VITAL STATISTICS FOR 1983

The statistics For 1983 on Forms SD25, SD52 and SD52A will be distributed tohealth and local authorities over the next Few months. There are changes Fromprevious years in the content, format and presentation of the tables. Themajor changes were outlined in a consuItation document, OPCS Circular (Stat)No U/1983, sent out last May. This article explains and illustrates thechanges in more detail. The specimen tables are reduced in size from thestandard computer print which will be sent to customers.

Pre-printed stationery will no longer be used; all parts of the tables,including headings and text, will be produced by computer. This makes forgreater Flexibility if alternative forms of output, for example microfiche,are needed in future. Good quality single-part stationery will be used,producing an improved print. The paper will be narrower, moving to the 132-character width wnich is the standard For computer printing.

District Medical Officers, as -Jell as District Administrators, will be sentstatistics for their areas.

SD25 - mor ta l i ty s tat i s t ics

There are changes in presentation. A line is printed for each of the 95 causesof death even when there are no cases. The consistent layout will enableeasier comparisons to be made between DHAs and between years. The first of theFour pages is illustrated below:

SD52 - vital statistics

There are substantial changes in content and formatDeaths of infants. The breakdown showing legitimacy is dropped. The formerprocedure for assessing the legitimacy at death of infants was not auffi-clently accurate. Better quality information ~111 be available from the OPCSInfant mortality file which ~111 be ready at the end of the year, and thefigures ~11 be publxhed In the DH3 series.

Maternltles. Numbers and percentages of maternities by place of confinementare included in addition to numbers of birthsLlvebirth rates. The standardised fertlllty ratio replaces the area compara-bility factor, the local/regional adjusted rate and the ratlo of the local/regional adjusted rate to the national rate- The standardised fertility ratloshows how the local/regional rate compares with the national rate, making dueallowance for the age structure of the local/regional population The generalfertility rate, the total period fertility rate and the proportion of livebornInfants whose blrthweight is below 2500 grams are introducedDeath rates. The standardised mortality ratio replaces the equivalent rate3to those listed above for live births

The new SD52 consists of three partspopulation, births and deaths; the middleand maternities by place of confinement,and death rates with their definitions

Statistics on tape

A tape containing the SD25, SD52 and SD52A statistics will be supplied onrequest to each RHA. It will have figures for England and Wales, standardregions, and each local and health authority The tape may be copied by theRHA to other health authorities within the region and can be supplied by OPCSto other organisations on repayment Information concerning the tapespecification and cost can be obtained by writing to the Editor

Timetable

The target dates for despatching the paper copies are. SD25 - end April, SD52- end May, SD52A - end May The tape for 1983 should be available at the endof July 1984; in later years it should be available in June

POPULATION ESTIMATES AND PROJECTlONS

Final mid-19@1 and a retrospective, consistent series of mid-1971 to mid-1980population estimates for DHAs (and local government areas) in England andWales have recently been published in OPCS Monitor PPl 84/2. These particularestimates were 'all ages, both sexes’ aggregates, but figures by five-year agegroups and sex are available The boundaries of the DHAs are those as at April1982 Revised intercensal population estimates for the old health authori-ties will not be produced, but conversion lists, indicating how figures forthe old areas could be derived from those for the new health and local auth-ority areas (and vice versa), are available

The mid-1982 set of population estimates are now being finalised and, alongwith the final estimates for mid-1983, will be published in a further Monitorin May or June The procedure of having ‘provisional’ estimates followed bya ‘revised’ or ‘final’ set will no longer apply as all the Census informationrequired has now been incorporated

Population project ions analysed by sex and 5 year age groups have beenprepared for each DHA for 1981-2001 (inclusive). The 1981 populations usedare the final DHA estimates mentioned above These projections were derivedfrom those for metropolitan districts and shire counties by taking account ofthe age structure of each DHA’s 1981 population and the probable future trendof its population (in aggregate)’ They are thud slightly more approximatethan the local authority area population proJections because the migrationcomponent is more important and yet less information IS available about it

These population estimates and projections, and figures on internal popula-tion movement as recorded at the National Health Service Central Register,are available from OPCS on computer tace as well as on paper

TRANSFERABLE DEATHS

Following consultations with members of the health service, OPCS has resumedthe notification of transferable deaths starting with January 1984 registra-tions.

The procedures are similar to those in operation up to the end of 1980, exceptthat the area unit is now the district health authority. Inward transfers,that is deaths of residents of the area which are registered in other areas, arenotified in the form of photocopies of the registration particulars. Outwardtransfers, that is deaths of residents of other areas which are registeredwithin the area, are notified as a list of registration numbers. Notificationsarising from a month's registrations are normally sent out about seven weeksafter the end of the month.

The identification of transferable deaths is carried out continuously duringthe monthly clerical processing of the death entries. The Area Coding Sectionof Vital Statistics Branch is responsible for postcodixg the usual residence ofthe deceased and for coding the type of institution in which death occurred. Asan additional task the coders now provide a code for area of occurrence, asdescribed below.

If the death occur?d at home or is otherwise 'non-transferable' because of thetype of institution or length of stay (see 'Area coding of deaths' inNewsletter No 1) then no further action is required. If the death wasregistered in a registration sub-district which is wholly within a DHA, oroccurred in an institution which is on the coders' lists, then the DHA coderelating to that sub-district or institution is inserted on the death entry.Otherwise the address where the death occurred is postcoded.

As a second clerical operation, and ignoring the 'non-transferables', thepostcodes of usual residence are translated into DHA codes using speciallyprepared lists. The few postcodes relating to address of occu==ence aretranslated at the same time. The two DHA codes, occurrence and residence, arecompared; if they differ the death entry is photocopied and the entry number islisted under the DHA of occurrence.

The photocopies are sorted to DHA of usual residence and represent the 'inward'transfers. They are despatched, along with the lists of entry numbers repre-senting the 'outward' transfers, within a few days of completing the month'sclerical processing.

Copieshave been distributed to all health authorities and the hospitals and "nitsconcerned. The report contains detailed tables on admissions, discharges,deaths and resident patients and some information on trends from 1970.Statistics on the legal status of in-patients and for the Special Hospitalsare included in separate sections of the report.
