WHAT IS A DENTAL IMPLANT?A dental implant is a grade 4 titanium - material highly compatible with the human body - screw which is placed in the upper or lower maxilla with a minor surgical procedure.The implant replaces the natural root of a missing tooth, so once the implant has been osseointegra-ted it restores lost function and aesthetics. Thus, an anchor is obtained to provide stability, comfort and security to the future dental prosthesis, the visible part of a dentition.

After a complete examination to assess the health of a patient’s teeth, including radiodiagnosis tests to determine the quality of the bone, the dentist must provide the patient with all the necessary information and medical advice, and ascertain whether this is a good candidate for a dental implant treatment.

In case of an affirmative answer, the dentist will prepare a detailed treatment plan that will follow these steps:

TREATMENT STAGES

1st Preliminary examAfter an initial assessment, the dentist determines if the patient can undergo a treatment with implants and, should this be the case, explain what, how, where, when and how many implants will be placed. There are some medical conditions that may prevent, hinder or discourage oral restoration with implants, among which it is worth highlighting serious diseases affecting bone metabolism, malignanacies affecting the bone or high-dose radiotherapy. A high consumption of any kind of tobacco can also be detrimental for implant placement.

2nd SurgeryThe surgeon will insert the implants into the defined area with a relatively minor surgery and use anaesthesia to avoid any possible pain. Following the surgical protocol for implant placement, implants can be inserted in a safe and stable manner. While the insertion time for a single implant does not usually reguire a long time (approx. 15 min.), the specific case and the total number of implants to insert will determine the duration of the surgery. After surgery, the patient must follow the cleaning, hygiene and aftercare instructions provided by the medical team. After the appropriate time has passed, always established by the doctor and patient’s healing rate (usually 7 - 10 days), the sutures are removed.

3rd Rest and healing periodDuring the healing period, implants are firmly integrated into the bone, process knows as “osseoin-tegration”. Thanks to the intimate bone-to-implant contact, the implant will integrate itself into the bone and, with time, it will surround and cover the implant, slowly immobilizing it and providing the desired stability.The amount of time that practitioners allow an implant to heal before placing a restoration on it varies widely. This process lasts up to 6 months depending on the treatment, type and quality of the bone, usually 2 to 3 months for mandible and 4 to 5 months for upper maxilla but always at the discretion of the surgeon.Once the implant is integrated, the implant cover screw is removed with a simple surgery (lasting about 20 min.) and a healing abutment takes its place to shape and model the gums, and heal the tissues. After a healing period that usually does not last longer than 1 or 2 weeks, the surgical treatment is then considered finished.

4th ImpressionThe surgeon takes measures to develop a model that will be an exact replica of the patient’s mouth, and sends it to a dental laboratory so that the patient’s new teeth can be designed. While the laboratory produces the prosthesis, follow-up visits with the patient are scheduled to check the suitability of the new perma-nent teeth.

5th ProsthesisThe surgeon and the dental laboratory determine which pros-thesis is better suited to the needs of the patient, i.e., a prosthe-sis that will restore lost function and aesthetics in a manner as similar as possible to the patient’s natural teeth.Currently the introduction of new materials, such as zirconium (highly biocompatible, very hard material), and new technologies, such as CAD-CAM (computer design and manufacture of dental prosthesis), provide new possibilities and alternatives that produce even better aesthetic results.

IMPLANT MAINTENANCE AND HYGIENE

Implant maintenance and hygiene must be done both by the dentist and by the patient. The dentist must check the state of the implants with an annual radiological control and any follow-up visits deemed necessary; while the patient must continue tending them at home following the instructions and using the tools provided by the dentist. There are three different, and equally important, phases in the follow-up and monito-ring of the patient’s dental care: during the osseointegration phase, after the prosthe-tic restoration and during the maintenance phase.

What is implant maintenance?Implant maintenance must be done both at the clinic by the practitioner and at home by the patient following the instructions and tools provided by the dentist.There are three different and equally important phases in the follow-up and monitoring of the pa-tient’s dental care: • During the osseointegration phase, when the implant is integrating into the bone. • After prosthetic restoration, when the substitute pieces are available.• During the maintenance phase.

How often must a patient go for follow-up visits?Nowadays there is no agreed standard for maintenance follow-up visits, it is advisable to visit the dentist every three or four months with an annual radiological control.

What happens if I am pregnant?
