Inspection Report
of Independent Engineer- Barwa-Adda - Panagarh section of
NH-2
Independent Engineer services for 6-laning of Barwa-Adda -
Panagarh section of NH-2 from Km. 398.240 to Km 521.120
including Panagarh Bypass on DBFOT Pattern.
1. Introduction
During the inspection of the project the staffs of Independent Engineer and
Concessionaire’s representative and EPC /Sub-Contractor’s representative were
present.
During inspection and site visit, the Concessionaire’s representative &
Independent Engineer’s key personnel were discussed in meeting on various
issues as mentioned below.

2. Concessionaire Progress
Work Progress:-
The Concessionaire has already started the activities at Durgapur section,
Asansol section and Raniganj section and Dhanbad section for C & G,
Embankment, Sub-grade, GSB, WMM, CT sub base/base course , DBM,BC,
widening of pipe culverts, Construction of box culverts, PUPs and Vehicular
under passes, Grade separators, Over Passes, Under Pass, Minor bridges &
Major Bridges.
The concessionaire is being advised that to start the work activities at all
available front and increase the resources and plan & Machinery should be
mobilized fully and put together to give full momentum to the progress of the
project highway with proper methodology and well planned sequence of operation
should be used to have better results and desired rate of progress of the project.

INDEPENDENT ENGINEER OBSERVATIONS ON THE
STATUS OF WORK & PROJECT ISSUES
A. PROJECT ISSUE:-
1. Progress of work to be ensured as per Project Milestones of Schedule-“G” of Concession
Agreement.
2. DW&SD - of Jharkhand State (Km. 405+350 to Km. 436+700):- NHAI has
recommended to the competent authority the modification to project plan,
profile/features from Km. 405+350 to Km. 436+700.
a. IE reviewed the concessionaire proposal and issued the GFC of P&P and COS
for principal approval has been done by NHAI. Work is going on.
3. Godindpur and Nirsa (Govindpur : Km. 406+460 to Km. 410+000 and Km 422+000 to
KM 435+000) for COS and construction in restricted width of 45 mtrs.):- Construction
in above stretch is not started for want of principal approval of COS and construction
in restricted width of 45m.
(A) Govindpur Section :
a. IE has reviewed & issued the GFC of P&P for Govindpur Section.
b. Principal approval for COS and construction in restricted width of 45mtrs
is still awaited from NHAI. Concessionaire wants Principal Approval from
NHAI for doing work in 45M width.
(B) Nirsa Section : Meeting chaired with Honorable Minister of MoRTH a committee
was formed comprising of DGMS/NHAI RO, PD,IIT Kharagpur Civil Department
HOD/MLA-Nirsa to jointly finalize the project highway proposal.
a. Mr. P.N.Singh, M.P., held the meeting with Nagrik Samittee, Nirsa, NHAI
Concessionaire, I.E. and it is decided to do work in 45meter width available
without further acquirement of land, demonization of shop & building except
where VUP is to be provided as per CA for time being. Work is held up from
Km 427+000 to Km 430+000 still due to Public Protest. In-Principal approval
is still awaited from the Competent Authority in restricted width of 45m ROW
verses 60m ROW.
b. IE has reviewed the plan for Nirsa Section in restricted width of 45m ROW.
4. Bograchatti :-. Land acquisition processes are under progress from Km 468+000 to Km
470+000
B. QUALITY CONTROL:-
1. Documentations of Quality Control to be maintained in Lab, Structure-wise for
Structure and Chainage-wise (Km-wise) of Road Work for each Activity/Item
Separately for the executed work.
2. “Material” to be used only from Approved Source as per Section 100 Clause 117 of
MORT&H Specifications.
3. Stock Register for Material like Cement, Admixture, steel, Bitumen and other major
material to be maintained properly in Lab.
4. Third Party Test of Materials i.e. Cement, Steel, Bitumen, Emulsion, Admixture etc
are to be conducted Frequency Wise as per IS/IRC and record to be kept in order.

5. Third Party Test of Sub-Grade material and CBR to be conducted @ 10km interval
and record to be kept in order.
6. The Concessionaire has been instructed to follow the RFI system properly in time
regularly. It should be provided at site on daily for inspection without any delay in
future.
C. PROGRESS OF WORK (CONCLUSION):-
1. Concessionaire has advised to start the activities at all available front where land is
available.
2. Panagarh bypass : Concessionaire and Contractor has assured that Panagarh bypass
shall be completed within July 2016.
Panagarh town existing portion of road is in good condition. It should be maintain on
regular basis.
3. Concessionaire has been advised to expedite the activities of work, so that the progress as
per asking rate in order to maintain for the completion of the project as per schedule
time.
4. Longitudinal joints of BC layer at some location in MCW have been still observed. The
same should be strictly avoided in future and proper care is required during lying.
Submit the required finish surface report as per clause 902 and 903 of MORTH
specification of the above locations.
5. At some location horizontal cracks have been observed in new and old concrete in raised
Kerb. This is due to improper construction joints. Concessionaire has been instructed to
rectify the above defects and please ensure that the joints should be done properly and
line & level should be also maintained as per drawing and specification.
6. In adequate and improper curing arrangements has been still observed in City Sub
base/base course layer. Concessionaire has been instructed to layer should be immediate
covered after compaction completed with hessian cloths/ gunny bags properly to avoid
evaporation of water from mix and maintain adequate curing up to 7 days.
7. In adequate and improper curing arrangements has been observed in Kerb.
Concessionaire has been instructed to Kerb should be immediate covered with curing
compound after laying of kerb and after next day covered with hessian cloths/ gunny
bags properly and maintain adequate curing up to 14 days.
8. Shoulder top layer has been started without approval of material. Shoulder top layer
should be 150mm thick of granular material confirming to the requirements given in
Clause 401 of MORTH Specifications. Concessionaire has been instructed to work should
be carried out after approval/review of granular material for shoulder top layer.
9. Curing of Bridges/Structures, RE panels, Drains, pre-cast girders needs to improve
specially on major bridge/structure location like ROB : 443+710, G.S. : 398+820, 456+315,
MJB : 464+665 casting yard of P.S.C./R.C.C. girders at ROB : 443+710. The mode of
curing at various locations has to be improved with deploying additional water tanker/
Boring with pumps, curing man etc. to ensure that constructed Bridge/structural
members may be wet for a minimum period of 14 days or as per the specification round
the clock.

10. The construction of super structures at various location like at Muchipara and G.S.
506+205, 501+985, 496+595, 476+045, 475+665, 464+185, 456+315, 398+820, ROB :
520+383, 443+710 & MNB at different locations. At all these above locations, it is
required to fence with heavy duty safety net around the construction area (Bottom and
sides of Sub-Structure/Super-Structure) from safety aspects.
11. The lunching of Girder at various location like G.S. 501+985, 456+315 and ROB :
443+710 are going on without verification/submission of the credential / expertise of the
engaged crane services. So, it is required to submit all tested certificates of fitness of
crane and its accessories, as well as the experience certificates of similar work done by
the crane provider/services for verification.
12. The Casting of P.S.C/ R.C.C Girders at various locations in section P2 are going on
without hard foundations, it may cause damage of Girders during coming raining season
due to settlement of soil. So you are advised to take immediate remedial measures to
avoid such type of incident.
13. The steel reinforcement of pedestal and Dirt wall has been found to be cut by thieves. So
specific methodology to insert/grout the steel reinforcement in concrete has to be
submitted before executing the works.
SAFETY & MAINTENANCE OF EXISTING ROAD:-
1. The overall safety measures during construction at site were inspected/reviewed and still
required adequate arrangement for safety.
2. Safety measures at all bridges, ROB etc. to be taken seriously during construction period,
so that no untoward happening should occur in future.
3. All Deep excavation portions should be barricaded.
4. Concessionaire was also advised to ensure the safety of workers by providing PPE
(personal protective equipments) to the workers/ labour during execution.
5. Advance marking sign regarding portion of any of the highways damaged/closed or
diversion or any other information which needs to be advised to the road users should be
prepared in advance and kept ready to enable immediate necessary action in the matter
to be able to ensure hassle free flow of traffic on existing highway.
6. Unauthorized median opening should be immediate closed and median opening signage
should be provided at authorized median opening locations. No any action has been taken
by concessionaire.
7. Signage, Retro Reflective Tapes, Warning Sign should be provided at construction zone
portions and also arrangement of traffic during construction and maintenance shall
confirm to the requirements of IRC SP: 55, clause 112 of MORT&H Specification and
provisions of concession agreement.
8. Trimming of over growth of plants and shrubs in median portion activity had been
stopped. Advised to start and complete the trimming activity as early as possible.
Trimming should be done all existing good plant. Watering should be done regularly
9. Cleaning of carriageway, median, bridge footpath & shoulders to be completed
immediately. Median cleaning is not satisfactory.
10. Maintenance of existing shoulder of project highway has to be taken up immediately.

11. Construction of new shoulder of project highway (where 3 lane DBM/BC completed) to be
taken up immediately.
12. RCC Railing and Kerb in structure area shall be painted immediately as per IRC
specifications.
13. All junction location existing/new Kerb should be painted immediately as per Technical
specification.
D. HINDRANCES AND OBSTRUCTIONS:-
1. Acquisition of Balance Land :- Balance land is to be expedited by NHAI. In
Jharkhand it is going on.
2. Shifting of Utilities i.e. Water line/PHE, Electric poles and towers (partly), Gas pipe
lines, Petroleum pipe lines. Presently Electrical utility work, water pipe line and oil
pipeline are in Progress but needs to be expedited for Completion of Project Highway
on schedule date. There are hindrances persist at many places for want of this
concessioner is facing problem and progress work is delayed.
3. Relocation of Religious Places.:- Shifting of religious structures to be expedited by
NHAI, especially DVC more temple and Punjabi more temple.
