Prevention and Control of Infection: A Program by all for all

KAUH

Prevention and Control of Infections (PCI) program in King

Abdullah University Hospital is one of the unique programs at

the country level. It is an evidence-based program with

multifaceted framework and multidimensional objectives. It

provides all the unique following characteristics:

• This program is fully supported by the general manager with

the appropriate governance guidance

• The infection control committee is formulated according to

international guidelines and is composed of all stakeholders

including two American Board Certified infectious diseases

specialists. This committee has very clear set of points of

reference

• The infection control unit is composed of 4 well-trained

infection control officers who are highly qualified and very

familiar with international infection control practices

• Policies and procedures develop and implemented are

consistent with state of the art international guidelines but, at

the same time, sensitive to local communities needs. They are

updated at least once per two years

• Surveillance systems are according to USA NHSN guidelines

which makes it possible for us to benchmark our infection rates

with the American rates

• Transparency is maintained along all levels. We share or

numbers and results with all concerned in a blame-free

environment with emphasis made on opportunities for

improvement

• We believe in team work where every employee of KAUH

knows exactly his/her role to decreases infections in our

hospital

• Continuous training is adopted by infection control unit that

provides yearly and as needed customized special training for

hospital employees

• Social responsibility is well practice by our infection control

unit. We participate in many community activities and we

participate in training officers in other governmental and

private healthcare facilities

Our PCI program at KAUH is an ambitious program, supported

by a well-informed administration, based on evidence-based

approach, and aims to make its’ share with other hospital

programs toward safe and quality healthcare services

KAUH Departments, Divisions and Subdivisions

participated actively and effectively in the 3rd change

day which was organized by the HCAC.

Hundreds of pledges were carried out on the change

day, all of these pledges focused on one thing

"Improving Quality and Patient Safety“.

Facilitating processes, Children activities, Patients

engagements, hand hygiene promotions were examples

of many activities that occurred in the hospital

KAUH participates on the 3rd change day

The thorough investigations that included brain MRI

and brain Angiogram revealed a large complex AVM

occupying the left hemisphere and classified as

spetzler –Martin Grade 3. He was offered surgical

resection as a treatment of choice and this can be done

at KAUH as one of the specialized centers in the

region.

Prof. Barbarawi added that after discussing the case

with neuro-interventional radiologist at the hospital,

Dr. Khalid Alawneh, the decision was taken to address

the malformation mentioned surgically, being the best

way to get rid of this malformation once and for ever.

He said that the department of neurosurgery KAUH is

characterized by conducting this type of operation in

Jordan with excellent results as it is comparable to the

best specialized global centers, pointing out that such

operations are performed for patients who suffer from

abnormalities in the arteries of the brain, which may

lead to seizures or brain hemorrhage or sudden death.

The operation went successfully and the vascular

malformation was taken out completely, the patient

needed 4 united of blood transfusion and nine

aneurysm clips to control bleeding and to achieve a

complete resection of the lesion guided by neuro-

navigation and microscope.

A unique micro surgical resection of grade 3 arterio-venous malformation

(AVM) at KAUH

The neurosurgical team at the King Abdullah University

Hospital, led by prof. Mohammed Barbarawi, A consultant

of neurosurgery and cerebrovascular surgery; vice Dean of

the Faculty of Medicine at the Jordan university of

Science and Technology University, has successfully

resected a large AVM for a patient of 28 years in an

operation that lasted 8 hours and using a 3D-

neuronavgation guided microsurgery, in a circumstance

that considered one of the highly advanced and complex

brain surgery that requires high skills and integrated

specialized neurosurgery center.

Prof. Barbarawi added that the young patient who

underwent the surgery had presented to the outpatient

clinic, suffering from recurrent bouts of grand mal epilepsy

and chronic headaches.

glaucoma, pediatric ophthalmology and strabismus surgery,

amblyopia management, screening for and treating

retinopathy of prematurity, oculoplastics and esthetics of the

eyelids and the lacrimal drainage system.

Dr. Ababneh also stated that the faculty and the personnel

uphold the utmost level of professionalism and excellence in

dealing with the patients on the diagnostic and therapeutic

levels all that made easy and accessible by having at hand

the best instruments and equipment available for example:

the FFA (fluorescein fundus angiography) helps diagnose

retinal and choroidal pathologies, OCT (optical coherence

tomography) gives a cross sectional image of the retina,

Pentacam used in corneal disease diagnosis and the Visual

Field.

Dr. Laila Abanah also added that the ophthalmology

department at KAUH has gained a renowned reputation on a

national and a regional scale that was reached by the medical

and technical advancements in the field, made possible by

the equipment provided, and maintain a professional level by

constructing lectures, seminars and workshops: all led to the

progress that was achieved today.

The Ophthalmological Unit at KAUH provides exquisite and thorough services

to the patients

Dr. Laila Ababnah, head of ophthalmology division

part of the special surgery department at KAUH and a

faculty member at JUST, stated that the different

subspecialties available at KAUH equipped with the

newest technologies and state of the art instruments

which considered one of the best in the Kingdom and

the region and [provide quality healthcare services to

diagnose and treat different eye diseases under the

supervision of an exquisite team of specialists that

were graduated from the best universities of USA and

Germany in different subspecialties collectively

providing: cataract extraction surgery by

phacoemicification, vitreoretinal surgery, different

retinopathies requiring LASER and intraocular

injection treatments, cross linking in keratoconus,

where the blood bank separates the platelets from the other

blood components through the centrifuge. The amount of

platelets separated in this method has little significance, so

the doctors tend to give the patient six to eight units of

platelets in order to preserve the life of the patient.

Accordingly this process is not that efficient, especially in

cancer patients after taking the chemical doses. Due to these

reasons the blood bank is under constant pressure in order to

provide for and satisfy other patient's needs of platelets, in

which it needs to withdraw large amounts of blood to

separate enough platelets for clinical use.

Dr. Mohammad pointed out that studies have proven one unit

of platelets donated through Plateletphersis is equal to eight

units of platelets extracted through random centrifuge and it

can increase the platelet numbers 15000 to 30000. Hence the

importance of donating blood platelets directly.

Blood Bank in King Abdullah University Hospital is

considered a pioneer center for platelet donation, as it is the

only hospital in the north region that can host this type of

procedure. Due to the growing number of patients, and

because platelets half-lives are just five days, the

administration of the hospital provided the blood bank with

three devices to facilitate platelet donation. The staff of the

blood bank has committed itself to be a part in community

awareness about the importance of donating platelets, and

participate in voluntary blood donation campaigns, and

currently working on a permanent list of platelets donors as

is currently the case in blood donation.

(Plateletphersis) donations at KAUH to save patients' lives

The platelets are one of the main components of blood,

which relies its significance in stopping bleeding in

emergency cases, as supervisor of the blood bank Dr.

Mohammed Al-qudah mentioned. He also added that,

patients may suffer from a severe shortage of these

platelets, especially in patients who are undergoing

chemotherapy, such as leukemia patients, patients with

bone marrow transplant, transplant patients of various

organs such as the liver, heart surgery, patients with

multiple myeloma, patients with bone marrow failure

(Aplastic Anemia), patients with acquired

immunodeficiency (AIDS), and patients with decreased

platelet's numbers of unknown reason (Idiopathic

thrombocytopenic purpura), or decreased platelet's

function, pointing out that in both cases doctors are keen

to give the patient certain amount of platelets to stop

bleeding and to preserve the life of the patient. Dr.

Mohammad also mentioned that doctors extract these

platelets through normal blood donation,

Prof. Alzoubi pointed out that in such cases we can resort to

a new therapeutic way to treat epilepsy by electrically

stimulating of the brain through the vagal nerve using the

VNS device.

For his part, Dr. Alomari said that this device is one of the

most important alternative and safe methods to treat

epilepsy. VNS device sends electrical impulses from an

implanted battery to the vagal nerve. The surgical procedure

usually does not exceed two hours in terms of duration.

The New generation VNS device (AspireSR) has an implicit

mechanism of monitoring heartbeat changes due to epilepsy

and self-activates when the epileptic seizures occur, leading

to reduction of duration, frequency and severity of the

attacks in 50-90% of the cases, and also reduce the number

of times the attacks occur at night during sleep .

Dr. Alomari added that vagal nerve stimulation can also be

used in selected cases for the treatment of depression.

Advanced technology at KAUH for vagal nerve stimulation.

An ENT team, supervised by Prof. Firas Alzoubi, led

by Dr. Ahmad Alomari and assisted by Dr.

Mohammad Alsalem and Dr. Mahmoud Tanash, was

able to implant the newest generation vagal nerve

stimulator (VNS) device (AspireSR) for a patient who

suffered from epilepsy for 11 years and was refractory

to maximum medical treatment, the surgical

procedure was successful.

Prof. Alzoubi mentioned that about 30% of epilepsy

patients do not benefit from drug therapy alone, and

the presence of multiple foci in the brain that induces

epilepsy makes brain neurosurgical procedures very

difficult to perform as it may damage other sensitive

functions in the brain.

The procedures took a long time and were tricky and

complex; however the piece was finally retrieved from the

heart of the patient which spared her from an open heart

surgery.

Dr. Al Omari said that the procedure was performed in the

new interventional radiology room which recently equipped

with a state of the art biplane Angio suite unique in our

region.

New achievement at interventional radiology department

Extraction of venous catheter from the heart of young lady

The interventional radiology team at the department

of radiology succeeded in retrieving a 25 cm detached

central line piece from the right side of heart.

The patient is a 42 year old lady with breast cancer,

she is on chemotherapy giving through a Porta Cath .

She suddenly started complaining of chest tightness

and tachycardia .

CT scan was done and showed that the internal piece

of the catheter was detected from its place and slipped

into the right atrium and right ventricle of the heart.

Dr. Mamoon Al Omari and Dr. Mwafaq El heis, both

consultant radiologist who performed the procedures

said, the operation was done under local anesthesia

Matalka said that the importance of the new electronic

system is to facilitate pharmacists' task in dealing with

the large number of patients.

He pointed out that this system, developed by the

engineers of the department, will save time and effort

while processing patients' prescriptions, and thus should

enhance the accuracy of dispensing, which will reflect

positively on patient satisfaction.

It is worth mentioning that KAUH pharmacy dispenses

nearly 1,200 prescription daily from all available

specialty clinics.

Patients who reported having e-prescriptions were more

likely to feel favorably toward the electronic method.

Patients reporting use of the new system were somewhat

more likely to have adherence discussions often or most

of the time than patients who had not used it. Many

patients reported that they were thankful for the new

method of electronic tracking their prescriptions as it

saves time and effort while picking up their medications

from the pharmacy.

The electronic prescription tracking system at KAUH gains patients' satisfaction

Patient satisfaction is a key outcome measure by which

one can assess the quality and efficacy of KAUH care

provision. To improve the quality of services provided by

KAUH Pharmacy department and meet patients' needs and

satisfaction. KAUH Pharmacy Department in cooperation

with the Information Systems Department introduced The

Electronic prescription Tracking System.

Director of Pharmacy Department at KAUH, Dr. Taiseer

Malkawi said that this new system would enable patients

to trace their prescriptions on electronic displays placed in

the waiting area showing the progress of their prescription

processing which reduce inquiries and improve delivery

efficiency.

He added that the KAUH will be on standby position to

receive the pathological cases where it was to strengthen the

various sections of the hospital needs of the medical staff,

medicine and ambulances.

In the same context, deputy to the CEO and head of the

Emergency Committee, Prof. Basil Obeidat said during the

committee meeting attended by committee members, that

topics were discussed the all details of the necessary

preparations and plans for dealing with cases, to provide the

best medical and services to inpatients and outpatients from

all parts of the Kingdom.

He added that the action plan included measures and

arrangements for technical and management to

comprehensive health care provided through the hospital

level, and focused mainly on sections of ambulance and

emergency and raise the level of readiness and willingness to

accept the outpatients, especially as they are working 24

hours a day.

Prof. Obeidat pointed out the emergency teams to be in a

state of complete readiness and around the clock, to deal

with forced weather conditions that may occur during the

current winter season, where they will be provided with the

Civil Defense Department of the names and addresses of

dialysis kidney patients to enable them to conduct the

dialyzing process in a timely manner for each patient.

KAUH administration activates the emergency plan

KAUH activates its Emergency Plan for difficult

weather conditions in order to provide the best

medical care for patients and outpatients, by raising

the preparedness of all departments and increase the

number of medical, nursing and administrative staff in

all hospital facilities.

KAUH CEO, Prof. Ismail Matalka said that the

preparations and arrangements are necessary to cope

with emergency situations that may arise from

expected snow during the winter. Added that it has

been completed and is in full readiness, stressing the

importance of cooperation and coordination among

the hospital and various medical sectors, particularly

civil defense department and high readiness for the

transfer home patients to the hospital, especially

patients who need dialysis.
