Dental handpieces are constructed with a number of internal channels and pathways. These features are often difficult to clean, although the use of a validated automated washer-disinfector may be successful provided that handpiece and washer disinfector are of known compatibility. Where this is established, sterilisation using a type B or type Ssteriliser is likely to be useful.

Expert advice - Only unwrapped solid instruments can be processed in N class autoclaves. To test daily use the AutomaticControl Test and Class 6 Indicator test Strips. Although not a requirement of these strips provide a good way of checkingthat the N type autoclave is working and they provide evidence of this for auditing (see pages 19-21).

The testing protocols for washerdisinfectors are very similar to thoserequired for ultrasonic baths. They fallinto three main categories; weekly,quarterly and annual testing.

A weekly protein residue test should be carriedout on one of the instruments that has beenthrough the washer disinfector cycle. However,unlike with ultrasonic baths, instruments don’tnormally need to be rinsed with RO (reverseosmosis) or distilled water, because the washerdisinfector will incorporate this as part of the rinse cycle as standard. Manufacturersrecommends the use of a rinse aid in order tohelp reduce water spotting on stainless steelinstruments, which can cause rusting andinhibits the lubrication of joints - this is aparticular concern in the case of forceps.

If an instrument fails the protein residue test,firstly, double-check that the washer disinfectoris not displaying any visible error codes andthat it contains detergent. Run another cycleand if the test fails again an engineer should beconsulted, as this could be indicative of aproblem with the washer disinfector itself.

Weekly safety checks are also required for washerdisinfectors to confirm there is no visible damagethat could invalidate its processes. This includeschecking that rotor arms function works withoutstiffness, the door opens and closes smoothlyand the rubber, or silicone gasket around thedoor is not damaged. Although there is no test as such for these checks, they should berecorded and filed as part of the practice’sdecontamination processes.

On a quarterly basis, washer disinfectors mustcomplete a cleaning efficacy test (soil test), using aPCD (process challenge device) and CEI (chemicalefficacy indicator). It’s important to note that this isa manufacturer-led test; therefore their guidelinesshould be adhered to. However, in the absence ofany manufacturer guidance, Then this adviceshould be followed. Finally, all washer disinfectorsrequire annual validation by an accredited engineer.The use of washer disinfectors is considered bestpractice, helping eliminate the need for manualcleaning and reducing the risk of sharps injuries.Ultimately, The Dental Councils Code of Practicerequires all practices to meet best practicestandards of a Washer Disinfector and Vacuumassisted autoclaves.

If any of these parameters are not met, theindicator will not change colour. In the case ofvacuum autoclaves, they also require a steampenetration test once a day, using either a Helixdevice or Bowie and Dick.

A printable data logger automatically recordsand prints out this test. Providing writtenconfirmation of sterilisation times, temperature,and whether the test was passed successfully,creating a unique batch number. If you have aprinter rather than a data logger the test printoutsshould be kept in airtight bags to prevent the inkfading, thereby providing documentary evidenceon the effectiveness of the autoclave.

Older autoclaves, those with an analoguemotherboard, can be connected to a printer.Consult your manufacturer for advice. Autoclaveswithout a data logger or printer can be tested usinga stopwatch to monitor the min/max temperaturetime and sterilisation time, this is the ACT.

These times should be noted in a logbook. When

temperature readings are compared to previous

days they should be the same.

At the end of each day, the inner chamber -

where clean and dirty water resides - should be

physically drained and left to dry. This helps

prevent the formation of bacteria. To keep the

autoclave in good running order, door seals and

gaskets should be dried after each cycle with a

lint-free cloth.

Residual air tests and an air leakage test must

be carried out weekly, however, some autoclaves

cannot perform these tests. In this case, consult

the manufacturer for advice.

Manual cleaningFollowing a wash cycle, instruments must bevisually inspected and if debris is present, theycannot be sterilised until debris is removed. Clinical research states that materials such ascement should be cleaned straightaway, asresearch suggests that doing so makes thecleaning process easier. There are a number ofspecialist pre-wash foams and sprays availablethat help to break down these materials and start the decontamination process before cleaning takes place.

Approximately 70 per cent of dental practices stillconduct some sort of manual cleaning and althoughit is still acceptable and sometimes necessary,manual cleaning is very difficult to validate due toinconsistencies in individuals cleaning methods.Therefore, it’s important to have protocols in place,such as a set immersion technique, which theteam must follow and document.

Tests for autoclavesVacuum and non-vacuum autoclaves requireseveral tests, the first is an ACT (automatic controltest) that tests the parameters of the autoclaveand needs to be completed every morning.

This test necessitates the use of a class 6indicator (also known as a TST), which measurestime, steam and temperature within the autoclave.

This indicator will only change colour when threespecific criteria have been met;

• The time has been achieved (a minimumof 3.5 minutes, or whatever time themanufacturer advises).

• The steam has penetrated into the strip.

• The correct temperature has beenachieved (134˚C - 137˚C).

Firstly, let’s clarify the differencebetween testing and validation.Validation is a way of confirming thatdecontamination equipment is workingin the way the manufacturer intended.Validation also demonstrates thatthe equipment complies with the CE-marking process, under the terms of theMedical Devices regulations; and that allinstruments cleaned by the equipment,are done so reliably and consistently.

According to Code of Practice,practitioners must have theirequipment validated and servicedannually, by an accredited engineer.Nowadays, certainly in England andWales, most service contracts willinclude an element of validation.

In addition, when you purchase decontamination equipment, there are certaintests that you, as the end user, must complete to ensure that the physicalconditions required for decontamination and sterilisation, are always achieved.

PROTEIN RESIDUE TESTSOn a weekly basis, ultrasonic baths must undertake protein residue tests todetermine if any residual protein remains on the dental instruments. The bathwill contain RO or distilled water, and is likely to contain a cleaning solution -enzymatic or otherwise, as recommended by the manufacturer.

At the end of the cleaning cycle, a random instrument should be taken from the bath, rinsedideally with RO or distilled water, then swabbed for protein residue. If the instrument passesthis test, all instruments may be moved to the next stage of reprocessing.

ULTRASONIC ACTIVITY TESTOn a quarterly basis, there are two tests that need to be carried out. The first is a foil ablationtest (or standard foil test), also known as the ultrasonic activity test. This test determinesthat every possible area of the ultrasonic bath has cavitation happening within it.

Once the foil is in place, the bath should be run on a standard cycle, or a time determinedby the manufacturer. Once completed, the foil is removed and blotted dry, and then inspectedfor cavitation. This will appear as little perforations in the foil.

The foil should be cross-referenced with that of the previous quarter to make sure that theperforations are in the same place, or as close as they were previously. If they are not, or there areno perforations, this is an indication that cavitation is not taking place in that part of the bath.These foil pieces should then be stored in a logbook, for reference in the following quarter.

CLEANING EFFICACY TESTThe cleaning efficacy test is the second quarterly test, and is also known as the soil test.

The cleaning efficacy test requires the use of either an STF strip or a PCD (Process ChallengeDevice) and CEI (Chemical Efficacy Indicator). The latter test employs the use of an indicator,which is normally painted red. This goes inside the PCD and then into the ultrasonic bath.After a predetermined time (manufacturer dictated), the indicator is removed and thereshould be no visible red left, which is an indication that the cleaning efficacy is satisfactory.Some manufacturers state this should be a monthly test, so always check first.

SAFETY CHECKSYou should regularly carry out safety checks to ensure that there is no damage to the bath;the inner chamber is in good order, the lid closes properly and there is no visible damage

externally. These checks need to be documented and kept for a minimum of two years, in order to satisfy HSE requirements.

Pouching InstrumentsAll dental instruments should be pouched beforethe autoclave. This process will not only sterlisethe instruments but will also sterlise the pouchso that storage can be undertaken. Any dentalinstrument that is not pouched must be usedwithin the days clinical sessions. The sterlisationof pouched instruments can only be completedthrough a Vacuum, type B, or sometimesvacuum assisted, Type S. Regarding the latterplease consult the manufacturer for advice.

After pouching, it is important not to overload the autoclave. A gap is required between eachpouch, sufficient to allow the air to circulate andeffectively dry the instruments. Pouches packedtoo tightly create bacteria ‘traps’.

Ideally, instruments should be kept pouched until needed. It is currently only best practice tostore sterilised instruments in a dedicated room,so if they are stored in the surgery, instrumentsmust be kept in a closed airtight cupboard, out ofthe 1.5 metre aerosol area.

Cleaning Efficacy Visual Check:This happens after the washer disinfector orultrasonic bath and before the autoclave.Each instrument that has been cleaned mustbe inspected under an illuminated magnifierto ensure that all dirt and debris has beensuccessfully removed. Any instruments thatare found to still have debris must go throughthe cleaning stage again.

Protein Residue Test:This test is used to ensure that the instrumentswhich have been cleaned are free fromprotein that is not visible to the naked eye.After the cleaning process a random instrumentis taken from the load and a protein detectionmethod is used to determine the cleanlinessusing products such as Clean-Trace, ProtestQuick and Medi Check.

Automatic Control Test:This test is used to determine that all thestages of the steriliser are consistent withprevious days. This will involve the user timingeach stage and recording the times in a logbook. The test will then be signed off daily,but only if the results are consistent with theprevious day. If the steriliser has a printer ordata logger this is carried out automatically,providing that cycle monitoring is present (pleaseconsult the manufacturer if you are unsure).

Cleaning Efficacy Test:This test is designed to challenge the cleaningefficiency of the washer disinfector andultrasonic bath. A test soil strip is placed intothe equipment and a cycle is commenced.Once finished the test strip is checked forany remaining soil residue and the resultsare recorded. Please use products such asSTF Load Check, PCD & CEI Indicators orWash Check Indicators.

Ultrasonic Activity Test:This is designed to ensure that the cavitationeffect of the ultrasonic is working effectively

and evenly. Using strips of adhesive tape across the top of the bath, suspend ninestrips of prepared foil in the bath in a 3 x 3 grid. Ensure that the rolled bottomend of each foil strip is no more than 10mm above, but not touching the bottomof the bath. Run the bath for a pre-determined time, remove the foil strips and blotdry and record the results. The strips should all reflect similar erosion patterns.

Steam Penetration Test:This test is used for Class B and S sterilisers only. This is designed to ensure thata successful vacuum has been achieved so the steam is able to penetrate aninternal lumin. This will involve one of two tests, either a Helix or Bowie and Dick.Please consult with your autoclave manufacturer as to which test is needed.

Air Leakage Test:This test is designed to show that, should a leak be detected, the machine willfail the cycle. Dependent on the age of the autoclave this may be carried outautomatically. If you are unsure please consult the manufacturer.

CP(D) Engineer Validation:The validation should be carried out by a qualified engineer and is designed toshow that the equipment is working within preset parameters and as dictatedby the manufacturer.

Expert advice - For cleaning efficacy tests and protein residuetests, where the cycle does not have a rinse stage, items shouldbe rinsed as a normal procedure before these tests are carriedout, otherwise the tests could return false positives.

A likely overlap may mean that this role is duplicated, but theresponsibility must be demonstrated.
