If you have already shared your NPI with BCBSIL,
please be aware that we have extended our dual-
identifier acceptance phase beyond the May 23, 2007,
compliance date to allow you to further test and prepare
for an NPI-only environment. During this extended
dual-identifier acceptance phase, it is very important
to submit all paper and electronic claims with your
new NPI and your BCBSIL provider number. Do not
submit claims using only your NPI until you receive
notification from BCBSIL.
Electronic Submitters:
When transmitting your claims to BCBSIL
electronically, please continue to include your NPI
and your legacy or BCBSIL provider number. It is
imperative that you contact your software vendor
billing service, and/or clearinghouse to ensure that
they have your NPI and are equipped for dual-
identifier submission on claims.
Paper Submitters:
Providers must submit claims using only the revised
version of the CMS-1500 Health Insurance Claim
Form (version 08/05). Providers utilizing the
CMS-1500 claim form must bill with either:
• BCBSIL provider number and NPI; or
• BCBSIL provider number only.
Note: The old version of the CMS-1500 (version
12/90) is no longer being accepted.
Remember: Sharing NPIs with other providers and health
plans is required. As outlined in current regulation,
providers must share their NPI with any entity that
may need it for billing purposes—including those
who need it for designation of ordering or referring

How do I know BCBSIL has received my NPI?
BCBSIL sends confirmation postcards on a
bi-monthly basis to providers once we have validated
and loaded your NPI information into our system.
Please allow three to four weeks for receipt of your
confirmation postcard.

At BCBSIL, we strive to process your claimsquickly and accurately. In an effort to ensurethat your claims for BlueCard (out-of-area)members are processed in a timely manner,please follow these steps:
1. Submit all Blue claims to BCBSIL electronically, or via paper on the revisedCMS-1500 (08/05) claim form to:

2. Include the member’s complete identification number, including the three-character alpha prefix, when you submit the claim. Submit only claims with valid alpha prefixes. Claims withincorrect or missing alpha prefixes and/ormember identification numbers cannot befiled correctly.
3. In cases where there is more than onepayer and a Blue Cross and/or Blue ShieldPlan is a primary payer, submit Other PartyLiability (OPL) information with the BlueCross and/or Blue Shield claim. Uponreceipt, we will electronically route theclaim to the member’s Blue Plan. Themember’s Plan then processes the claimand approves payment. BCBSIL will reimburse you for services.

4. Do not send duplicate claims. Sendinganother claim, or having your billingagency resubmit claims automatically,actually slows down the claims payment process.

Health Care Debit Cards
Some Blue Cross and Blue Shield members have a health care debit card, with value-addedfeatures to assist you with collecting member cost sharing amounts. The card allows members to pay for out-of-pocket costs using funds from their spending accounts.
BCBSIL Members in BlueEdge PPO
Stand-alone health care debit cards are issued to membersin certain employer groups who have our BlueEdgeConsumer Driven Health Plan (CDHP) product.Copayments and other out-of-pocket expenses can bededucted from a member’s Health Savings Account(HSA), Health Care Account (HCA) or FlexibleSpending Account (FSA).
BCBS Out-of-Area Members (BlueCard)
Members with health care debit cards belonging toother Blue Cross and Blue Shield plans may have adebit card with the nationally recognized Blue Crossand/or Blue Shield logos, along with a major debit cardlogo such as MasterCard® or Visa®. Out-of-pocket costsmay be covered by funds from the member’s HRA, FSA or HSA.
It’s easy to use—simply swipe the card like an ordinary debit card
The cards include a magnetic strip so providers can swipe the card at the point of service to collect the copayment and other out-of-pocket expenses through any debit card swipeterminal. The funds will be deducted automatically from the member’s spending account.
Helpful tips to remember
1. Ask members for their current member ID card and regularly obtain new photocopies(front and back) of the member ID card. Having the current card will enable you to submit claims with the appropriate member information (including alpha prefix) andavoid unnecessary claims payment delays.
2. Check eligibility and benefits electronically by using NDAS Online for BCBSIL andBCBS out-of-area members. You can also call our Provider Telecommunications Center (PTC) at (800) 972-8088 for BCBSIL members, or (800) 676-BLUE (2583) forout-of-area members. Make sure to provide alpha prefixes.
3. If the member presents a debit card (stand-alone or combined), be sure to verify thecopayment amounts before processing payment. Select “debit” when running the cardthrough for payment. No PIN is required for use. You will not incur a fee when swiping ahealth care debit card.
4. Please do not use the card to process full payment up front. If you have any questionsabout claims payment, please contact the BCBSIL Provider Telecommunications Center(PTC) at (800) 972-8088. For questions about debit card processing instructions or payment issues, please contact the debit card administrator’s toll-free number on the backof the card.

Medical PolicyDisclosure Statement
In the April Blue Review under the article entitled Proper Speech Therapy Billing – New,we printed an incorrect CPT code for the "treatment of speech, language, voice, communication, and/or auditory processing disorder; group 2 or more individuals.” Code92508 was incorrectly published as 925078. We regret this error.
Correction - Speech Therapy Billing
Your NPI is currently being sent in the 835 transaction when submitted to us in the837P/837I, via DDE for Professional claims or received on the paper claim forms.
Note: For each NPI, there will be one payment/remit/check created. There will also be onepayment/remit/check created when the claim was submitted using the legacy number. As aresult, there can be multiple payments in the 835 and PCS/EPS.
NPI via Institutional DDE went into production effective May 8, 2007.
Institutional/Professional Segment Identifiers:
GS02 usually identifies if the data contained in the 835 transaction is for an Institutional or Professional Provider. If the file contains both professional and institutional remitsbetween one ISA and IEA, there will only be one GS segment, based on the first provider number identified in the file, and it will be as follows:

Prescription Drug Formulary Update
As a result of recent decisions made by ourPharmacy and Therapeutics committee, somechanges will occur to the Blue Cross and BlueShield of Illinois (BCBSIL) Formulary effectiveJuly 1, 2007.
Proventil HFA will be removed from the formulary effective July 1, 2007. Proventil HFAis an inhaler used in the treatment of asthma.The following chart lists formulary alternativesfor the medication.
New Generics Available
for Norvasc and Ambien
Two highly utilized medications have recentlyhad generic alternatives approved. Norvasc, amedication used to treat high blood pressure,and Ambien, a medication used to treat insomnia, both have FDA approved genericalternatives that are now available.
Lipitor Scheduled
for Removal from Formulary
The Pharmacy and Therapeutics committee hasmade the decision to remove Lipitor from the formulary effective October 1, 2007. Lipitor is ahighly utilized medication for the treatment ofhigh cholesterol. There are now three genericalternatives in the cholesterol drug class: lovastatin, pravastatin, and simvastatin. In aneffort to help control medication costs in the cholesterol class, while promoting safe and effective therapy, the decision was made to removeLipitor from the formulary. More informationabout the management of the cholesterol class will be available in further editions.
