Description of the inspections carried out: Introduction (Attendees, BV scope of work)

M/s Indian Chain Pvt. Ltd. has signed a contract for Third Party Inspection

Service with Bureau Veritas (India) Private Limited, for Inspection of items as

mentioned above. The Inspection Visits was fixed on 17th and 18th June

2013, and the attendees were as under:

1. Mr. Koushik Saha– M/s Bureau Veritas (India) Private Limited, Kolkata.

2. Mr. V.K. Juneja – M/s Indian Chain Pvt. Ltd.

Applicable Documents & Status of Approval (Type of documents reviewed & endorsed)1. Raw Material T.C Reviewed and found in line with QAP Requirements.

2. QAP Signed

Manufacturing Progress Status (Quantities) – Complete as per inspection call.

Details of inspection activities carried out with respect to scope of work (Visual

examination, witnessing NDT, dimensional checks, material identification…)

1. Material offered in Unpainted condition.

2. Material visually checked and found no harmful surface discontinuities.

3. Material dimensionally checked on random basis and found conforming to

P.O Requirements.

4. Proof Load Test carried out on 100% offered material at 41 MT and found

no permanent deformation.

5. Breaking Load Test carried out on 10% of Offered Qty (1 Nos.) as per QAP

at 102 MT on chain consist of 5 Links, and found no failure.

6. Internal Inspection Report reviewed and found in line with P.O and QAP

requirement.

8. Calibration Certificate reviewed and found in line with QAP requirement.
