At Lowe’s, we’re committed to supporting you and your family, and we demonstrate that commitment through our comprehensive and competitive benefit offerings.

Annual Enrollment for 2022 benefits is Oct. 4–22. This is the time to select the offerings that will work best for you in the coming year. Please review the information in this guide and keep it for reference throughout the year. The more you know, the better prepared you’ll be when you or a covered member of your family needs care.

Full-Time Medical Plan ChangesFor 2022 you’ll have the choice of three medical plans: Option 1, Option 2, and a new high deductible health plan, the Choice Account with Optional HSA. This new plan is being introduced to help simplify our medical plan offerings.

The former Choice Account and Choice Account Plus plans are merging into this new plan. If you are currently enrolled in either the Choice Account or Choice Account Plus plan you will be automatically enrolled in the Choice Account with Optional HSA for 2022 if you do not make a change during the three-week Annual Enrollment period.

All three plans will remain with the Blue Cross Blue Shield (BCBS) network. Even though our BCBS plan is located in Alabama, associates across the country have access to a national network of quality providers and facilities (refer to pages 17-18). Kaiser Permanente will continue to be offered in states where available.

• Option 1 and Option 2 medical plan designs are not changing. Enrollees in these plans will need to continue to complete the Activation Reward to receive the biweekly $50 savings to offset your medical premiums. All BCBS members will receive new ID cards for 2022 even if you did not change medical plans.

• In 2022, the Activation Reward will apply to Option 1 and Option 2 plans only. Associates enrolled in the Choice Account with Optional HSA may still view the Benefits video and complete the survey, but the Activation Reward will not apply.

• The HSA Dollar for Dollar match up to $500/$1,000 will only continue in 2022 for associates whose base salary is $50,000 a year or less. This salary cap is based on annual salary.

Enhanced Fertility Support Benefits For 2022 your fertility benefits through Progyny will include fertility preservation and donor tissue coverage. More information is located within this guide.

Specialty Drug Copay AssistanceYour pharmacy benefits with CVS will now include access to the PrudentRx program for members using certain specialty medications. This program brings considerable savings to you when purchasing your prescriptions. If you take qualifying specialty medication, PrudentRx will contact you directly.

Vision Plan Enhancements P

For both the High and Low Vision plans, we are adding the following enhancements (refer to page 22):

• $20 copayment for retinal screening for all members.

• Retinal screening for members with diabetes at no cost. $20 per exam for additional exams and services for members with diabetes, glaucoma or age-related macular degeneration.

• Treatment and diagnoses of eye conditions, including pink eye, vision loss and cataracts available for all members.

What’s New? New Year, New Benefits for 2022We have several benefit enhancements that will be offered to full-time and part-time associates in 2022.

Dental Plans In 2022 the out-of-network Dental High and Low Plan benefits will be reduced. Visit an in-network dental provider to maximize your plan savings when receiving dental care (refer to page 21).

Accidental Death and Dismemberment (AD&D) PlanMetLife Accidental Death and Dismemberment Plan will now be offered through The Hartford. The Hartford will continue to provide the same AD&D Insurance to pay a lump sum in the event of a covered accident. Specifically, these covered accidents are ones that could not be foreseen and are fatal or dismembering.

Life InsuranceLife insurance coverages will also move to The Hartford from MetLife. The Hartford is honoring all pre-approved coverage amounts. Associates will not need to go through evidence of insurability (EOI) or re-approval for any coverages they already are enrolled in.

Lowe’s will continue to invest in our associates‘ financial well-being by providing Employer Paid Life Insurance benefits for all who are full-time benefits eligible.

Long-Term Disability (LTD)We all learned the importance of health and disability coverage this past year. New York Life has agreed to waive evidence of insurability (EOI) for our hourly associates who enroll in the voluntary LTD plan. This is a one-time offer during 2022 annual enrollment. More information can be found on the enrollment site when you make your LTD elections.

Virtual BetterHelp Therapy P Lowe’s calm program continues to offer virtual therapy through BetterHelp. Connect with a licensed therapist via text, live chat, phone or video.

Substance Use Center of ExcellenceIf you or someone in your family is struggling with substance use disorder, we now have a Centers of Excellence program to help. The program provides a higher level of coverage for certain substance use disorder treatments at several national treatment facilities, to assist full-time associates and their dependents enrolled in a Lowe’s BCBS medical benefit option.

SmartChoice for saving and spending accountsInstead of Your Savings Account™ (YSA) administering the Healthcare FSA, Health Savings Account and Dependent Care FSA, SmartChoice will be your one-stop shop for your tax-favored savings and spending account needs.

Reminders

• Annual Enrollment will open Monday, Oct. 4, and close at 11:59 p.m. CT on Friday, Oct. 22. Log into MyLowesBenefits.com and click the Annual Enrollment tile to get started.

• Enrollment for most benefits is passive, so insurance coverage in place on Dec. 31, 2021, will roll over to the most similar coverage available on Jan. 1, 2022, unless you make changes during Annual Enrollment. Associates currently enrolled in the Choice Account or Choice Account Plus plan will automatically be enrolled in the Choice Account with Optional HSA plan for 2022 unless a change is made during the three-week Annual Enrollment period.

• Flexible Spending Account contributions (health or dependent care), Health Savings Account contributions and Activation Reward are active enrollments. That means you must take action to re-enroll each year or these benefits revert to $0.

• For hourly associates, Lowe’s will match 50% of your contributions to the Dependent Care Flexible Spending Account (DCFSA) up to a maximum match of $1,000 per year.

▶ The DCFSA can be used to pay for daycare, camps and other eligible expenses with tax-free money, and Lowe’s will make a contribution to help.

▶ You must pledge an annual contribution during Annual Enrollment to receive the Lowe’s match.

Virtual visits with providers allow you to get the support you need from the comfort of your home. It’s an affordable option for quality medical care. For first-time users in BCBS Option 1 or Option 2, your first Teladoc visit is at no cost to you.

General Medical: Talk to a doctor anytime, anywhere. You have access to a network of doctors that can treat every member of the family. Prescriptions can be sent to your pharmacy of choice if medically necessary. Complete your registration ahead of time so you’re ready to use Teladoc when needed.

Lowe’s offers several financial programs to support you during all stages of your life.

Traditional and Roth 401(k) now managed by PrincipalWe are excited to offer you another way to work towards your savings and retirement goals. Lowe’s associates are eligible to contribute to a Roth savings account within the Lowe’s 401(k) Plan through Principal (formerly Wells Fargo). This is another way associates can contribute to their 401(k) accounts, but with different rules on when contributions are taxed.

Roth contributions, like the pre-tax salary deferral contributions you can already make under the Lowe’s 401(k) Plan, are amounts you contribute from your paycheck toward your retirement savings. Unlike pre-tax contributions to your 401(k) account, which are excluded from your taxable income, Roth contributions are not excluded from your taxable income but are generally not taxed when a qualifying distribution occurs at retirement.

You can access your account by logging into myHR > My Health & Well-Being > View and Update my 401(k). Please contact Principal at 800-547-7754 or principal.com if you have questions about the Roth 401(k) salary deferral option.

P Lowe’s offers three Allstate Benefits plans to help cover additional medical costs. Two of these plans include a Wellness Benefit. If any covered person has preventive care performed during the year, they can provide proof in a claim to Allstate and get a $75 check for both the Accident and Critical Illness plans (if enrolled in both).

TIP: If you’re enrolling for the first time, wait 30+ days to have the preventive visit or the wellness claim won’t count.

• Critical Illness: Receive a cash payout if a covered person has a serious disease or health condition like a heart attack, stroke or cancer.

• Off-the-Job Accident: Receive a cash payout if a covered person is hurt in an accident and incurs a health expense.

• Fixed Indemnity: Receive a cash payout if a covered person has a hospital stay.

▶ Note: If you are expecting a baby in 2022 and delivering in a hospital, this benefit can help offset the cost of the hospital stay for you and your baby.

Need a leave of absence? For full-time associates: The Lowe’s Disability, Leaves and Accommodations Service Center, managed by Sedgwick, is available to help you with your workplace accommodation and leave of absence needs. They can assist with at-work medical accommodations, religious accommodations, medical and personal leaves of absence, military leaves and short-term disability. You can reach them via MyLowesBenefits.com by clicking Leaves, Accommodations or Worker’s Compensation on the Sedgwick tile to begin a claim. You can also call 844-HR LOWES (844-475-6937) Monday through Friday 7 a.m. to 7 p.m. CT.

For part-time associates: Call Sedgwick to initiate a leave of absence. If enrolled in the part-time Short-Term Disability plan, you will be advised to contact Allstate to start a disability claim.

The statement is a personalized view into your “total rewards” package, which includes not only your compensation but also the value of your retirement and health and welfare benefits, and more, as listed on your statement. The statement provides you visibility to a rolling, 12-month view of the monetary amounts of your benefits and compensation. Your statement is accessible in Workday under the Benefits and Compensation sections (myHR > My HR Tools > Workday > Profile > Compensation or Benefits.)

Family MattersBecause what happens at home affects your whole life, including your work, Lowe’s offers associates a variety of benefits to help ease your mind when it comes to family concerns.

Lowe’s calm program provides access to care in a variety of ways with enhanced benefit solutions like BetterHelp. You can receive counseling in person, online or through web-based care programs.

To make it easier to access licensed counselors, we offer confidential, online services through BetterHelp. You can send a counselor a secure, confidential message (text, video or voice message) whenever you need, and the counselor will typically engage at least two times a day, five days a week. You still have access to in-person counseling and will continue to receive five sessions at no cost to you or your household members. For access to in-person counseling sessions, please call 877-543-3875.

• For virtual therapy, one audio or real-time video or live chat session equals one pre-paid counseling session. One week of text messaging is equivalent to one pre-paid counseling session.

In addition to BetterHelp, calm also provides web-based, confidential care programs for support and information about day-to-day concerns. With calm, receive the support you need to continually improve yourself at work and at home. Health Savings Account (HSA)FOR FULL-TIME ASSOCIATES

Your Health Savings Account (HSA), which uses before-tax savings to pay for eligible health-care expenses, can grow faster for associates enrolled in Choice Account with Optional HSA because Lowe’s will match your Health Savings Account (HSA) contributions dollar for dollar up to a maximum of $500 for associate-only coverage, or $1,000 if you’re covering dependents. Remember, you must contribute funds in order to receive the Lowe’s match. The HSA Dollar for Dollar match up to $500/$1,000 will only continue in 2022 for associates whose base salary is $50,000/year or less. This salary cap is based on annual salary.

Did You Know? Your HSA includes a feature called HSA Advance. If you need more money to cover your eligible expenses, HSA Advance will accelerate future contributions to your HSA deposit account to automatically cover HSA card charges. With HSA Advance, future payroll contributions are available early to cover those expenses. You don’t need to do anything extra. Your future payroll deductions will then be applied to any accelerated amount you’ve used.
