… To lose one or several teeth is not a problem any-more …

PRESENTATIONCurrently to lose one or several teeth is not a problem as dental implants rehabilitate all functions that original teeth did, even in those cases with full dentures.Placing an implant is a safe, uncomplicated, reliable procedure that enables the restora-tion of lost teeth either due to cavities, periodontal disease, and/or trauma among others with a simple surgical procedure. Dentures can be also replaced by implants as over time dentures become uncomfortable and unpleasant due to excessive mobility.Implants offer an optimal and functional solution at any age, but always once an indivi-dual’s bone growth is completed (usually after adolescence), and in almost all clinical situations.Elderly are who can most benefit from using of implants because the aesthetic results are more significant. An implant acts and works like a natural tooth enabling patients to talk, eat and smile with complete ease. They are as practical and as natural teeth, and do not cause any discom-fort.Replacing missing teeth with dental implants improves the patient’s appearance, increas- es self-confidence and provides peace of mind.

… To replace the natural roots of miss- ing teeth …

WHAT IS A DENTAL IMPLANT?A dental implant is a grade 4 titanium - material highly compatible with the human body - screw which is placed in the upper or lower maxilla with a minor surgical procedure. The implant replaces the natural root of a missing tooth, so once the implant has been osseointegra-ted it restores lost function and aesthetics. Thus, an anchor is obtained to provide stability, comfort and security to the future dental prosthesis, the visible part of a dentition.

After a complete examination to assess the health of a patient’s teeth, including radiodiagnosis tests to determine the quality of the bone, the dentist must provide the patient with all the necessary information and medical advice, and ascertain whether this is a good candidate for a dental implant treatment.

In case of an affirmative answer, the dentist will prepare a detailed treatment plan that will follow these steps:

TREATMENT STAGES

1st Preliminary examAfter an initial assessment, the dentist determines if the patient can undergo a treatment with implants and, should this be the case, explain what, how, where, when and how many implants will be placed. There are some medical conditions that may prevent, hinder or discourage oral restoration with implants, among which it is worth highlighting serious diseases affecting bone metabolism, malignanacies affecting the bone or high-dose radiotherapy. A high consumption of any kind of tobacco can also be detrimental for implant placement.

2nd SurgeryThe surgeon will insert the implants into the defined area with a relatively minor surgery and use anaesthesia to avoid any possible pain. Following the surgical protocol for implant placement, implants can be inserted in a safe and stable manner. While the insertion time for a single implant does not usually reguire a long time (approx. 15 min.), the specific case and the total number of implants to insert will determine the duration of the surgery. After surgery, the patient must follow the cleaning, hygiene and aftercare instructions provided by the medical team. After the appropriate time has passed, always established by the doctor and patient’s healing rate (usually 7 - 10 days), the sutures are removed.

3rd Rest and healing periodDuring the healing period, implants are firmly integrated into the bone, process knows as “osseoin-tegration”. Thanks to the intimate bone-to-implant contact, the implant will integrate itself into the bone and, with time, it will surround and cover the implant, slowly immobilizing it and providing the desired stability. The amount of time that practitioners allow an implant to heal before placing a restoration on it varies widely. This process lasts up to 6 months depending on the treatment, type and quality of the bone, usually 2 to 3 months for mandible and 4 to 5 months for upper maxilla but always at the discretion of the surgeon.Once the implant is integrated, the implant cover screw is removed with a simple surgery (lasting about 20 min.) and a healing abutment takes its place to shape and model the gums, and heal the tissues. After a healing period that usually does not last longer than 1 or 2 weeks, the surgical treatment is then considered finished.

... During the healing period, implants are integrated into the bone ...

4th ImpressionThe surgeon takes measures to develop a model that will be an exact replica of the patient’s mouth, and sends it to a dental laboratory so that the patient’s new teeth can be designed. While the laboratory produces the prosthesis, follow-up visits with the patient are scheduled to check the suitability of the new perma-nent teeth.

5th ProsthesisThe surgeon and the dental laboratory determine which pros-thesis is better suited to the needs of the patient, i.e., a prosthe-sis that will restore lost function and aesthetics in a manner as similar as possible to the patient’s natural teeth.Currently the introduction of new materials, such as zirconium (highly biocompatible, very hard material), and new technologies, such as CAD-CAM (computer design and manufacture of dental prosthesis), provide new possibilities and alternatives that produce even better aesthetic results.

IMPLANT MAINTENANCE AND HYGIENE

Implant maintenance and hygiene must be done both by the dentist and by the patient. The dentist must check the state of the implants with an annual radiological control and any follow-up visits deemed necessary; while the patient must continue tending them at home following the instructions and using the tools provided by the dentist. There are three different, and equally important, phases in the follow-up and monito-ring of the patient’s dental care: during the osseointegration phase, after the prosthe-tic restoration and during the maintenance phase.

What is implant maintenance?Implant maintenance must be done both at the clinic by the practitioner and at home by the patient following the instructions and tools provided by the dentist. There are three different and equally important phases in the follow-up and monitoring of the pa-tient’s dental care: • During the osseointegration phase, when the implant is integrating into the bone. • After prosthetic restoration, when the substitute pieces are available.• During the maintenance phase.

A correct maintenance must be based on the following points: • Stipulated follow-up visits based on the oral hygiene of the patient. • Revision and assessment of the implants. • Revision and assessment of the prosthesis. • Mechanical oral hygiene in the dentist’s office. • Regular radiological monitoring using X-ray images. • Oral hygiene at the patient’s home.

How often must a patient go for follow-up visits?Nowadays there is no agreed standard for maintenance follow-up visits, it is advisable to visit the dentist every three or four months with an annual radiological control.

How must the oral hygiene be for a patient with implants at home? The patient’s proper oral hygiene must begin before the placement of implants in the mouth to obtain a better result. The dentist will prescribe a special toothpaste and give some guidelines to follow on a case by case basis.

FREQUENTLY ASKED QUESTIONS

... During the intervention the patient does not feel any pain ...

What happens if I am pregnant?

It is recommended to wait until the baby is born and the mother is fully recovered to start planning the implant restoration.

Does tobacco affect implants?

Yes, tobacco causes a decrease in the blood supply to the oral tissues, which diminishes the contribution of oxygen to that area, en-dangering the osseointegration of implants.

Will implants last my whole life?

If cared properly, the longevity of implants is normally always more than 20 years. It is never possible to ensure that an implant lasts for a lifetime, as it always depends on a good maintenance and oral hygiene, and the patient’s health condition.

What happens if the implant does not integrate?

Lack of osseointegration is a rare but exitsting possibility. In such cases, either a “rescue implant” - characterized by being thicker and longer - is placed or, after a suitable healing period, the process can be restarted with an implant of similar characteristics.

What happens if I suffer from periodontal disease, diabetis, take Sintrom, etc?

Patient candidates for implant rehabilitation must have optimal oral health conditions and that includes having the periodontal dis- ease under control. In addition, advances and techniques increasingly less invasive enable anticoagulated patients to use implants without any problem.

Are implants very expensive?

As any other long-lasting medical proce-dure, an implant treatment is relatively expensive. The patient must always be fully informed and understand the cost of the rehabilitation with implants.

Will people notice that I have implants?

No, since the objective of an implant pros-thesis is to create a harmonious and natural smile consistent with the facial features. The new teeth will look and feel like natural teeth.

Is implant placement painful?

No, thanks to the evolution of anaesthetics and the action of pain relievers and anti-im-flammatory medications used in surgery, a treatment with implants is usually painless.

Is there an age limit?

No, in daily clinical practice the number of patients of advanced age asking for implant rehabilitations has increased.

Yes, as long as the dentist considers that the patient meets the conditions for that procedure. This type of implants are known as post-extraction implants.

Can a dental implant be placed just after teeth are extracted?

No, immediataly after surgery a temporary prosthesis will occupy the space left by the missing tooth or teeth, so the patient is never outside the dental practice with a gap.

Will I be without teeth?

The main advantage of a treatment with implants is the possibility to restore missing teeth without damaging the remaining healthy teeth.Prosthesis are functionally more stable, effective and long-lasting, achieving a natu-ralness far removed from conventional treatments (bridges, partial prosthesis, ...).Dental implants stop bone loss and stimulate the growth of adjacent bone and soft tissue providing a great aesthetic result. They are manufactured in grade 4 titanium, a material highly compatible with the human body, in order to avoid a possible rejection by the patient.Dental implants help build up self-esteem as they greatly improve aesthetic appear- ance and speech, and help the patient to have a better quality of life.

TEAM WORKFor a successful treatment with dental implants it is important the collaboration of all team members:The dentist bores the responsability to coordinate the whole treatment, perform the surgery and decide the most appropriate type of prosthesis for each patient. The prosthodontist and the laboratory technician produce the prosthesis that will later be mounted on the implants. It is important to have a qualified auxiliary team, as well as a good dental hygienist, for further care and maintenance. It is also of the utmost importance the patient’s commitment and motivation to under-take the necessary steps before, during and after the implant treatment.

... It is impor-tant the colla-boration of all team members ...
