Instructional Guide: How to register for Go365 & complete your Health Assessment
Online Instructions
Online: Registration
Visit www.livingwell.ky.gov and click the link near the bottom to log in to Go365
Online: Registration
Click “Register now” or sign in with your HumanaVitality username and password*
* If you have previously created a HumanaVitality account, you will NOT register for Go365, you are already registered, sign in with the same username and password.

Enter the requested information, then click “Continue”
DOB and Zip code must be an exact match to what is on file with Go365/ Humana.
The member ID number will be located on your Go365 ID card. This is the same as your former HumanaVitality ID number. Contact 1-855-478-1623, if you need this number.

Fill out the requested information, then click “Submit”
Tip: Write down username and password or store it as a contact in your phone for future use!
Click the button “Continue to Go365”

Click on the alert message or click the button “Retake Your Health Assessment”
Click the “Start” button in one of the 6 sections to begin

Remember: In order to receive credit for completing the LivingWell Promise, you must complete ALL 6 sections.

Click and drag the blue circle to answer the following question about your physical activity, then click “next”
Note: There are 7 questions in this section.
Answer the following question about your eating habits, then click “next”

Answer the following questions about your mental health, then click “next”

Answer the following question about your lifestyle habits, then click “next”

Answer the following question about your medical history, then click “next”

Once all 6 sections are completed, you will receive your Go365 Age!

If you have completed all 6 sections of the Health Assessment and received your Go365 Age, your LivingWell Promise has been kept.
Only the primary plan holder is responsible for completing the LivingWell Promise. Cross-reference members are BOTH responsible for
completing the LivingWell Promise.
From the Dashboard, scroll down to find the Achievements tile and click on “View Achievements”

Click to download the “Achievements report PDF” for proof, or scroll down to see the date you completed your Health Assessment

Mobile App: Download
Download the Go365 App* from the App Store for iPhones or the Google Play Store for Androids
*You will have to download the new Go365 App, the old HumanaVitality app can be deleted.

Answer all of the questions within each section, then tap “Submit”

Your Health Assessment will be complete when ALL 6 boxes have a check mark
