THE HEALTHY LIVING PHARMACY (HLP) FRAMEWORK IS A TIERED
COMMISSIONING FRAMEWORK AIMED AT ACHIEVING CONSISTENT
DELIVERY OF A BROAD RANGE OF HIGH QUALITY SERVICES THROUGH COMMUNITY PHARMACIES TO MEET LOCAL NEED, IMPROVING THE HEALTH
AND WELLBEING OF THE LOCAL POPULATION AND HELPING TO REDUCE
HEALTH INEQUALITIES.
THE HLP FRAMEWORK IS UNDERPINNED BY THREE ENABLERS:
* WORKFORCE DEVELOPMENT – A SKILLED TEAM TO PRO-ACTIVELY
SUPPORT AND PROMOTE BEHAVIOUR CHANGE, IMPROVING HEALTH AND
WELLBEING;

OK, WHERE DO I START!?
➢ The following slides will give information about the roles of a HLP Champion. Please remember that at all times you should be supported by your HLP leader. If you do not feel you are receiving the backup you need contact us.
➢ You are a part of the Tees HLP family….You are not alone!
1.SIGNPOSTING
FOLDER……➢ Your Pharmacy should have a signposting file or
folder. It is your role as a champion to find it!
(it maybe hidden, covered with dust somewhere!!)
➢ Either make a new folder or use the one you
have make sure it is up-to date and includes
the following……
**Please note that pharmacies within large chains may provide and file or folder for
you to use. Do not duplicate work!
Out of hours services
NHS choices
Local community groups: eg. weight/exercise
Dementia cafes
Nearest EHC provider
Sex health team
Alcohol services
Mental health services
Other pharmacies & services in area

Many pharmacies within a large chain will have been provided with a HLP folder. Use your companies file of choice do not duplicate work! Contact us for details.
➢ All Pharmacies have received an arch lever file from Tees LPC, have a rummage around the shelves and see if you can find it!
➢ Don’t worry if not, simply contact us and we will supply the inserts for you.

3.PHARMOUTCOMES & DATA
COLLECTION….
➢ If you do not have a pharmoutcomes log in ask your line manager to request one from [email protected]
➢ We strongly recommend that you get into the habit of checking PO daily. It is an important communication tool.

You do not have to be HLP
champion to have a log
in….what about the rest of the
team?

The services section is
where you will see all
the live services your
pharmacy provides
and how to action
them.
Under HLP, there are
two sections for you to
complete.
HLP CAMPAIGNS
Each time you host a
campaign either for a day or a
month Remember to record it
on PO as your evidence.
Print a copy of your PO entry
before you press save to add
to your evidence folder along
with a photo or case study.
HLP DATA COLLECTION
➢ Your pharmacy must record a minimum of 20
interactions per month every month. It is your
role as champion to ensure this is done….that
does not mean you need to do it all!!
➢ Some pharmacies print the screen opposite 20
times and hand to different members of the
team, there are no rules to how you do it…..you
just need to do it in what ever way works best
for your team.
➢ Interactions are any conversation that you have
around a health topic (literally every interaction
over your counter can relate in some way!)
Eg cough & cold remedy/OTC product given
/advise given - can be marked as minor ailment
interaction.
Your leader needs to
complete the
Pharmoutcomes
Assessment, once
you are fulfilling Level
1, 2 or 3 criteria
please email Sandie
Hall for an
accreditation visit
and certificate.
A certificate lasts for 2
years providing you
continue to work to
the same level.
For your quality payment
you need to be working
at Level 1, but you can
work towards and
achieve Levels 2 & 3 if
you wish.
4. CASCADE
➢ Information will be directed to you via Pharmoutcomes, the closed Facebook page and Tees LPC website.
➢ Best Practice events are available every 8 weeks which is an opportunity to seek information, training, resources and networking amongst other HLP Champions in Tees. (see training/events tab on website)
➢ It is up to you as a champion to access this information and share with the rest of your team so that they to can help to promote public health messages.
5. NETWORK LOCALLY
➢ MAKE FRIENDS!
➢ FIND OUT WHAT IS GOING ON NEXT DOOR
➢ HELP THY NEIGHBOURS- THEY WILL RETURN THE FAVOUR!
➢ FIND OUT WHO ARE THE KEY PEOPLE IN YOUR COMMUNITY
➢ USE THE INFO FROM YOUR LOCAL HEALTH PROFILE-WHAT ARE YOUR LOCAL AUTHORITIES HEALTH PRIORITIES.
➢ TALK TO YOUR LOCAL PUBLIC HEALTH TEAMS
➢ ASK YOU PATIENTS WHAT THEY WOULD LIKE AS A CAMPAIGN
➢ SPEAK TO LOCAL AND NATIONAL CHARITIES
➢ WHO IS YOUR NEAREST HLP CHAMPION?……WORK TOGETHER!
