Instructions for Renewing a Registration as a Backflow Prevention Assembly Tester (BPAT) with the Iowa Department of Public Health

Begin by opening your web browser and navigating to the Iowa Department of Public Health Regulatory Programs webpage by clicking on the link below or typing it into your browser:

CREATE AN ACCOUNT

Your first step will be setting up an account the State of Iowa’s Authentication and Authorization (A&A) service, unless you already have an A&A account. A&A accounts are typically in the format of firstname.lastname@IOWAID. A&A is a payment system shared among many state agencies. NEW USERS: To start the process, you will need to create an account. Select New User Registration.

If you know that you already have an A&A account or if you get a message stating that you already have an account, please click Sign In rather than Create An Account on the below screen. If you have forgotten you’re A&A account password, please click Forgot Password or call the A&A helpdesk for assistance at 515-281-5703 or 800-532-1174 To create an account:

Enter your First Name and Last Name Click the Register button.

NOTE: You must have a valid personal email address to create an account. • This screen requires you to enter your email twice. All other fields should be completed

automatically, if the @iowaid field is blank, enter your first name.last name.

• Enter your email address in the Email: box and again in the Confirm Email: box

• Click on the Save Account Details button. If a message appears stating that the email is already being used by an existing account, you will need to call the OCIO service desk at 515-281-5703 or 800-532-1174 for assistance.

On this screen, click the OK button.

The next screen will direct you to check your email (the email address you entered on the previous screen) to complete the process of creating an account.

Open the email with the subject “Account Confirmation”. Next click the link in the body of the email in Step 1. under Account Activation Process: section.

You will be taken back to the A&A account setup website shown on the next page.

Select security questions for Question 1 and 2 and provide and confirm answers. Write a security question for Question 3 and provide your answer. Click Save Identity Baseline when done.

Next, you will need to create a password for your account by entering it twice, once in the “Enter new password:” box and again in the “Confirm new password:” box.

Click the Save New Password button when complete

Once you have successfully signed in, you should be taken back to the IDPH Regulatory Programs page.

Enter your Social Security number in the “SSN” box without any dashes, enter your Social Security number again in the “Confirm SSN box”, and Date of Birth in the correct box.

Click the Continue button .

You are now on the Web Registration Profile page to set up your profile.

Your email address, SSN and Date of Birth will be prefilled.

Complete or update the Basic Profile Details and Physical Address Details section, entries marked with a red asterisk, *, are required fields.

NOTES for completing this section:

• Please provide your full, legal name, can use Initial for middle name. • Click the down arrow next to some boxes to make a selection if necessary. For City – please

type in city name and do not use dropdown arrow. • Please enter your home address. If you use a PO BOX you must still provide a Street number and

name for your physical address. • Unit Type is for entering; PO BOX, APT, Unit or Suite The Unit Number is the corresponding

number and/or letter. • At least one phone number is required then you may also provide a work phone and cell

number.

Click Continue button at bottom when finished.

You are now on the Home>My Profile screen.

Verify all information is correct. You can make any corrections necessary except for your name or date of birth. Please contact the AMANDA help desk if either of these needs corrected.

Record the PIN in upper right corner of the screen, this can be helpful if you have problems and need to contact the helpdesk or for your training provider if they needs to roster you to a class.

Click Continue button when finished

You are now on the My Programs page. This is where you will renew your Backflow Prevention Assembly Tester registration.

FOR THOSE WHO COMPLETED AN IOWA APPROVED CONTINUING EDUCATION (CE) CLASS PLEASE READ

Your training provider must link your account to the CE class session in AMANDA before your renewal can be completed. If you have not completed an approved CE class stop here.

If you have completed CE class, you may want to verify with your training provider that they have linked your account to the class session before proceeding. You can continue and complete the application but your renewal will not be issued until your training provider or BPAT program staff has made the link.

Have the PIN umber you recorded from the previous page handy.

Click Sign Off to log out of your account if you wish to stop here.

To finish renewing your BPAT registration you will need to log into your account here:

If you are returning after logging off, click on Sign In, the below My Programs page should appear. Otherwise continue below

Your existing licenses including your BPAT license will show under the Programs for [Your Name], License # section. All numbers now start with BPAT. Please contact the department if your BPAT license is not showing.

Click on Renew.

You will get the following pop-up message asking if you are sure, click OK

You are now on the Backflow Prevention Application Form page.

Read the instructions carefully before trying to complete the three sections under the Application Form area.

Click the Expand All when ready to complete the Affirmation, Third Party Certification and Backflow Tester Out of State Credentials sections. All 3 of these sections must be completed even if they do not all apply.

Affirmation Section

All 6 questions must be answered. If a yes answer is provided, a brief description of all relevant activities must be entered in the area provided and additional details can be provided in an attachment if necessary. Be sure to attach any other documentation specified.

NOTE: These questions have changed since the last renewal period or if your initial application was received prior to January 1, 2017. You only need to answer “Yes” if the issue or incident has occurred since you last submitted a BPAT renewal or initial application or you have not reported it in the past.

See next page for instructions on adding attachments specified by any of the questions below.

To add any required documentation, you will need to click the Add New Attachment button at the very end of the application form section.

When clicked the Atatchemnt Description area will expand.

To select a Type of attachment, click the dropdown arrow.

Enter a description of the file, Click Choose File button .

This will open your file explorer, navigate to the directory where the file is you want to attach and double click it click it once then click the Open button in the lower right corner of the window.

The name of the document should be showing next to the Choose File button.

Continue this process for each document needing to be attached.

NOTE: If you attach a document in error, it cannot be removed by you. You will need to contact the Backflow Assembly Tester Registration Program staff listed at the bottom of page 1 of these instruction to have it removed.

Select one of the following from the list:

• Court Documents • License • Non-Iowa Permit/Certification/Registration • Physicians Record • Proof of Certification

Third Party Certification and Out of State Credential/Training Sections

If you took an Iowa approved 5-Hour continuing education course, answer No to both.

If you are using this section to qualify for renewal, only one of these sections should marked yes.

If you answer yes to either of these, additional documentation must be uploaded/attached (i.e.: copy of your current card or certificate). Follow instructions on previous page for adding attachments.

When all sections are complete, click the Continue button at the bottom.

Be sure to attach all requested supporting documentation before you pay the registration fee or your application will be delayed or may be denied. If you need to gather information you can leave the application webpage and return later to continue. Do not hit the Cancel button, this will void your entire application.

When you click Continue you will get the following confirmation pop up message, click OK to continue.

You are now taken to the Terms and Conditions page.

Please read the terms and conditions and if you agree click the box next to I agree with the terms and conditions statement to check it.

Then click the Continue button .

You are now on the Make Payment page.

Please read the directions on this page before proceeding.

Click the Pay Later button if you need to attach additional documentation as requested on page 14 or 16.

If you click the Pay Later button, you will get a reminder pop up that your application is not considered submitted until payment is made. You will be returned to your My Programs page where you will see your registration listed and its status.

If you click the Pay Now button, you will be directed through the online payment system. Continue to select Pay Now when asked, and complete your online payment information, skip to page 19 to complete payment process.

The next few pages explain how to return to the system and complete an application if needed.

If you need to return to the application to pay or add attachment, double click on Details to reopen your application.

If the Details section says Edit, this means that you have other questions within the application forms to answer. Please review all sections of the form before attempting to pay.

To add attachments, go towards the bottom of the Program Details page click on Add New Attachment or Upload Attachments to add the additional documentation required.

To make a payment, click on Make a Payment and follow the directions through the payment program.

If you click the Home button, you will be taken to the Home screen and if you are done, you can click Sign Off and you will be taken to the below screen.

If you clicked My Profile you will be taken to your information page, see page 10, and if you are done, you can click Sign Off and you will be taken to the below screen.

If you clicked the My Programs button, you will be taken to your main licensing information screen where you can see the status of your license, see page 11, and if you are done, you can click Sign Off and you will be taken to the below screen.

If there is no required review by program staff and if needed, your training provider has linked your account to a class session you will be emailed your registration card with in typically 1-2 business days. If you did not provide an email address, it will be mailed to you. If further program staff review is needed you will be contacted if additional information is needed or your card will be issued when review is complete. You can verify your status by returning to the above page and clicking on Public Search and search on your name.
