INSURANCE HIGHLIGHTSThis Program provides replacement handset service to Cricket Communications customers enrolled as certificate holders under Master Policy No. IM 5470993. + You are not required to purchase insurance to activate wireless service. You are not required to enroll in the portable electronics insurance program in order to purchase or lease portable electronics devices or services. The employees of this location are not qualified or authorized to discuss or evaluate insurance coverage. Any questions regarding the Handset Protection Program should be directed to eSecuritel at (855) 309-8342.
+ You are not required to purchase the Service Warranty Agreement to purchase the lost, stolen and accidental damage coverage under the Handset Protection Program. You may purchase the insurance on a standalone basis for a monthly premium of $4.00 or $9.00, based on your handset, by calling eSecuritel at (855) 309-8342.
+ This coverage may provide a duplicate of other sources of coverage. Contact your insurance agent to determine if you have coverage for your wireless device under existing insurance policies you may own for your wireless device. Your renters or homeowners policy may provide the insurance you need. Ask about any policy exclusions. Review your deductible and coverage amounts to be sure they still fit your needs.
+ You are not eligible for the Handset Insurance Program if you have completed 2 claims within the past 12 months.
+ You may cancel at any time by calling Cricket Customer Service at 1-800-CRICKET (1-800-274-2538). Any unearned premium will be refunded in accordance with applicable law.

SERVICE WARRANTY AGREEMENT HIGHLIGHTSThis Service Warranty Agreement may be referred to as an
extended limited warranty in CT, DC, FL, IN, LA, ME, MA, MI,
NJ, NC, PA, RI, SD, or WI and a service contract in all other
states. THIS SERVICE WARRANTY AGREEMENT IS NOT A
CONTRACT OF INSURANCE.
Service Warranty Coverage
Replacement of Wireless Handset if, under normal
conditions and use, the Wireless Handset on record with
eSecuritel fails to operate properly due to manufacturer’s
defects or workmanship after the expiration of the
manufacturer’s warranty.
Eligibility
The Service Warranty Agreement is not available for
purchase on a standalone basis. You must remain an active
enrolled customer of the Handset Protection Program
provided by Cricket Communications, under Master Policy
No. IM 5470993 to be eligible for this Service Warranty
Agreement. If you fail to remain an active enrolled subscriber
of such Handset Protection Program, regardless of cessation
reason, this Service Warranty Agreement shall be terminated.
Service Fee
Service Fee is one dollar per month.
Purchaser & Seller
The Purchaser (Subscriber) of this Agreement is the owner
of the covered equipment. The Seller of this Agreement is
listed on your receipt.

+ The obligations of eSecuritel under this Agreement are backed by the full faith and credit of the eSecuritel’s parent company Brightstar Corp 9725 NW 117th Ave, #300, Miami, FL 33178 (1-304-421-6000) except in the states listed below where the obligations are insured pursuant to a service contract reimbursement insurance policy issued to eSecuritel. If eSecuritel does not perform its obligations hereunder within sixty (60) days after the Subscriber files a claim with eSecuritel, the Subscriber is entitled to file a claim directly with insurer indicated for the customer’s state at the below address.

HOW DO I ENROLL?It’s simple! To be eligible for immediate coverage, sign up when you purchase or upgrade to a new device or within 30 days of purchase or upgrade. If you enroll after 30 days of purchasing your device, your coverage will begin 30 days after enrollment. Please note, your handset must be fully operational at the time of Program activation to be eligible for coverage.
HOW DO I REPORT A CLAIM?Step 1: If your device was lost or stolen
Call Cricket Customer Care at (800) 274-2538 to suspend your service and protect against unauthorized use of your account and device.
Step 2: Call eSecuritel
Call eSecuritel’s Customer Care Center at (855) 309-8342. Claims must be reported within 60 days of the incident, loss or first failure. Proof of loss, ownership, and/or identity will need to be provided, if requested. Only the account holder or authorized user of the account may file a claim.
Please have the following information ready:
+ The wireless phone number + The account holder’s billing address + Wireless phone model and unique serial number (ESN or MEID) + Date, time, location and detailed description of the incident or first failure + Signed affidavit of loss or theft (if requested)
Please keep your replacement device’s receipt, original box, manual and all packaging materials for warranty purposes.

The monthly subscriber fee includes the insurance premium and service contract fee. Deductibles are based on the non-contract, non-subsidized new retail price of the phone model when you originally enrolled your phone. The deductible must be paid before
you receive your replacement equipment and is non-refundable.
What is covered?
The Handset Protection Program covers the wireless handset (device, standard battery & wall charger) from loss, theft, accidental damage (including liquid damage) and malfunction (after the manufacturer’s warranty expires).
What is not covered?
Normal wear, pre-existing damage/malfunction, and cosmetic damage to your wireless handset are not covered. Other exclusions apply.
When am I covered?
To be eligible for immediate coverage, you have 30 days from purchase of your new Cricket handset or upgrade to enroll in the Program. If you select coverage afterwards, your coverage will begin 30 days following purchase of the Program.
Is the program renewable?
Sure! Renewals are automatic for as long as you pay your monthly subscriber fee.
What are the claim limits?
Up to 2 replacements within a 12-month period for both your lost, stolen, accidental damage and malfunction coverage components of the Program.
What kind of replacement equipment will I receive?
Replacements may be new or refurbished equipment. Refurbished equipment may have minor cosmetic flaws and contain non-original manufacturer parts and accessories. If the exact make and model is not available, your replacement will be of like kind and quality.
How will I be billed?
Your monthly subscriber fee will be added to your monthly Cricket Communications wireless bill. Payment is due in advance of coverage.
How can I cancel?
You may cancel at any time by calling Cricket Customer Care at 1-800-274-2538.
