Service Description: The following information

discusses how to access and apply for free and

low cost health care through the Ability to Pay

(ATP) and Pre-Payment Program (PPP). Also,

attached is a list of the County Hospitals,

Comprehensive Health Centers, a list of the

county and private clinics, whether they take

Medi-Cal, ATP, or PPP, and the medical services

that they provide.

More information can be obtained through the

Inter Net www.peoplesguide.org. Which has a lot

of general information on services throughout

L.A. County. Information on our of state services

can be obtained through the web site for the

American Public Human Service Association at

www. aphsa.org.

In an emergency, it is a legal and ethical duty for

every clinic or hospital emergency room to provide

emergency care regardless of immigration status or

ability to pay. Many private hospitals plan ways to

avoid this obligation, but you can get urgent help by

asking persistently. Emergency rooms also must

arrange for translators if necessary.

Ability to Pay (ATP) and Free Care The County provides free or low-cost health services to anyone, including low-income people who are not eligible

for Medi-Cal or who have medical expenses that Medi-Cal, Medicare, and private insurance will not fully cover. One

plan is called the Prepayment/ATP (“Ability to Pay”) plan.

Ability to Pay (ATP) and Free Care – continued The county has also contracted with clinics called

“partners” who have to provide free care for you if you meet the ATP standards. If you are on GR you should automatically qualify. If you qualify for free care, you

cannot be forced to pay any fee by a county clinic or hospital. You should receive the same quality of care as

those who pay

How Do I Use Free Care of ATP? For clinic care, you must apply every six months. The screener will figure out what you must pay, if anything, for each visit. You can get free clinic care if your total net adjusted monthly income after deductions is below the following:

For hospital care, you must apply each time you are admitted to a county hospital. Your application may be used for a stay of up to 30 days. If you are hospitalized longer, you must complete another application. You can get free hospital care if your total net adjusted monthly income after deductions is below the following:

If your monthly income is higher than those shown on the tables, you will have to pay some money for services. The higher your income is, the more you will have to pay.

How Do I Apply? To apply for ATP, you must make an appointment for a financial screening at a county hospital or clinic that has an ATP worker. For free care at “partner” clinics, you see the screener at that clinic. (See County Health Care Facilities).

At those clinics, you can sign a simple form called a “Certificate of Indigency” if you qualify for free care, and it is good for 6 months. Sometimes you will be able to be seen the dame day, but may have to wait. Be sure to tell the worker if your need for care is urgent. You do not have to wait for your financial screening to get care. If you qualify for Medi-Cal, you will be asked to apply for benefits and provide proof that you are not eligible or are only eligible for restricted benefits before an ATP application will be taken. Be very firm about your right to apply for free care and be sure to ask for ATP at County or “partner” facilities. If it is your first visit to that hospital or clinic, they should give you a written notice regarding available plans, including ATP, to reduce the cost of your medical care. If you do not receive this notice at your first visit, ask for one. If the worker or the clinic screener you see tells you that you do not qualify for ATP or free care and you do not agree, ask to meet with the worker’s supervisor within 10 working days. If you were found ineligible once, you may reapply. If you are unable to keep your appointment, call the ATP worker immediately or you may be billed for the full cost of any treatment you have already received. Even if you do not qualify for ATP, you still have a chance to pay a low-cost fee for the services within 7 days.

Pre-Payment Plan If you don’t qualify for completely free care, you can still get low cost health care through the ATP-Prepayment plan. You do not have to prove your income, family size, or resources if you pay the following standard fees within seven days of treatment. Remember that, if you do not pay these fees, you will be billed for the amount:

What Else Should I Know? If you have any questions about your care, need referrals, or have a complaint, call the County Health Line: 1-800-427-8700. You can also call the Health Consumer Center: 1-800-896-3203, on Monday, Tuesday, Thursday or Friday between 9:00 am and 4:30 pm. If you get free clinic care, you receive free medicines. If you receive clinic care at low cost, you receive “critical medicines” at no extra cost. Non-citizens with restricted Medi-Cal coverage can apply for ATP to cover non-emergency care. Before using ATP, you must use any other medical benefits you have such as private insurance, or outpatient Medicare. ATP will cover your

deductible for private insurance but will not cover inpatient Medicare deductibles and Medi-Cal “share of cost”.

Transportation for medical appointments may be funded by Medi-Cal. Prior authorization is required. The hospital social worker should be able to provide assistance. Regional Center Service Coordinator can provide assistance with Regional Center transportation vendors. Local Dial-A-Rides are sources of transportation for those traveling within their city.
