International Visiting Graduate Student (IVGS) acceptance

Use this form to: • Request acceptance to participate in research activities at the University of Waterloo as an international visiting

graduate student. Instructions:

1. Complete sections 1 and 2. Obtain the signatures from your institution in section 3.2. Submit the completed IVGS Acceptance form and supporting documents (as indicated under the declaration section

of this form) to your Waterloo supervisor. Your Waterloo supervisor will submit the signed form and supportingdocuments to the administrative assistant of the academic department for the chair’s approval.

3. The approved IVGS Acceptance form and supporting documentation will be forwarded to the manager, graduatereporting and mobility program, Graduate Studies Office, Needles Hall.

Important note: the international visiting graduate student’s registration may be terminated at any time with written notice by the University of Waterloo.
