1. Introduction

During the inspection of the project the staffs of Independent Engineer and

Concessionaire’s representative and EPC /Sub-Contractor’s representative were

2. Concessionaire Progress

Work Progress:-

1. The Project Highway activities are in progress at Durgapur section, Asansol section and

Raniganj section for Highway and structure works.

2. At Dhanbad (Jharkhand) section work has been stopped due to APPCO (sub-Contractor)

has left the site with his all resources.

3. The concessionaire has beenadvised to immediate arrange the other sub-contractor to

start the work activities at all available front in Jharkhand section.

INDEPENDENT ENGINEER OBSERVATIONS ON THE

STATUS OF WORK & PROJECT ISSUES

A. PROJECT ISSUE:-

1. GEECL Gas Pipeline:- The Gas pipeline is running parallel along the project

stretch on LHS from Km. 455+750 to Km. 505+900 affecting approximately 25

km.SR/MCW.:- Decision Pending. GEECL is adamant to shift the gas pipeline.

2. Govindpur and Nirsa ::-

Govindpur (Km 406+500 to

Km 410+000) :- Decision for “In-Principle” approval is required from

Competent Authority to make 6-laning work (At-

Grade) in 45m restricted width of Govindpur and

Nirsa Bazaar.

Nirsa (Km 426+700 to Km

430+000) :-

No work at Nirsa and Govindpur even measurement of encroached portion and land

acquisition within 45m and at Junction 60m not to be done due to protest by people

there. D.M., Dhanbad has informed by PD weekly regularly. A joint meeting will be

organize by DM, Dhanbad with all stake holders, M.P., MLA, etc in future soon.

3. Bograchatti:-. Land acquisition processes held up due to D.M. order from Km 468+000

to Km.470+000. DGMS report is against construction there. Further action to be taken

by Authority. As per IIT Kharagpur suggestion it will take 3 years to complete work

there.

4. Front Availability of Project Highway:-105.51Km (85.86%) of MCW(equivalent six-

lane) and 105.210 Km. (70.57%) of SR land handed over to Concessionaire as on 30th

September 2017.

B. QUALITY CONTROL:-

1. Documentations of Quality Control to be maintained in Lab, Structure-wise for

Structure and Chainage-wise (Km-wise) of Road Work for each Activity/Item

Separately for the executed work.

2. “Material” to be used only from Approved Source as per Section 100 Clause 117 of

MORT&H Specifications.

3. Stock Register for Material like Cement, Admixture, steel, Bitumen and other major

material to be maintained properly in Lab.

4. Third Party Test of Materials i.e. Cement, Steel, Bitumen, Emulsion, Admixture etc

are to be conducted Frequency Wise as per IS/IRC and record to be kept in order.

5. Third Party Test of Sub-Grade material and CBR to be conducted @ 10km interval

and record to be kept in order.

6. The Concessionaire has been instructed to follow the RFI system properly in time

regularly. It should be provided at site on daily for inspection without any delay in

future.

C. PROGRESS OF WORK(CONCLUSION):-

1 Concessionaire has advised to start the activities at all available front where land is

available.

2. Panagarh bypass:.

Old ROB rehabilitation work is under progress at Km. 520+373. It should be likely to be

completed by 31st October 2017.

Provision of signages in completed stretch should be strict compliance of IRC 67 and IRC

SP 87 and Concession Agreement.

Direction Information Sign boards for Fly-over/VUP approaches should be as per Fig.9.2

of IRC SP 87:2013.

Some remarks regarding directional sign boards are given in letter no. SAICPL-

Sugam/Barwa-Adda-Panagarh/IE/NH-2/2014/2454 dt. 05.09.2017 should be implemented

immediately.

Reflective Pavement Markers (Road Studs) should be fixed as per Table 9.1 of IRC SP

87:2013.

3. Concessionaire has been advised to expedite the activities of work at available front

location specially at section – I in Jharkhand, approx 11.3 Km. land is now available.

BUT WORK HAS BEEN STOPPED SINCE 15THSEPTEMBER 2017 DUE TO

APPCO (SUB-CONTRACTOR) HAS LEFT THE SITE WITH HIS ALL

RESOURCESS AND WINDUP ALL ACTIVITIES.

4. Longitudinal and horizontal joints of BC layer at some location still unsatisfactory.

Concessionaire has been instructed to the same should be rectified and please ensure

the above defects should be strictly avoided in future.

5. Damaged portion of Service road at km 491(RHS), Km.509+570(LHS) near VUP&

Km. 447+780(BHS)should be immediate rectified as per T.S. But, still not rectified.

6. Concessionaire has been instructed to submit the drainage arrangement for super

elevation portion of MCW with Out-let arrangement of entire section of project

highway. Still not submitted.

7. Laying of CC Paving blocks have not been done as per specification in separator

portion at Section III. Laying of CC Paving Blocks should be strict compliance of IRC

SP 63.No any action has been taken by concessionaire till now.

8. The construction of super structures at various location such as G.S. 464+185 in

progress and required to continue fence with heavy duty safety net under the

construction area (Super-Structure) from safety aspects barricading of construction

areas.

9. Cracks in service road from km 520+070 to km 520+100(RSR), km 520+470 to km

520+500(LSR) and Km 512+6000 to Km 512+900(LSR) has to be rectified as per

Technical Specification. Still not rectified at site.

10. The surface of RCC wall, Girder, deck slab of Grade Separator and VUP needs to

finish by grinding at joints at Ch. 456+312, Ch. 464+185, Ch. 469+750, Ch. 496+609,

Ch. 501+996, Ch. 504+287 & Ch. 506+200.

11. Fixing of PGR in section II have not been done as per drawing. Concessionaire has

been instructed to rectify it. But, till not rectified.

12. Inadequate signages and safety arrangement have been observed at separator portion

between service road and MCW in Section I.

13. The Horizontal alignment of crash barrier of Grade separator at km 456+360(LHS)

has been observed that out of tolerance. Concessionaire has been instructed to submit

the co-ordinate of this section for further necessary action.

14. Pop out and broken corners observed in some panels of Innovative Concrete at

Panagarh bypass section. The same should be repaired immediately as per guideline

of IRC-77. Still nor rectified.

15. It has been observed that in median portion, without providing the sufficient layer of

good quality soil, proper preparation of median surface and required size of pit,

plantations activities have been done. This is not acceptable. Concessionaire has been

instructed that in median portion all unsuitable soil should be replaced by good soil at

least 300 mm depth and ensure that the plantation activities should be done as per

guide lines of IRC SP: 21.NOMINAL PLANTS ARE ALIVE IN SECTION – II

&III,MOST OF THEM ARE DEAD.

16. It has been observed that concessionaire has started the slope protection activities at

km 462+240 with Geo green blanket (Coconut husk). Concessionaire submitted the

methodology and drawing to IE for its review. Concessionaire has been asked for

some observations by IE. It is still not submitted by Concessionaire for further

review.

17. At so many location kerb & RCC crash barrier painting has been left his layer with

the concrete surface. Concessionaire has been instructed to re do the defective section

of kerb painting. Still no any action taken by the concessionaire.

18. Slope pitching in approaches of minor bridge at ch. 405+345 & ch.415+115 has not

been done as per specification and reviewed drawing. Thickness of pitching observed

less than the specified limit. The same should be rectified.

19. Concessionaire has not taken any action against providing and fixing of Anti-glare in

reduced median section having width 2.5m and below in the project highway.

20. Concessionaire has been already requested to submit the details drawing of anti-glare for

IE review. But still not submitted by concessionaire.

21. Concessionaire has taken up some activities without consent of IE and modified the

IE reviewed drawing by itself and work executed accordingly at site. This is the

violation of Concession agreement and not acceptable. Concessionaire should submit

his comments with proper justification immediately.

22. Concessionaire has been requested to take necessary action to provide the required

sign board, road marking & safety arrangement as per IRC:SP 55, IRC 67, IRC :SP

87, IRC 35 & CA guidelines.

SAFETY & MAINTENANCE OF EXISTING ROAD:-

1. The overall safety measures during construction at site were inspected/reviewed and still

required adequate arrangement for safety.

2. All Deep excavation portions should be barricaded and concrete crash barrier to be

provided.

3. Concessionaire was also advised to ensure the safety of workers by providing PPE

(personal protective equipment’s) to the workers/ labour during execution. During visit, it

has been found that at many structure locations, workers are working without PPE &

safety shoes.

4. Advance marking sign regarding portion of any of the highways damaged/closed or

diversion or any other information which needs to be advised to the road users should be

kept ready to enable immediate necessary action in the matter to be able to ensure hassle

free flow of traffic on existing highway.

5. Unauthorized median opening should be immediate closed and median opening signage

should be provided at authorized median opening locations.

6. Signage, Retro Reflective Tapes, Warning Sign should be provided at construction zone

portions and also arrangement of traffic during construction and maintenance shall

confirm to the requirements of IRC:SP:55, clause 112 of MORT&H Specification and

provisions of concession agreement.

7. Cleaning of carriageway, median, bridge footpath & shoulders to be completed

immediately. MCW (median side)and service road (drain side) cleaning is not

satisfactory.

8. Waste materials/debris from shoulder side and where new signages have been fixed

should be immediate removed.

9. All junction location existing/new Kerb should be painted immediately as per Technical

specification. Paint should be confirm as per IS-164 requirement

10. All roots of trees etc. on existing bridges, ROB should be removed immediately and

cleaning should be done immediately. Roots of grass in median are hanging towards curb.

It should be cut immediately.

11. Pot repairing is slowly going on but not as per specifications. Pot repair portion is

bumping during travel by vehicles. Concessionaire has been reminded to do work swiftly

with proper rolling and in shape and size as per specifications. Formation of potholes are

observed at Kalla More Minor bridge (RHS), Singaran Bridge (RHS) in service roads at

Gandhi More, ABL – Indo American More, Muchipara are also in distressed

condition.REQUIRED COMPLETE RENEWAL.

12. Over all pot holes repairing are not satisfactory.Series of Pot holes observed at Raniganj

Service road and Birangi (SR) have not been entertained during this period.

Concessionaire has been instructed to immediate fill the pot holes.

D. HINDRANCES AND OBSTRUCTIONS:-

1. Acquisition of Balance Land: - Balance land handing over in Jharkhand is going on

swiftly now 11.3Km. front has available to concessionaire for work. But work has been

stopped by APPCO (Sub-Contractor) and left the site and windup all activities.

2. Shifting of Utilities i.e. Water line/PHE, Electric poles and towers (partly), Gas pipe

lines, Petroleum pipe lines. Presently Electrical utility work, water pipe line and oil

pipeline are in Progress but needs to be expedited for Completion of Project Highway on

schedule date. There are hindrances persist at many places for want of this concessioner

is facing problem and progress work is delayed.

3. Relocation of Religious Places:- Shifting of religious structures to be expedited by NHAI.
