) with the intent to increase the number of patients benefiting from a transplant.

• Increase the momentum, recommit to an ongoing collaborative process, and provide insights into specific, experience based practices.
