1.4 Using the System
Intended Use/Indications for Use
Brainlab Elements Image Fusion Angio is a software application that is intended to be used forthe co-registration of cerebrovascular image data.The application can be used in clinical workflows that benefit from the co-registration of vascularimage data as a planning or preplanning step for the treatment of cerebrovascular malformations.
Intended Patient Population
There are no demographic, regional or cultural limitations for patients. It is up to the user to decideif the system shall be used to assist a certain procedure.
Intended User Profile
The application is intended to be used by medical professionals and their assistants working in thefield of neurosurgery, neuroradiology or radiotherapy planning.
Intended Use Environment
The system shall be used in a hospital office environment or rooms appropriate for surgicalinterventions.The system can be installed on a standard Windows computer with the defined minimumrequirements.A more specific intended use environment (e.g., a specific planning station or navigation platform),may be present when the device is used in combination with other devices. The intended useenvironment is then stated in the technical file of the corresponding system.

Updates

Virus Scanning and Malware
Brainlab recommends protecting the system with state-of-the-art anti-virus software.Be aware that some malware protection software (e.g., virus scanner) settings can negativelyaffect system performance. For example, if real-time scans are performed and each file access ismonitored, then loading and saving patient data may be slow. Brainlab recommends disablingreal-time scans and performing virus scans during non-clinical hours.
WarningEnsure that your anti-virus software does not modify any Brainlab directories, specifically:• C:\Brainlab, D:\Brainlab, F:\Brainlab, etc.• C:\PatientData, D:\PatientData, F:\PatientData, etc.

Brainlab Training
Before using the system, all users must participate in a mandatory training program held by aBrainlab authorized representative to ensure safe and appropriate use.
Supervised Support
Before using the system for surgical procedures where computer-aided navigation is consideredcritical, perform a sufficient number of complete procedures together with a Brainlabrepresentative.

Background
Image Fusion Angio is software that allows you to register digital subtraction angiographies tovascular images in order to combine flow and location information. You can fuse 2D DSA (digitalsubtraction angiography) sequences to MRA, CTA and 3D DSA sequences.Image Fusion Angio proposes a fusion network based on predefined pairing rules. For furtherinformation contact Brainlab.
Working Principle
Image Fusion Angio provides an algorithm to fuse the selected image sets. The algorithmmatches two image sets together with common anatomical structures for optimal fusion results.Manual adjustments or complete manual fusions are supported in addition. Fusion results canalways be reviewed, accepted or discarded.NOTE: Scan protocol recommendations are given in the following chapters or can be found in theBrainlab Scan Protocols.
Scan Recommendations for Best Results
To ensure best results, image data for Image Fusion Angio should follow these scanrecommendations: • Full DICOM information (e.g., complete DICOM header and indicating acquisition parameters)• Good raw image quality (e.g., high resolution, high contrast, minimal artifacts)
3D Scan Recommendations
The following sequence types are recommended for Image Fusion Angio:• 3D DSA scans• Contrast-enhanced vascular MR scanning protocols such as TOF or T1 with contrast. Fat
suppression imaging is recommended.• Contrast-enhanced vascular CT scanning protocols• A minimum of 50 slices• Maximum pixel size: 0.7 mm x 0.7 mm• Maximum slice distance: 1.5 mm
2D Scan Recommendations
The following is recommended for 2D DSA scans:• Follow the instructions in the DSA scanning protocol• At least six frames per sequence• Scanning type: STATIC

Reviewing Image Sets
WarningAlways review the fusion result between the 2D angiographic projections and the vesselsin the 3D scan before proceeding.
Image Modification
Modifying image sets outside of Image Fusion Angio when already in use can lead to corruptionsor inaccuracies.NOTE: The fusion is based on common anatomical structures and is affected by brightnessvariations within the image slices.
Typical Workflow
Step
1. Select patient.
2. Select your workflow (e.g., Cranial > Angio Planning).
3. Select patient data from Data Selection.
4. Start Image Fusion Angio.a. When using data from a bi-planar scanner, images will be automatically paired.b. When using data from a single plane scanner or processed data, the software will pro-
pose frontal-lateral image pairs to be reviewed and approved.
5. Use Adjust and ROI to roughly align the images.
6. Select Fusion to fuse each image set.The algorithm will fuse the 3D scan to the 2D projections.
7. Manually align image sets as necessary.
8. Accept or discard each fused image set.
9. Select Done.Image Fusion Angio closes.NOTE: Depending on your license, color intensity projection (CIP) images may be createdupon closing.

Fusion Options
Button Description
Fusion: Starts an algorithm to fuse the 2D and 3D image sets. You can cal-culate a fusion at any time.
Undo: Undoes the last action. Select repeatedly to undo multiple actions.
ROI: Opens a drop-down menu of available regions of interest (ROI). Selectone to apply it to the current image set.
Adjust: Activates the rotate, move and resize tools in the view area. Alignimages manually using the tools prior to fusion for a good starting point, orafter fusion for manual correction.
Blending: Select to incrementally fade out the 3D image (e.g., during man-ual adjustment).

3 USING IMAGE FUSIONANGIO
3.1 Accepting Image Pairs
Background
Image Fusion Angio analyzes the patient data you selected and suggests pairs of 2D images.This applies to all data from single plane scanners, or when images were further processed.If bi-planar images are used, the application reads the pairing information from the images andthis part is skipped.Suggested pairs may be shown as:• Fused (e.g., a pair approved in a previous session)• Proposed (e.g., the software assumes these images belong together)• Paired, after approval• Unpaired (e.g., after discarding or when no proposal can be made)
You must manually approve each pair before starting fusion with the 3D image.If an automatically suggested pair is not suitable, you can manually change the pairing.

How to Accept or Decline Automatically Generated Pairs

You must review each pair before continuing with the fusion process. Only paired images can befused.The active pair, shown with an orange border, corresponds to the displayed image set.
Step
1. Review the pairing in orange and confirm that it is acceptable. If it is not acceptable, se-lect X to decline the pair.
2.
When you accept the pair, the status changes to PAIRED.NOTE: If you load image sets that have already been fused, they appear as FUSED.Images from bi-planar acquisitions appear as AUTO-PAIRED. Neither image can bemodified.
3. Continue with the remaining pairs.To switch to a different pair, select your preferred image pair.Only accept the pairs you want. When a minimum of one pair is accepted, you may pro-ceed to the next step.
4. Select Next. The fusion screen opens.

How to Manually Create New Pairs

If the software does not automatically suggest a match between two suitable images, you canmanually create pairs.
Step
1. Select an unpaired Frontal image.An orange border appears around the image.
2. Select a corresponding unpaired Lateral image.An orange border appears around the image, and a dotted white line connects the twoimages.
3. If the pair is satisfactory, select the check mark.The images move up under the previous paired image set and the status changes toPAIRED.
How to Change Default Image Orientation

The software assigns the images to frontal or lateral based on the information provided with thedata. By default, all frontal images are displayed in the left column, and all lateral images aredisplayed in the right column. You can manually change the orientation if an image was assignedincorrectly.
Step
1. Select the arrow button ① for the image that you wish to change.The orientation menu opens ②.
2. Select the appropriate icon (e.g., the lateral representation).The image moves to the other column and displays the correct orientation.
3. Continue with the pairing process.

3.2 Adjusting the ROI
Background
Performing a fusion on a manually defined ROI restricts the fusion algorithm to the ROI area. Itenables higher fusion accuracy in a limited spatial area.The following ROI are available: • All Vessels (default)• Carotid Artery Left• Carotid Artery Right• Carotid Artery Left + Right• Basilar Artery
If not enough information is available in the image set, a more general list of ROI is provided:• All Vessels (default)• Hemisphere Right• Hemisphere Left
NOTE: If the software does not receive enough information, ROI functionality is disabled.
How to Adjust the ROI
Before selecting Fusion, use preselected ROI options to improve alignment accuracy within alimited spatial area.
Step
1.Select ROI.A menu with ROI options opens. The content of the menu depends on theinformation available in the image set.
2.
Select the ROI most useful for your purpose (e.g., usually the artery that is displayed inyour DSA image). The software shows the image set with the selected ROI superimposedin red.NOTE: Ensure that sufficient anatomical information is covered by the region of interest inorder to improve the fusion results.

3.3 Fusing Image Pairs

After you have accepted pairs and the initial position has been calculated, you can initiate fusionbetween the 2D images and the 3D image. The 3D image is displayed as a red maximumintensity projection (MIP) superimposed over the 2D image.The initial screen shows the proposed fusion as calculated by the software.The toggle ② indicates which image pair is displayed in the view area ①.You can perform any of the following options:• Start a fusion• Set an ROI• Manually align the superimposed images
How to Start a Fusion
The Fusion button on the toolbar becomes active any time a fusion input parameter has changed,such as:• The Movie Player Control position• The position of the image after using Adjust• The ROI
When the button is active, you can start a fusion of the 2D image set with the 3D image.
Step
1. Select Fusion.The software calculates a fusion of the 2D image set with the 3D image.NOTE: You must select Fusion to apply the fusion algorithm to the newlydefined ROI.
2. The suggested fusion is displayed. You can now accept the fusion, or discard it and ad-just it manually before performing fusion again.

How to Restart a Fusion
You can restart a fusion by resetting fused images back to the initial position.
Step
You can either select Undo from the toolbar or select X next to Fused on the Fusion Header.NOTE: Using X to break the fusion allows you to return to the initial image set alignment. If anROI was manually defined to perform a fusion, the break action removes it.

Background
If automatic fusion results are unsatisfactory, you can make manual adjustments.Aligning the image set manually establishes a starting point to aid the registration algorithm and toimprove fusion results.

How to Use Blending

Blending allows you to compare the alignment of the 3D image set with the DSA image bygradually changing the opacity of the 3D image. You can then manually improve alignment. Thesoftware uses a red overlay for vessels.

Select Blending.
2. Drag mouse/finger to the left or right inside the image to incrementally adjust the transpar-ency.Alternatively, start the Movie Player Control to view Blending.NOTE: The default for Blending is set at approximately one-third of the slider. For bestfusion results, reduce the contrast on the slider after manually aligning – but before se-lecting – Fusion.

How to Use Windowing
Windowing enables you to adjust brightness and contrast for each of your fusion pairs.

How to Use Movie Player Control

Use Movie Player Control to automatically play your DSA image sequence and compare to theplacement of the MIP image.
Step
1. Select the button ① for Movie Player Control underneath the image you want to view.Blending or Windowing of the image sequence will be played in a loop.

About Finalizing Fusion Results
Before you can close the software, you must review and finalize all pending fusion results. Youcan:• Approve a fusion• Discard a fusion
NOTE: Fusions must be approved in order to use them in other applications.
How to Approve a Fusion Result
Step
1.
Review the fusion result. If the result is satisfactory, accept it on the Fusion Header.
2.
• When you accept the result, the pair is shown as fused.• If you do not accept the result, make adjustments with Adjust and Fusion until results
are satisfactory.
3. The fusion result is saved for further processing. The software loads the next pair.

3.7 Color Intensity Projection (CIP) Images
Background
CIP image sequences are automatically generated for each fused DSA image upon leaving theapplication.NOTE: Availability of this feature is license dependent.
How to Interpret CIP Images
CIP images provide temporal information on blood flow from a number of sequential DSA images.The arrival time of contrast agent at a given location is color coded into a single composite image.The color of each pixel indicates the time that contrast arrived at that location. Structures with highcontrast attenuation (e.g., larger vessels and vascular lesions) appear brighter.

Here is a DSA image at the beginning of the sequence ① and the corresponding CIP image ②.• Red and orange: Areas that already received contrast agent.• Yellow and green: Areas that are currently receiving contrast agent. These are the areas with
the highest concentration of contrast agent in a given frame.• Turquoise and blue: Areas that have not yet received contrast agent.
