The ITI Section South East Asia would like to invite you to its first ITI congress with the theme

“Achieving Esthetic Success in Implant Dentistry” that will take place at the Centara World

Congress Center in Bangkok on May 16 and 17, 2013.

The scientific faculty headed by Professors Daniel Buser (University of Bern) and Urs Belser

(University of Geneva) is supported by distinguished regional speakers, who will share the platform

to present the latest information on esthetics in implant dentistry. Topics under discussion will

include:

Professor Buser:

• Anatomical and surgical aspects of implant placement in the esthetic zone

• Implant placement post extraction: treatment options and recommendations

• Long-term stability of contour augmentation with GBR in esthetic sites

• Surgical handling of esthetic failures

Professor Belser:

• Treatment planning principles for the esthetic zone revisited – a critical appraisal

All fees are quoted in Thai Bhat (THB) and include any applicable taxes. To benefit from reduced rates, please return the registration form by January 31, 2013 at the latest.

The late registration fee will be applied to any forms postmarked later than January 31, 2013. Registrations received after April 30, 2013 will be processed as last-minute registrations and will not be confirmed before the congress. For last-minute registration, please show proof of payment or pay directly upon arrival at the registration desk.

If fees are forwarded late and are not credited to the account of CDM by the start of the ITI Congress South East Asia 2013 (bank transfers may take up to 4 working days), a copy of the remittance order certified by your bank must be supplied as proof of payment.

As only a limited number of attendees are allowed into the ITI Congress South East Asia 2013, we recommend you register early enough in advance to avoid any kind of disappointment.

Please note that confirmation of your postgraduate status signed by the Program Director or Dean is mandatory in order to benefit from the reduced registration fee. Please send this letter to the CDM registration office (Email: [email protected] / Fax: +66 2965 8919). Your registration will only be accepted after the confirmation letter has been received.

Registration fees for the ITI Congress South East Asia 2013 must arrive at CDM at the same time as the registration form or within the following two weeks, at the latest two weeks before the congress (by May 1, 2013). The only currency accepted is Thai Bhat (THB).

All payments must be addressed to: CDM, indicating the keyword “ITI Congress South East Asia 2013”. Please also state the participant’s name clearly as otherwise payment cannot be identified and assigned to your registration. If payment is made by bank transfer, please ensure that any transfer/bank charges are covered by you and not passed on to CDM. The full registration fee must be received by CDM to ensure your registration.

Registration will be confirmed by email or letter once payment has been received. Should you need an invoice in order to effect payment, please contact the registration office. Please show the booking confirmation to collect your congress documentation on site at the congress venue.

Registration of postgraduate students will be processed once the status confirmation letter has been received by CDM.

CDM must be notified of any cancellations in writing, either by letter, fax or email.

Cancellations received by January 31, 2013: full refund of fees paid less THB 800 administrative fee

Cancellation received after January 31, 2013: no refund can be made

All delegates will receive a certificate of attendance, indicating the hours attended the congress (breaks and corporate sessions will be deducted).

The certified hours are convertible into CPD credits within the delegate’s country of origin. Delegates are asked to submit the certificate of attendance to the respective authority within their country.

All lectures will be held in English. Simultaneous translation is not available.

The ITI is unable to make any hotel bookings on behalf of the delegates. Thus it is the delegates’ responsibility to book their accommodation on their own.

A lucky draw will be conducted in early February. Amongst all bookings received before January 31, 10 rooms will be granted a free of charge upgrade to the “Deluxe World” category. The respective delegates will receive a notification by email.

During the congress an exhibition will be held. It will offer an excellent opportunity for com-panies to show their latest products. Companies wishing to exhibit may obtain an exhibitor manual by emailing their contact information to [email protected].
