Overview of Indiana Community Health Centers The Indiana State Department of Health (ISDH) and Office of Primary Care (OPC), administers the Community Health Center (CHC) Operating Grant. Funds are received from part of the Master Tobacco Settlement as authorized by the Indiana Senate Enrolled Act 108 – 111th General Assembly March 2000. The ISDH-OPC promotes the development and operation of community-based primary health care services in areas of need that improve the health status of medically underserved populations of Indiana. A community health center is defined as consumer driven, providing primary health care services by state licensed professionals, which are also comprehensive in scope, coordinated within the community, acceptable, accessible, affordable, appropriate, and available. Specific expectations of all community health centers seeking to apply are as follows:
• Consumer driven: Must have at least 30% consumer Board, have Board meet at least once a quarter, and have the minimum of a nine (9) member Board. A consumer is defined as a patient of record who has been seen in the last three (3) years and considers the community health center to be his/her medical home for primary care and preventive health services
• Primary Health Care Services: Primary medical care must be provided on-site with a
provider delivering primary care services at least 20 hours a week. Specialty care, mental health care, and dental care must be provided on-site or by a formalized written community referral arrangement noted by Memorandum of Agreements. Primary care services must be provided without regard to ability to pay and must demonstrate an established sliding fee schedule.
• State Licensed Professional: Health professionals must be licensed with the appropriate
Indiana state agency and compliant with all applicable laws.
• Corporate Compliance Program: Written policy on site, with documentation of monitoring the community health center’s adherence to all relevant laws, regulations and procedures, including HIPAA.
• Comprehensive in Scope: Depending on the facility, primary care must be available for
all life cycles (infants, children, adolescence, and adults), and include preventive care services. Access to pharmaceuticals, x-rays, labs, and hospitalization must be coordinated and provided through direct service provision or formalized written referral, noted by Memorandum of Agreement, to support the continuity of care for all patients.
• Comprehensive care must be:
o Acceptable – to all those residing in the community regardless of cultural
heritage, financial status or personal circumstances. o Accessible – where it’s needed o Affordable – based on income o Appropriate – continuous, comprehensive, prevention-oriented primary care o Available – when it’s needed

Overview of Mapping The Indiana State Department of Health maintains a geographic information system to support the geospatial needs of all programs within the agency with applications or projects which utilize demographic, infrastructure, and epidemiological data to enhance the integration, illustration, and analysis of morbidity and mortality trends and surveillance/preparedness activities. Community Health Center maps are based upon address information provided to the ISDH Office of Primary Care by CHC grantees. Since the intent of the maps is to depict location of services, an effort was made to distinguish between addresses of administrative offices and those addresses which do offer CHC services to the public. Ultimately, it is the responsibility of the grantee to inform ISDH of changes in provider locations. The maps included in this document depict only those locations known to offer services as of December, 2007. Clinic addresses may be geocoded (assigned a geographic coordinate) at a street address or estimated to the center of a zipcode, zip+2 or zip+4. The maps do not distinguish between the types of geocoding. If this distinction is needed, please contact ISDH GIS. Additionally, clinic locations may be adjusted for display purposes. For those counties with 3 or more clinics, local maps are included to help determine more precise clinic locations. Each clinic on the map is depicted by an ID within an orange triangle. This ID corresponds to the ID in the clinic list provided in this document. The list contains clinic attributes such as name, address, and phone number. To assist in locating a clinic from the list on the state map, a reference key and grid are provided. For example, a clinic identified in the list by ID 99 and Grid ID A1 can be found on the map by locating the intersection of the map area A (along the top and bottom) and 1 (along the sides).
