Cremation is the process of reducing the human body to basic chemical compounds in the form of gases and bone fragments.

This is accomplished through burning—high temperatures, vaporization and oxidation. Choosing cremation does not

eliminate other decisions you and your family want to make regarding a funeral or memorial service. In fact, cremation is

simply the first of many decisions you will make as you strive to share your grief and honor the memory of your loved one.

Following the death and cremation of a loved one, there is the question of what to do with the cremated remains. You have

many choices regarding the final resting place. Interment can be in your family plot, church garden or other memorial site such as a columbarium (an arrangement of niches). You may also choose multiple urns, allowing the cremains to be interred in more than

one location or shared among family members.

Rather than burying, scattering the cremains or storing them in a single urn, cremation jewelry is ideal for sharing among family

members. You may keep a small portion of the cremains as keepsakes. Whatever you choose a traditional burial or cremation, the memorial you plan should be a tribute to your loved one and a

~General Information~

Clothing It is the responsibility of the family to have all the clothing to be worn by the deceased cleaned and delivered to the funeral home (3) days prior to the date of service. Any personal items (rings, chains, watches etc.) placed on the deceased are the responsibility of the next of kin or legal representative. J.E. Washington Funeral Services Inc. and staff will be held harmless from any liability due to loss or damage.

Processing the death certificate We will present the completed death certificate to the attending physician/coroner after the family has provided all necessary statistical information. The attending physician/coroner will certify the cause of death and notify the funeral home when the death certificate is completed. The completed certificate is then filed in with the Bureau of Vital Statistics in the county in which the death occurred. The entire process may take approximately 10 business days. Death certificates that are to be completed by a medical examiner or coroner may take up to several months to complete. Social Security Benefits The funeral home may file Form SSA-721 to notify the social security administration of the death. The social security requires the next of kin or legal representative to schedule an appointment or file online for benefits. Burial benefits are paid directly to the spouse or minor children upon eligibility. Veterans Administration The funeral home will assist the next of kin or legal representative in filing for V.A. burial benefits. All honorably discharged veterans are entitled to receive at no charge a flag, grave, standard grave liner, headstone, military funeral honors and burial in a local national cemetery. Proper documentation must be submitted.

What to Do After the Funeral 1. Prepare a list of persons to receive acknowledgments of flowers, memorial contributions, etc. Send appropriate acknowledgment cards. (You may want to send "thank you" notes to those who gave their time as well.) 2. Check promptly on all debts and installment payments, including credit cards. Some may carry insurance clauses that will cancel them. 3. If deceased was living alone, notify the landlord and utility companies. Notify the post office and complete a change of address form. 4. The funeral director will prepare Social Security Form SSA 721, however you may contact their office to apply for benefits. If Social Security Checks are automatic deposit, notify the bank of the death. 5. Obtain a copy of the deceased's will or trust. Select a lawyer if legal advice is needed. You will need to notify the executor of the will or administrator of the trust. 6. Notify all life, health and casualty insurance companies in order that you may: -Claim benefits due under policies on the deceased -Make any beneficiary changes on policies where the deceased was named the beneficiary,

Determine if the deceased was owner of any policies of dependents or business associates, as cash value of such policies must be included in the deceased's estate. NOTE: You should check all policies, even if you think they may have lapsed. If you do not have a copy of the policy, the insurance company will have complete records. Sometimes old insurance policies have merged with a different company. 7. Check with deceased's present and past employer(s) for possible insurance and other benefits. 8. Contact business or service organizations of which the deceased was a member for possible benefits. (Ex. Masonics, Elks, Eastern Star) 9. Contact the nearest Veteran's Administration office if the deceased was a veteran. He /She may be entitled to a free grave marker and burial allowance. They will require full name of the deceased, branch of service and service serial number. 10. Contact Civil Service Commission if the deceased was employed in the Civil Service. 11. Contact the Internal Revenue Service office to determine tax-filing requirements. Please feel free to contact our office if you need assistance or have any
