Which Dental Plan is Right for You? Americans Spend Over $100 Billion Annually on Dental Care.

Regular dental check-ups are a necessary part of maintaining your overall health. Dental issues can affect areas other than your mouth. Research has shown a direct connection between oral health and other health issues such as heart attacks and strokes. Routine dental exams can provide early detection of serious problems that you may be unable to see or feel.

Despite the significant health benefits of routine dental care, almost 40% of seniors fail to visit a dentist at least once during the year. The #1 reason is cost. Americans spend over $100 billion on dental care each year. Dental insurance helps with the costs associated with making sure you stay healthy.

You have two distinct types of dental plans to choose from: Preferred Provider Organization (PPO) and Health Maintenance Organization (HMO). Both plan types are comprehensive and cover procedures ranging from the routine cleaning, up to crowns, dentures, and implants. As with all dental insurance, work in progress and replacing missing teeth are not covered by these plans. The main difference is in how they work:

PPO Plans You may visit any dentist. The dentist does not need to belong to the insurance company network for you to receive benefits. However, if the dentist is part of the network, you may receive significant additional cost savings.

Network Dentists have contracted with the insurance company and have agreed to provide members significant discounts off covered procedures, typically 25% - 40% off their regular rates.

Non-Network Dentists have no affiliation with the insurance company. Your insurance will still cover your treatment, but the dentist will not provide you with any discounts. Therefore, your out-of-pocket costs are likely to be significantly higher than if you used a Network Dentist.

You are not locked into any particular dentist. You may use any dentist anywhere in the United States and you do not have to inform the insurance company of your dentist choice.

HMO Plans HMO plans have a network of highly qualified dentists that are pre-screened and thoroughly evaluated prior to their acceptance in the network. Your must use one of these dentists. You cannot use a dentist unless they belong to the network.

With HMO plans, there are no cost surprises. Every covered procedure has a set co-pay, so you know exactly how much everything will cost before having work done.

There are no set annual benefit maximums. Whether you only require a routine cleaning or you need multiple crowns and an implant, you will never run up against a calendar maximum dollar benefit like PPO plans have.

All services are available from your first day of coverage. There is no waiting period for any covered service. Whether it’s just a cleaning or an implant, all covered services are available beginning your first day covered by the plan.

You may switch dentists as often as you like, just by calling the insurance company. 3

Dental PPO - Ameritas Freedom to Use Any Dentist - No Network Restrictions

This Ameritas PPO dental plan is a comprehensive, full service plan that covers over 360 procedures, from the routine preventative care to the major including crowns, dentures, and implants. Whether you need routine care or something more extensive, this plan will have you covered.

Every covered member and dependent each has a robust $2,500 in-network calendar year benefit. Coverage is available for the Association member individually, or they may also insure their spouse/domestic partner, and their dependent children up to age 26. Coverage is available to children age 26 and older if they are permanently disabled and the member lists them as a dependent on their tax return. Grandchildren are only eligible if you have full legal custody, not temporary guardianship.

This plan offers a unique Dental Rewards® benefit which provides members additional “banked funds” to use for covered procedures. Any member or dependent who use less than $750 of their annual benefit will automatically have additional funds deposited toward their calendar year maximum in future years.

As an added benefit, members who visit the dentist at least once during the year will have their Basic Services Network benefit level increased by 5% the following year - up to 90% maximum.

Ameritas has the largest dental network nationwide. There are over 325,000 providers to help ensure that you have access to quality dental care at deeply discounted prices. Ameritas has a well earned reputation of having some of the best Network Provider discounts in the industry. Additionally, many Ameritas providers have agreed to extend deep discounts on non-covered procedures, such as cosmetic procedures like teeth whitening.

Search for Network providers in your area at: www.AmeritasGroup.com

This chart illustrates the significant estimated savings you could realize by using an In-Network dentist.

No wording here changes the actual benefits defined in the Patient Charge Schedule or Certificate of Insurance. Consult with your dentist to confirm which ADA code your procedure falls under.

It is highly recommended that you request a Predetermination of Benefits prior to starting any work. This is a notification to the patient and the dental office as to whether the planned procedures are covered by the insurance, what the member co-pays will be, and whether the planned procedures fall within the benefit maximums and procedure limitations.

Certain exclusions and limitations may apply to services. Certain dental work may require multiple procedures with a co-pay for each procedure, or additional cost for precious metals, or exclude the cost of final restoration. Please consult with your dentist prior to beginning any work for clarification of costs.

Vision with Exam - VSP Annual eye examinations are vital. Not only can they discover early signs of potentially blinding eye conditions, such as glaucoma, diabetic eye disease, or macular degeneration, but they can also detect signs of serious health conditions, like hypertension, cardiovascular disease, diabetes and high cholesterol, that can easily go unnoticed by you. Unlike most of your body, your eyes do not usually hurt when there is something wrong.

This PPO plan is designed for members who have medical coverage other than Kaiser or Health Net as these medical plans include coverage for an examination.

The plan allows you to see any provider nationwide, but if you use one of the 37,000 VSP Choice network providers, you will receive the highest benefits and lowest costs. VSP Retail Affiliate Providers include Costco and Visionworks and VSP has direct pay agreements with Walmart Vision and Sam’s Club Optical.

Many VSP Choice network providers offer additional discounts on various eyewear enhancements such as anti-reflective coating, progressive lenses, polycarbonate lenses, photochromic lenses, scratch-resistant coating, Laser (LASIK / PKR) surgery, retinal screenings, additional pairs of glasses, and sunglasses.

Find local VSP Choice Network providers at: www.VSP.com

Coverage is available for the Association member individually or you may also add coverage for your spouse or domestic partner, and your dependent children up to age 26. Coverage is also available to children age 26 and older if they are permanently disabled and the member lists them as a dependent on their tax return. Grandchildren are only eligible if you have full legal custody, not temporary guardianship.

Legal Shield From Traffic Tickets to Wills, Real Estate or Divorce: Professional Legal Advice for Less Than 50¢ a Day

Legal Shield was founded over 40 years ago, with the belief that everyone deserves access to quality, affordable legal protection. Businesses and the wealthy can afford to pay attorneys $400 an hour to protect their interests, but can you? With Legal Shield, you gain access to attorneys to level the playing field and give you the protection you need, all for less than 50¢ a day.

Legal Shield’s nationwide network of affiliate lawyers, with an average of 19 years of experience, is there to help you with any legal issues. Whether it’s personal, business related, or criminal, you will have quality legal representation and know all of your rights.

When you join Legal Shield, you’ll experience the safety and security that over 1.5 million members enjoy. Access to convenient quality no-cost legal help is only a toll-free phone call away.

Benefits of Legal Shield membership include:

Phone Calls and Letters We’ve all experienced situations where a person or company has taken advantage of us or refused to do as promised, and our personal calls and letters do not resolve the situation. Perhaps it was a store that didn't honor a return, or a poorly done repair job, or a merchant who didn’t honor their low-price guarantee? Unfortunately, many businesses know they can get away with these things, because few people will spend the time or hire a lawyer to being them to justice.

With Legal Shield, your attorney will write a letter or make a call on your behalf at no cost to you. When the other party sees that you have legal representation, they know you are serious and will work to get the situation resolved.

Will Preparation with Annual Reviews Having an updated Will is part of being a responsible adult. However, 55% of Americans don’t have one. The numbers are even more shocking for minorities: 68% of African Americans and 74% of Hispanics do not have a Will. Considering the benefits of having a Will, these are unfortunate statistics. Wills can:

• Protect your assets from probate and intestacy laws and greatly reduce time spent in costly probate court. • Provide peace of mind, knowing that your assets are protected and your loved ones will be cared for. • Control the gifting of assets to the specific person(s) you choose. • Ensure your committed relationship partner is provided for.

Legal Shield members receive a Will and annual updates at no cost, and spouses can have a Will prepared for just $20. For members requiring a higher level of estate planning, Trust preparation is available with a 25% discount.

Contract and Document Review While we all say that we would never sign a document without reading it thoroughly, the reality is that most only skim. Chances are, we wouldn’t make sense of the legal jargon fine print anyway.

With Legal Shield, at no cost to you, you can have an unlimited number of personal legal documents reviewed and explained to you in “plain English” by your attorney. Your attorney will also suggest any changes he or she deem necessary and if the other party has acted improperly, the attorney can contact them on your behalf to resolve the issue.

27

Page 29: INDEX []Routine dental exams can provide early detection of serious problems that you may be unable to see or feel. Despite the significant health benefits of routine dental care,

Whether you’re signing a new cell phone contract, booking a hotel room, or wanting to ensure that you get your full security deposit back from your landlord, legal document review can save you thousands of dollars and countless headaches.

IRS Audit Protection An IRS audit can be terrifying, even if we believe we did everything 100% honestly. Unfortunately, around 1.5 million people experience an IRS audit every year. For those who think only the wealthy get audited, over 80% of audits are done to households making less than $50,000 per year.

With Legal Shield, when you are notified of an audit by the IRS, you have immediate access to consultation, advice, and assistance at no cost to you. You can receive, at no additional cost, up to 50 total hours of your attorney’s time to help defend the audit.

Motor Vehicle Services Vehicle ownership creates the potential for many different problems. When there’s a car accident, our insurance company deals with the issue, but who takes care of more minor issues? What if your car is vandalized in a parking lot? What if items are stolen from your car after being valet parked or washed? What if another driver nicks your car while it’s parked on the street? - and the other driver is claims it’s your fault based on how you parked? - and the police issue YOU a citation! Problems happen and whether you were the at fault party or not, having an attorney on your side can be crucial.

If your driver’s license is revoked or suspended, your Legal Shield attorney will help you. They will put you in a position of knowledge regarding the law. If the unthinkable happens and you are involved in a fatal car accident and charged with involuntary manslaughter, negligent homicide, or vehicular homicide, your attorney will defend you, no matter how long it takes.

Trial Defense The United States is a litigious country. We have more lawyers per capita than any other nation and a new civil lawsuit is filed once every two seconds. Whether the suit has merit or is frivolous, lawsuits can cause tremendous emotional and financial stress.

With Legal Shield, if you or your spouse is named as a defendant in a covered civil or criminal action, your attorney will provide up to 60 hours of defense at no additional cost to you.

Legal Shield Monthly Premium

Member & Family: $14.95

Add 24-hour access to an attorney if you’re ever arrested or detained, injured in a motor vehicle accident, confronted with the state taking your children, or served with a warrant, for only $1 more.

For one low price, Legal Shield covers the member, their spouse, and the following dependent children:

• Under age 18 for whom the member or member's spouse is the legal guardian. • Under age 21, never married and living at home. • Under age 23, never married and a full time college student. • Under age 26, never married and living at the member’s home. • Any dependent child, regardless of age, who is incapable of sustaining employment because of mental or

physical disability and who is chiefly dependent on the member or member's spouse for support.

Note: Benefits listed are for California. Benefits outside California may vary slightly. 28

Term Life Insurance High Benefit Amounts - Low Costs

Term life insurance is designed for those seeking a high benefit amount, covering a set period of time (term). Term has the lowest cost, but rates are determined based on your health. Term pays a benefit to your beneficiaries upon your death, but it has no cash value.

Since coverage is based on your health (medically underwritten), a ParaMed visit is required. This is a fast and free height and weight check, blood and urine test, and a few family history questions, done at your home, usually lasting only a few minutes. Also, at no cost to you, the insurance company will obtain copies of your medical records from your doctor to confirm your answers on the enrollment form.

Minimum benefit amount is $100,000. Approximate monthly rates for a typical, healthy non-smoker are:

Note: People treated for cancer or depression within the last two years will not qualify for coverage. Amplifon Hearing Benefit It’s time to increase the quality of your life!

Hearing loss is a natural part of aging. Left untreated, however, hearing loss reduces the overall quality of life; sufferers have higher rates of depression and social isolation. It can also cause Auditory Deprivation, a decrease in speech understanding resulting from a hearing loss.

Clinical studies show that wearing a properly fitted hearing aid significantly improves overall quality of life. Yet, denial and embarrassment are cited as the main reasons for not getting a hearing aid. Even though many hearing aids are virtually invisible and deliver excellent performance, a Johns Hopkins study revealed that only a small percentage of people with hearing loss used a hearing aid.

Amplifon Hearing Health Care has partnered with RESDC to offer a FREE hearing aid discount program. Benefits include:

Around 25% off hearing aids, including name brands like Phonak, ReSound, Starkey, and

Siemens.

Around 40% off testing and diagnostics.

Low price guarantee: Will beat local hearing aid quotes by 5%!

Free one-year supply of batteries - approximate a $75 value.

Free one-year follow-up care.

Risk-free, 60-day, 100% money-back satisfaction guarantee.

Three-year warranty - one of the longest in the industry.

Financing available.

While infrequent, providers may, without warning, discontinue their affiliation with an insurance company. There is no

Some plans have a minimum commitment. Should you cancel coverage by any action, including stopping payment,

before the commitment is up, PGA, at its sole discretion, reserves the right to retroactively cancel your insurance to the

Cancelations require significant lead time to process and must be received by Noon on the 5th

collection of those monies.

It is the responsibility of the member to:

Report to PGA any change that affects insurability or the passing or ineligibility of covered dependents. Due to

Confirm they are enrolled in the correct and suitable plan.
