Criteria for Diagnosis of PCOS

PCOS definition NIH criteria 1990

Patient demonstrates both:

1.Clinical and/or

biochemical signs of hyperandrogenism

2. Oligo- or chronic anovulation

Rotterdam criteria 2003 (ESHRE/ASRM)

Two of the following three manifestations:

1.Irregular or absent ovulation

2.Hyperandrogenism (clinical or biochemical)

3. polycystic ovarian morphology on USG

AES Criteria 2006

Patient demonstrates both:

1.Hirsutism and/or

Hyperandrogenemia(clinical or biochemical)

2. Oligo-anovulation and/or polycystic

ovarian morphology

Azziz et al. JCEM 2006; 91: 4237-45

Exclude other etiologies of androgen excess

Clinical Criteria

1) Hirsutism

2) Acne

3) Male pattern balding

Peripubertal onset

Obesity contributes

• Oligo or amenorrhea – peripubertal / delayed menarche

-Obesity contributes

• Degree of cycle irregularity: simple clinical parameter

intermenstrual intervals >35 days chronic anovulation

Low Serum progesterone D21: to assess for ovulation

USG: In patients seeking fertility-to monitor and document ovulation

• Infertility and 3 fold risk for endometrial hyperplasia and/or CA

I. Androgen Excess

II. Ovulatory Dysfunction

III. Polycystic ovaries

Increased ovarian volume (>10 ml) w/ presence of

>25 peripheral follicles measuring

2-9 mm in diameter

Polycystic ovaries not specific for PCOS

Absence of other disorders to account for these symptoms

• Pregnancy serum hCG• Ovarian failure FSH• Hypothyroidism TSH• Hyperprolactinemia prolactin• CAH 17-OHP

• Ovarian tumor total testosterone (esp if >200 ng/dl)• Adrenal tumor DHEA-S (esp if > 800 mcg/dl)• Cushing’s serum cortisol

PCOS ---- Diagnosis of Exclusion

Anovulation:

*85 percent of cycles anovulatoryin first year of menstruation.

*59 percent in the third year

*25 percent still anovulatory in the sixth year

Metabolic features

Insulin resistance

insulin due high GH

hyperpulsatile GnRHsecretion

decreased levels SHBG ovarian & adrenal androgen

PCOM at USG

40% at 2 yrs

35% at 3 yrs

33.3% at 4 yrs

after menarche

Corresponds to physiologic condition during early adolescence

Not associated with abnormalities in

ovulation

menstrual cycle duration

androgens or IR

However all return to normal at the end of normal puberty but remain elevated in PCOS

Why Adult Criteria not applicable to Young PCOS?

Normal adolescent

Take home messages - PCOS Screening

Establish accurate diagnosis of PCOS

/identify the phenotype

ACOG/ AEPCOS recommends 75gm 2hr OGTT

Lipid profile (cholesterol, triglycerides, HDL/LDL)

Identify metabolic syndrome, use TG/HDL >3.2 to target subjects

No test of insulin resistance is needed to make diagnosis of PCOS or to select treatment

Page 18: in · PDF filein PCOS Dr. Piya Ballani Thakkar ... 1.Clinical and/or biochemical signs of ... LH/FSH >2: need not be documented I. Androgen Excess

Biological hyperandrogenism in PCOS is inconstant and has no specific profile

Serum AMH as a surrogate to follicle count seems promising, but issues about assays must be solved

Follicle excess is the best criteria for PCOS but threshold is highly dependent on technology used With newer technologies, it is 25...

There will never be a single criteria to define PCOS

Key Messages-------------Look Beyond

Most common endocrine disorder of women of reproductive age

Multiple phenotypes:

androgen excess, ovulatory dysfunction, polycystic ovaries

Multiple criteria …Making a diagnosis can be challenging!

Multidisciplinary: patients come in through different doors- obstetrician, dermatologist, internist , endocrinologist

Monitoring for cardiovascular and diabetic complications, and reproductive cancers - a key part of management

More risk of metabolic and cardiovascular complications in hyperandrogenic forms than non-hyperandrogenic forms\

of PCOS
