Job Safety Analysis

Department: MWD Operations Task: Laying down MWD Tools Date: Task: Hazards: Controls:

1. Pause. … Take a moment to identify any unsafe issues. Alert driller that operators will be going onto the drilling floor. Ask permission to enter the floor and ask for any hazards that the operators should be aware of before entering floor.

2. Breaking top monel from bottom monel and lifting monel above MWD spearpoint.

Working around suspended loads. Pinch points. Head injury.

Use two hands to stabilize monel as driller lifts the monel above the MWD spearpoint. Never stand between the driller and MWD personnel, good communication. Have driller bring monel joint high enough above the MWD tool so not to be hazard or get in operators way.

3. Lifting the MWD tool out of the monel and lowering the MWD tool down to the substructure area.

Working around suspended loads. Pinch points. Snag areas. Working near winch lines. Improper securing of MWD tool. Slipping of winch line.

Talk to winch line operator and give detailed instructions. MWD tool should be secured to winch line using Shell / Safety approved MWD harness. Never should winch line be directly attached to the MWD tool. Special care should be taken to never be in a position directly underneath the MWD tool.

4. Lowering the MWD tool to the substructure area.

Working around suspended loads. Pinch points. Snag areas. Working near winch lines. Improper securing of MWD tool. Slipping of winch line.

Talk to winch line operator and give detailed instructions. MWD tool should be secured to winch line using Shell / Safety approved MWD harness. Never should winch line be directly attached to the MWD tool. Special care should be taken to never be in a position directly underneath the MWD tool. Have a number of hands ready to receive the tool in the substructure area.

5. Carrying the MWD tool from the substructure area to the MWD workplace.

Improper lifting methods. Injury by tripping, uneven surfaces. Not knowing your surroundings. Injury by falling objects in the drop zone below the substructure area.

Ask a number of hands to help lift and carry the MWD tool using their legs and not their back. Make sure path if at night is well lit. Have a spotter not carrying the MWD tool make others around him aware of the tool being moved and have this

person help guide around any obstacles. Let the driller know the tool will be laying on the ground near the substructure area. Clear any objects on the rig floor that could fall off the rig floor into the substructure area.

6. Removing the Muleshoe sleeve from the UBHO after laying down UBHO sub from BHA. Breaking set screws from UBHO.

Pinch points. Back injury. Using the help of a rig hand to stabilize the UBHO on the rig floor break set screws loose. Knock out MWD sleeve using two hands on the knock out bar. Be careful moving rod back and forth as to not injure any personnel near you.

7. Carrying down MWD rig floor tools from rig floor to MWD workstation. Clearing all MWD equipment off of rig floor.

Moving equipment on rig floor from drilling operations. Gas from well causing rig floor to be unsafe. Drilling operator being unaware of MWD operator’s position. Injury descending stairs, tripping.

Alert driller that operators will be going onto the drilling floor. Ask permission to enter the floor and ask for any hazards that the operators should be aware of before entering floor. Carry only one article of equipment down the floor at any one time. Always use one hand to stabilize yourself going up the stairs.

PPE, special tools and other safety equipment required to do this job:

Hard hat, safety glasses, steel toe boots, gloves, and flame retardant clothing
