Introduction

1. The proposals are invited for establishment, modulation & up-gradation

of various units in District Hospital Balodabazar.

2. Interested reputed registered prime manufacturers/contract manufacture /

authorized dealers /agencies/ firms having experience of at least 3 years

of similar nature of works ( in Hospitals Only ) &prime manufacturers/

contract manufacture authorized dealers /agencies/ firms should have

average turnover of at least Rs. 5.00 Crore for last 3 years ( For

establishment, modulation &up-gradation of various units in Hospita

such as SNCU, OT(Major &Minor),ICU, ICCU, PUincluding

PICU,MCH Wing & Diagnostic Units (Pathology, Blood Bank &

Radiology Department) etc.

3. The forms could be obtained on any working day from the Office of Civil

Surgeon cum Chief Hospital Superintendent District Hospital

Balodabazar (C.G.) by depositing of Non Refundable Demand Draft of

Rs. 10000/- Ten Thousand Only) in favor of Jeevan Deep Samiti, District

Hospital Balodabazar (Payable at Balodabazar ).

4. The bid shall be submitted in a sealed envelope containing three separate

sealed envelopes marked as “A” “B” & “C” as detailed below :-

(i) Envelope “A” Contains: - EOI document (form) along with

required qualifications, experiences& EMD.

(ii) Envelope “B” Contains:- Copy of presentation along with

concept plan, drawings, details, any other technical information&

specifications if necessary.

(iii) Envelope “C” Contains:- Price bid&BOQ along with time

schedule in the form of bar chart for various works.

5. Envelope “A” shall be opened first and examined. Envelope “B” of only those participants shall be opened who have submitted all details required

in it. Envelope “C” shall be opened in working hours only after

presentation will be held.

6. The sealed proposals should be submitted so as to reach the Office of the

Civil Surgeon cum Chief Hospital Superintendent, District Hospital

Balodabazar , (C.G.) on or before 12.30 PM. of 30th June 2017 through

Speed-Post/Registered post only.

7. The bid shall be signed by the owner or persons on behalf of the

organization having necessary authorization/ power of attorney to do so.

Each page of application shall be signed (copy of power of Attorney/

Memorandum of association shall be furnished along with the application

and Original should be produced subsequently for verification and return.

8. The bidders are expected to examine all instructions, forms, terms,

project requirements and other details in the EoI documents. Failure to

furnish complete information as mentioned in the EoI documents or

submission of a proposal not substantially responsive to the EoI

documents in every respect will be at the bidder's risk and may result

in rejection of the proposal.

9. EMD of Rs. 10,00000.00 (Ten Lakh’s only) should be submitted with

Part I: General Terms

1.Goals of the expression of interest(EOI) :-

The objective of this EOI is to solicit proposals from the interested bidders for

participation in a bid process for selection of reputed agency for establishment,

modulation & up gradation of various units in District Hospital Balodabazar ,

such as , OT (Major & Minor), ICU, NICCU,Gaynee Wards, delivery room ,

ENT Setup , and many more . In addition to above works,the agency will be

responsible for CMC & AMC of the components of works including all type of

equipments, machineries, tools& its parts for the extended period of 5 years.

The agency should have to submit a certificate mentioning that he will be

responsible for CMC/AMC for components of works. All items wiil be quote

including 3 year warranty .

3. BIDDERS’ CONFERENCE

JDS will host a bidder’s Conference in District Hospital Balodabazar for

details regarding bid & components of works, the owner of agencies or their

representatives can attend the conference.

4. EOI PROCESSING FEES

A refundable processing fee for Rs. 10,00,000 (Rupees Ten Lakh only) in

the form of aFDR/TDR drawn in favour of"Jeevan Deep Samiti, District

HospitalBalodabazar " has to be submitted along with the EoI Response.

Bids received without or with inadequate EoI Processing fees shall be liable

to get rejected.

5. VENUE & DEADLINE FOR SUBMISSION OF PROPOSALS

Proposals, in its complete form in all respects as specified in the EOI, must

be submitted to Jeevan Deep Samiti at the address specified, in exceptional

circumstances and at its discretion, extend the deadline for submission of

proposals by issuing an addendum to be made available on the District's

website, in which case all rights and obligations of JDS and the bidders

previously subject to the original deadline will thereafter be subject to the

deadline as extended.

6. Final Selection of Agency

The bids will be evaluated according to the marks obtained and the

selection may be on a70:30 Quality Cost Based System (QCBS).

6. Terms & Conditions

The JDS have the right to change, alter, postpone or not to execute of any

of the work & the agency shall not be entitled for any compensation for

non-execution of the work except the fees, payments which are payable to

the agency up to the stage of work already completed.

Any disputes or differences in connection with the agreements can be

settled by arbitration & or under jurisdiction of Balodabazar only.

Components of work can be changed in the future as per requirements.

1. BACKGROUND

About Jeevan Deep Samiti Balodabazar

Jeevan Deep Samiti (JDS) is concerned with administration & development

of district hospital Balodabazar JDS executive committee is chaired by

District Magistrate, any fund which is sanctioned for special initiative or

program by Govt.(Central/ State) or other partners like CSR from DMF are

regulated &function under the overall administrative control of Jeevan Deep

Samiti Balodabazar.

Details of Project is given below -

TheJDS has implemented establishment, modulation& up gradation of

various units in District Hospital Balodabazar such as OT (Major & Minor),

ICU, NICCU, Gaynee Wards, delivery room , ENT Setup , and many

more.to provide quality Health care services to Mother, Children& Local

peoples in this tribal district. Balodabazar where health services are very

poor nearest referral unit is MEKAHAR Raipur .which is 85 km away from

district hospital. So by establishing units/wings in District hospital

strengthen the health services in Balodabazar & improve the health

indicators.

All the components of work listed above are as per tentative assessment the

agency can increase, decrease or make changes in component of work if it

will became more suitable for our requirement.Agency may add their ideas

& inputs for betterment of this project.

Components of work should be user friendly as per need of patients & there

relatives, easy movement of staff design should be of space planning based.

The components & planning of work should be as per standards & codes

such as IPHS & NQAS etc.

To prepare detailed architectural drawings to suit the schematic plan to

satisfy the committee. To provide supervision during execution of the works

when ever needed.

The agency should give details of their employees, authorized

representatives, experts & engineers which he may appoint for the execution

of work.

Need for the Expression of Interest

Establishment, modulation& up gradation of various units in District

Hospital Balodabazar such as OT (Major & Minor), ICU, NICCU,Gaynee

Wards, delivery room , ENT Setup , and many moreetc.is very complex and

keen work. Specialization is very needful for the purpose of this work. There

for EOI is called for specialized consultancy Firms, time period is very

important in this work to improve health services in Balodabazar.

suggestions about how it will enhance quality health care facility & esthetics in the institution.

4. AVAILABILITY OF THE EOI DOCUMENTS

1. RIGHTS TO THE CONTENT OF THE PROPOSAL

For all the bids received before the last date and time of bid submission, the

proposals and accompanying documentation of the Pre-Qualification proposal

will become the property of JDS and will not be returned after opening of the

pre-qualification proposals. JDS is not restricted in its rights to use or disclose

any or all of the information contained in the proposal and can do so without

compensation to the bidders. JDS shall not be bound by any language in the

proposal indicating the confidentiality of the proposal or any other restriction on

its use or disclosure.

2. ACKNOWLEDGEMENT OF UNDERSTANDING OF TERMS

By submitting a proposal, each bidder shall be deemed to acknowledge that it

has carefully read all sections of this EOI, including all forms, schedules and

annexure here to and has fully informed itself as to all existing conditions and

limitations.

3. EVALUATION OF PRE QUALIFICATION PROPOSAL

The bidders' Pre-Qualification Proposal in the bid document will be evaluated

as per the requirements specified in the EoI and adopting the pre-qualification

criteria spelt out in this EoI. The Bidders are required to submit all required

documentation in support of the pre-qualification criteria specified (e.g. detailed

project citations and completion certificates, client contact information for

verification, profiles of project resources and all others) as required for

evaluation.

4. LANGUAGE OF PROPOSALS

The proposal and all correspondence and documents shall be written in English.

5. PRE-QUALIFICATION CRITERIA

The invitation for bids is open to all entities registered in India who fulfill

prequalification criteria as specified below -

i. The Bidder should be a company registered under Indian Companies

Act1956.

ii. The Bidder should be a profitable company for the last three years and

must have an annual turnover of not less than INR 5 Crores for average

of the last three financial years ending 31st March 2016.

iii. The Bidder must have experience to work in health sector a to executed

transitioning of ‘Turnkey Projects( Same As per The Tender ) ’ of at list

3 projects. Atleast one project in Chhattisgarh government.

iv. The bidder (Service Provider) must have been assessed and certified from

the all necessary documents will submitted.

v. The company should pay all tax- commercial tax/ income tax/sales tax etc

& having valid tenure.

vi. Service centre Should be localized in Chhattisgarh at least stablish past 2

years .

vii. BIDDER SHOULD NOT BLAACK LISTED OR NO ANY CASE

PENDING IN ANY COURT OF INDIA AS ON DATE . SHOULD
