(1) How do published papers reflect current practice as mobilization has been reported in a small percentage of ICUs? (2) What is the appropriate level of clinical expertise or experience required to safely work in a critical care environment? (3) What intensity, frequency, and dose of physical activity will lead to optimal patient outcomes? (4) What generalization to other patient populations can be made since the majority of patients studied are found in medical ICUs? (5) Should all patients who require mechanical ventilation or ICU admission be referred to physical therapy? And (6) Are there optimal patient populations who would benefit most from early mobilization, as well as populations for whom physical therapy is clearly contraindicated?

Type 3 :Pleural effusion requiring thoracentesis, suspected pneumonia (radiological evidence without confirmation of bacterial meningitis), pneumonia with radiological evidence and confirmation in the bacterioscopy, pneumothorax,reintubation with mechanical ventilation period not exceeding 48h
Type 4 :Ventilatory failure: ventilator dependency in the postoperative period exceeding 48h, intubation with subsequent dependence on mechanical ventilation for more than 48h

1 Active-assistive exercises of the lower/upper extremities
– ankles and wrists, five sets of 10 repetitions; bed inclined at 45 degree.

– flex.-ext. of the bilateral shoulder, elbow, wrist, knee and ankle;
- adduction /abd of the hips (two sets of 15 for each) and
- orthostatic position/walking on spot (5 min) as tolerated.
3 Active exer., as in step 2, but in three sets of 15 in the sitting position and ambulation within the inpatient ward (5 min)

By hospital discharge, EG presented significantly higher parasympathetic HRV values, global power, non-linear HRV indexes and mean RR compared to UCG (p<0.05).
Conversely, higher values of mean HR, low frequency and the LF/HF (global sympatho-vagal balance) were found in the UCG.

The cardiac autonomous neural system is related to the functional properties of the heart and the HRV reflects the sympathetic and parasympathetic influences on the heart and intrinsic rhythm. As patients after CABG are prone to autonomic impairment in the early PO period, implementation of strategies shown to be effective in ameliorating this negative adaptation during the inpatient phase, such as exercise-based CR, is certainly warranted.
