1, Sheik Alla Baksh S/o Sheik lmam Saheb, Retired CGSR / Cuddapah office of SS/HX of Guntakal

Division submit the following details for favour of information.

1 have retired on superànnuation on 31/06/1996 aftef completing 38 years of service in

various cadres in Commercial Depadment as CGSR at Kadapa. My date of Birth is

01/07/1938 and my PPO No. 5906111 1697.

I am drawing my pension through State Bank of India, Kadapa Bazar Branch from SB N c

No. 10437350896 having IFSC No.: SBINO003730 and MICR No. 516002002 with PPO

unique ID No. R02877346

3. At the time of settlement of my retirement benefits, an amount of Rs. 10000/- was withheld

towards depadmental dues and Rs. 2000/- towards ftlture dues totalling Rs. 12000/- vide Lr.

No. A/PEN/GTURLYS/II69; dated 30/06/1996.

4. After several persuation, Vide Lr. No. G/C419/11/1045/96 dated 03/09/1997, Sepior

Divisional Commercial Manager / GTL has issued ''NO DUES'' in my favour and informed

the same to Sr, Divisional Personnel Officer / GTL.

In turn Sr. Divisional Personnel O#icer / GTL has issued Pay Order No. 888659 dated

23/09/1997 vide Lf . No. G/P500/ll/A854 dated 23/09/1997 to Sr. Divisional Accounts Ofîicer

/ GTL (Now Sr. Divisional Finance Manager) to release the withheld amount of Rs. 12000/-

1 have retired on superannuation on 31/06/1996 after completing 38 years of service in

various cadres in Commercial Depadment as CGSR at Kadapa, My date of Birth is

01/07/1938 and'my PPO No. 59061111697,

j am drawing my pension thtough State Bank of lndia, Kadapa Bazar Branch from SB A/cNo. 10431350896 having IFSC No.: SBIN0003730 and MICR No, 516002002 with PPOuniqùe ID No. R02877346

3, At the time of settlement of my retirement benefits, an amount of Rs, 10000/- was withheld

towards departmental dues and Rs. 2000/- towards future dues totailing Ré. 12000/- vide Lr,

No. A/PEN/GTL/RLYS/I 1697 dated 30/06/1996,

After several persuation, Vide Lr. No. G/C419/Il/1045/96 dated 03/.09/1997, Senior

Divisional Commercial Manager / GTL has issued ''NO DUES'' in my favôur and informed

the same to Sr. Divisional Personnel Officer / GTL.

5. In turn Sr. Divisional Personnel Ofhcer / GTL has issued Pay Order No, 888659 dated

23/09/1997 vide Lr. No. G/P5OO/II/A854 dated 23/09/1997 to Sr. Divisional Accounts Officer

/ GTL (Now Sr, Divisional Finance Manager) to release the withheld amount of Rs, 12000/-

withheld amount has not been credited to my SB A/c to the best of my knowledge.
